Fat grafting for whole breast reconstruction

Had a mastectomy with immediate reconstruction using LD flap and silicon implant 18 months ago. Have hated the implant since day 1 and it's only getting more and more uncomfortable.

PS1 suggested removing implant and doing a fat graft in one surgery, then 6 months or so later doing a second fat grafting procedure, nipple construction, and making the "good" one symmetrical. All fine and good but this is with a European public health system with a very long waiting list, so I'm now leaning towards paying out of pocket for a private plastic surgeon.

So I went to see PS2 who said he would do it in just one surgery: fat grafting, nipple construction (using skin from the genital area), and symmetrization of the other.

I'm not looking for large breasts at all, really just the minimum to have "something" is fine with me. And I am terrified of surgery so I like PS2's plan better. Plus I just want to get it all over with and never think about this subject again.

But is it really possible to reconstruct a (small) breast with just one session of fat grafting? I can't find a lot of info from people who do it that way. Have any of you ladies had this procedure?

    Maybe, if you have a LOT of fat to spare. But, I doubt one procedure will be enough. Ive had four and they still aren't great. BUT they did feeeel better after the first one. The problem is that not all the fat "sticks". At least 25% gets reabsorbed into the body. Then they have to go back on and fill and shape.
