    I've been on Tamoxifen (20mg/day - 150 pounds - 5'9") for 9 months now and haven't been able to dodge this side effect. In chemo proper I started to experience frequent resistant yeast infections and so when chemo ended and Tamoxifen began there wasn't really any change.. My onc has given me Diflucan and it seems to clear up the symptoms immediately.. but the effect is short lived.. despite my low sugar diet and every other effort I could possibly put forth to eliminate this side effect. I've searched and searched and can't really find any other cases like mine and so I'm here pleading here with all of you wise women for answers. Although the Diflucan does help it certainly isn't "curing" this.. and I'm not convinced that a fungal infection is absolutely to blame. This cracking that I get is right in the folds of the labia, minor and major, and often in the area of the anus itself. It's exceptionally slow to heal and regardless of how much lube I use they break open pretty much right away upon intercourse. I'm sorry if this is a little more graphic than many of you might like to read about but my onc is pretty much through with suggestions and has left this up to me to resolve. I'm seriously considering stopping the Tam if I can't get some sort of resolution. Please help!!

    Started Tomoxifen January 15, 2007.....prior to being put into menopause (age 51) and using Tomoxifen.....I never had any gyno issues in my entire life! I felt truely blessed! Now I have extremely red vaginal walls and labia with itching that occurs along both sides of labia. I also have experience skin cracking along the lower end of the tail bone......about 1 1/2" down into the crack of your behind (trying to be polite hereEmbarassed). So yes....I have experienced cracking and itching, constantly with me.

    You need to go to a gynecologist, this is not something your oncologist will know how to treat. I have a skin condition on my vuvla called lichen sclerosus and it sounds like it could be something like that. Its treatable, but you need to get to someone who knows the right treatment. It doesn't sound like a typical side effect of tamoxifen to me.,

    Cultures of the vagina/vulva area looking for common and unusual bacteria, viruses, and yeast may be wise given the non-resolution with Diflucan for presumed yeast in the setting of chronic low estrogen.



    Of course, topical estrogen is what the body is crying out for, and when the skin thins and fissures occur and bleed and are open to allow superinfection with bacteria, perhaps a short low dose course is best discussed with your gynecologist. Tamoxifen blocks estrogen receptors so that imo is a tad different from other hormonals and estrogen use.



    One idea, just an idea, you may try prior to the real estrogen, are phytoestrogens as in soy. When I first started here I wrote a post about Crisco, wondering if because it is a phytoestrogen, and not much if any gets absorbed systemically (unlike the topical or internal estrogen creams, rings, inserts, which result in low dose absorption), so questioned would it be a way to help keep the vulva vagina anal tissue moist and fissureless. I just don't know.



    No studies done on the latter. Studies have been done using real estrogen, and shortest time period, in lowest dose if necessary is mostly advised.

    It is more likely related to menopause rather than the tamoxifen.  Here's what more or less worked for me.  Talk to your gynecologist and onocologist first but I used small amounts of topical estrogen (about the size of your smallest fingernail), once or twice a week and a vaginal moisturizer and/or vitamin E once or twice daily.  It seems to me that someone, maybe Iodine recommended aquaphor daily.  Good luck.

    wow -- Im glad I came across this thread !!!  I have been having this same issue since late March -- 2008 ...  I have been on Tamoxifen since late July 2007.  after calling my primary doc - he indicated to use hydrocortisone 1% at morn and nite - for only 4 days - of course this would heal this.................   what I experience are little - 'paper cuts ' ( as in a red line, not a true open wound "cut" but a PAPER CUT look)  --- ouch  --- and these occur at the top of me on the labia near the clitoris area ---  OUCH  again -- and yes I have to say its more noticeable after sex ------- doesn't hurt during but the aftermath its noticeable for a few days -----

    But after having to use hydrocortisone after a few more occassions - I figured this isn't good in this delicate tissue area --- so called the ONCLOGIST to see if he would ' OK ' me getting a prescription from the primary doc (he said he would prescribe the following to me BUT I would need the onclogist's OK )  for  ESTRACE  -- topical estrogen cream......  been using a small amt of that like sushanna indicated -- I'm not sure if that has been relieving those  little  paper cuts -- as TENDER indicate as  ' FISSURES "  yes that is a better term !!!!  so at times I use the hydrocortisone to initially speed the healing of it BUT I also find the following product very helpful in keeping the area moist etc -  I got it from those typical vitamin stores  its called --- gosh what a simple name >>>  Personal Moisturizer with aloe and vitamin E  --- the maker of this is  EMERITA  if you pull up the website for EMERITA thay make other products as well ...hey  noticed on the bottle that this is a woman-owned company !!!  anyways the bottle read as follows: vaginal use as needed - water-based - unscented- flavorless and non-staining -- aloe Vera gel - Calendula Vit E and Ginseng creat a superior moisturizer that helps restore balance and comfort .................  I typically make sure I put some on esp b4 exercising  etc --  love the feel of it and it helps alot --- but also doesn't heal the fissure - just keeps the area silky wet and moisturized .............   but at time I do place a small dab of estrace in that area - to help restore some of that estrogen lost .....................     as TENDER mentioned  I have read on the  ' CRISCO " thread and haven't yet tried that -- but had tried the aquaphor that I still have since rads --- and I don't lik ethat greasy aftermath effect ---  I'll give crisco a try at bedtime ......

    Well I guess I didn't think this was related to my treatment and thought I was hurting myself when I was bathing and wiping. Go figure. ...another fun benefit!

    I had been experiencing this myself. After reading some threads here, I started using coconut oil every night to moisturize the area. I still get cracking on occasion, but not nearly as frequently as before.

    Shortly after I started on Tamoxifen I experienced a horrible yeast infection which is unusual for me. Pretty much all yeast infections I've had in the past were directly related to antibiotic use, so I didn't recognize for what it was until I was quite miserable-itchiness gave way to scratchy, burning, inflamed cracked, vulva, etc.

    I treat all yeast infections with a medicated tampon-smear coconut oil on the tip of a tampon and shake a couple drops of tea tree oil onto it (the coconut oil keeps the tampon from absorbing the tea tree oil). Relief is pretty fast-I use this method, changing the tampon every 6 hours or so for the first 24 hours then just inserting a medicated tampon at night for the next few days. You can also mix coconut oil and tea tree oil to apply externally to help relieve itching/burning but do not apply straight undiluted tea tree oil-it will sting too much. Diluted tea tree oil feels like a mentholated product-that kind of hot/cold sensation. I can't use Diflucan (makes me violently ill w vomiting and headache) and I hate the messiness of Monostat (and it doesn't really work well for me) so have used the tea tree oil several times in the past with a good response. Anyway, this yeast infeaction cleared up, but came back again about 10 days later, so I thought something is really out of balance (because having it come back isn't usual) and looked for more things I could do to prevent this since it looked like it could be an ongoing problem for me with Tamoxifen use.

    I treated again with the tea tree oil and kept using plain coconut oil both internally and externally daily which has also helped with sexual matters, cracking of the labia, etc. Coconut oil, unlike something like Crisco, has anti-bacterial properties. I upped my probiotic use, switched to all cotton panties, blow dry my pubic area after showering (honest, I think it helps, yeast love trapped moisture), go commando whenever I can (at home with cotton jogging pants/shorts Joe Boxer flannel pajama bottoms, sans undies). Air flow is important. These things seem to be working for me, knock on wood.

    Actually, underwear in general has not been my friend since being diagnosed with BC-bras don't fit right since I had the lumpectomy. I never would have thought I'd start going braless and commando at age 52! LOL.

    I'm not on tamoxifen, I'm on Al but I have the same problem. It's caused by the lack of estrogen. I met with a gyn last week and she recommended regular moisturisers such as Replens or natural alternatives: coconut oil (as previously mentioned), olive oil and vit e.

    I had terrible pain during intercourse caused by atrophy, for that I do regular kegel exercises, lubricants (silks) and an aesthetic cream called lidocaine, which numbs the area down there, so no pain. You can get it over the counter, no prescription required. The latter works wonders, in a recent trial it reduced pain by 90%.

    have been on tamoxifen 9 months..now have lichen sclerosis of vulva..had been using coconut oil to relieve burning and itching but gyn prescribing ectosone .1 % cream..says condition can cause scarring and actual covering of clitoris???very important to get proper diagnosis and treat locally to preserve vulgar tissues , not to mention sex life! Bloody cancer robs us of enough life! It's not getting that too! Good luck! Assume nothing!

    I've just been diagnosed with the paper-cut-type fissures after being on tamoxifen for 6 months. My gyn took biopsies and she doesn't know what it is. Have to wait 10 days. I would so appreciate any reassurance that those of you who are suffering with this have found relief. I just can't see my way out of this.

    janey, I've had these a couple of times now, and it was an atypical yeast infection both times. A couple of diflucan and things were at least 75%, with complete resolution of the fissures (which I could really not believe how painful they were, btw!!). I hope it's something straightforward for you too!
    Labelle, thanks for the pointers about the tea tree - I think I'm going to try this to avoid the diflucan in the future if I can!

    Thank you, floaton. Maybe it is a yeast infection. You've given me hope that it is. That would make staying on the tamoxifen easier. I just can't sleep thinking about it.

    Ask about taking Osphena for your symptoms. Helped me immensely. It's a selective estrogen response medicine and while it has not been tested with ER+ breast cancer patients, I had to do something or I would have had to quit the AIs.

    It does make sense that we might be more prone to yeast infections while taking Tamoxifen. Lots of women develop yeast infections when their hormones are in flux, like when they start taking birth control pills or during the first month of pregnancy. Tamoxifen definitely messes with our hormones.

    I started taking it again this week after a month off for a variety of things. Now I've got a big zit on my cheek (not normal at 53). I also broke out the first time I started taking Tamoxifen. Dang screwed up hormones! Hoping I don't get a repeat of the yeast infection too.

    Wicked stuff indeed. I am borderline menopausal thanks to groovy chemo at age 40 (six years ago and just went through estrogen tanking this past year). I definitely noticed more dryness, a pinching sensation. I have been using dilators and gynatrof (from a Canadian pharmacy, no estrogen, but hops, which is likely a plant estrogen of some sort). I should be more religious--even hit or miss they have been making an incredible difference. There is more info on this in the sexual matters section. All of it is interconnected, whether you are having sex or not. The urinary issues are potentially problematic, too.

    I will say I personally found Replens and coconut oil of no help at all. All Replens does, IMO, is coat things, which I suppose can be protective, but it's not getting to the root of the issue.

    I remember a really bad yeast infection for the first few months after starting tamoxifen. I stopped using soap down there and bought feminine hygiene gentle cleaners and the problem cleared up. I also had those painful fissures. I washed with that gentle soap for a while and my body must have gotten acclimated or whatever because it all went away and now I'm back to regular soap and haven't had any issues. Sex is still painful but I chalk that up to going through menopause while on tamoxifen. I am going to look into that lidocaine mentioned above tho and may have to invest in some lubricants

    I am going through what most of you are. I am frustrated and miserable. It seems as if I have been having membrane thinning vaginally, and also in the anus area. The little tears are painful. The itching on labia and clitoris does not feel good.

    I have been taking tamoxifen for about 3 months and was on femara and anastrazole prior to tamoxifen. I recently was diagnosed with a bladder infection, now a UTI. No intimacy for 2 years. Diagnosed HER2+++ stage 2. Had a lumpectomy on left with lymph node removal, radiation and chemo and then herceptin which caused a heart problem and osteoporosis.

    After all of this, I am tired and want to stop all medication because my life is non existent and as strong as I wanted to be and have been..I am now exhausted and emotionally very weak. Just writing about my feelings make me sad.

    I feel for you.

    As you will see from this site, many consider this to be a hormonal imbalance from being knocked into sudden menopause. Either way I found 'Unlimited's info above on 24 May to be really good. She mentions a gel called Personal Moisturizer with aloe and vitamin E --- the maker of this is EMERITA . I ordered it on their site and think it's great.

    ? Tsidi, you mentioned Herceptin caused osteoporosis? I was diagnosed with osteoporosis and they put me on tamoxifen. I've been on it 28 days, and I can't stand it. Between the hot flashes, and the intestinal pains, I'm just about ready to give up. It has been 13 months from hell since first being told of my diagnosis. I was only given information in bits and pieces and only just recently learned how high my Oncotype test results are!! I swear these doctors don't tell you the horror of the side effects, because most of us probably wouldn't even start with these horrific meds and treatments. Sorry just being honest. I feel like my life ended 13 months ago, and I certainly haven't gotten it back. I live in fear and horror of this of dreaded disease.

    I feel like it's true punishment having received this diagnosis, and I truly envy those that escape life without it. I have not ever been a jealous person, but I'm certainly jealous of those that don't suffer with cancer. It's the disease that keeps taking and taking and taking....

    I am offering this advice to anyone with tears in their labial skin with stinging and burning. I was using Ivory soap on the advice of my doctor but switched to handmade goats milk soap with NO preservative and that made a huge difference right away. This was after several years of misery. I also am using coconut oil as a daily moisturizer on my doctor's advice. It seemed a little weird but wow it really made me so much more comfortable. I am 68, 5 years postop with radiation on Tamoxifen. I never dreamed such simple solutions could bring such relief. Hope this helps!

    Hi Ladies. I am 47 and on Tamoxifen, also put into menopause as a result of treatment. I have experienced the insane itchy vagina for the last 3 months or so. It seemed to get especially bad after a second surgery to have my gallbladder removed where i was on 4 days of IV antibiotics. I also was on Penacillian for a skin rash infection from tamoxifen which as also contributed to what I thought was yeast or is it yeast? Who knows! All i know is i have an insanely itchy vagina inside and outside. At first i thought it was sex drive, now I know its not. Its so itchy I often cannot sleep. I try not to scratch it and I end up because I feel like I will go crazy. I have been on 6 rounds of monistat or canesteen and two rounds of diflucan. It has literally been affecting the quality of my life, happiness and sleep. I refuse to beat cancer only to stop my life due to an itchy vagaina. It is now moving to my rectum area as well. The other night i noticed I was red, swollen even. Nothing helped. If Monistat did, it came back. So here is what i am doing now to at least try to control symptoms until i figure it out.

    I bathe daily in bath with tea tree oil drops. It seems to immediately ease the itchiness and irritation and raw feeling. I keep the area very dry with clean towels. Sometimes the antifungal cream seems to help but it comes back fast. I also used Loratidine allergy medication 10mg. I take half in the day and half in the evening. This almost immediately takes the swelling and intense itchiness away for about 6 hours. So now I think it could be an allergic reaction or side effect. On top of this i have also now found that baby diaper with zinc spray is soothing the redness, rash and itchiness. I prayed about this and that was what i came up with, tried it and it is amazing. It is soooooo soothing and safe to use even around the vagina and rectum. The brand I got is ZINCOFAX spray. I also got baby wipes. When I go to the bathroom i use the wipes and then let it dry then smooth on some zincofax spray. It is immediate and long term relief of the burning and stinging from the cracks on labia and such. I couldn't wait to get back and post this as I am so grateful to find something that helps me. Also aveeno execma body wash in the baby section. It is so mild and not like a soap i think it has oatmeal.


    So I recommend, Loratidine allergy pills 10 mg, half in morn, half in eve. Zincofax spray on the area, baby wipes and aveeno baby wash for excema. Even with all this I still cannot go without the allergy pills. I also got off bread completely.

    For recurring yeast infection, my pharm told me to do two packages of monistat or canesten back to back for resistant yeast. Im going to try that too as I am desperate to get this fixed.

    hi, I have suffered from recurrent vaginitis from yeast, B.V., and even an overgrowth of group b strep my entire adult life, long before breast cancer. I wanted to share that I finally got rid of the recurrence of the yeast and b.v. infections by using boric acid suppositories (there is a good one with the brand name Ph.D. that can be found online). in my experience and from what I have read, the problem with pharmaceutical creams like monistat and clindamycin is that they wreak havoc on your vaginal flora and kill off all the good bacteria and don't address the underlying problem of your healthy vaginal ph balance that promotes the good bacteria. I often use this with acidophilus capsules used aa vaginal suppositories. Group B strep was a bit more challenging, but garlic cloves and allicin pills plus acidophilus suppositories tended to usually help. However make sure to check with your obgyn, especially if you have dryness and fissures because these remedies can be irritating. Also I always put coconut oil on suppositories before insertion which tends to prevent irritation in my case, and for its anti -inflammatory and antifungal properties.

    As for me, I started taking Tamoxifen in July and as usual have painful intercourse and lots of vaginal discharge (yuck) but my tests for yeast, b.v. , and group b strep (as well as everything else) come back negative, and so the obgyn diagnosed me yesterday with atropic vaginitis, which she says is common in menopause (which I am in) and tamoxifen doesn't help. She said the usual treatment is vaginal estrogen cream but given my diagnosis it's not safe. She suggested Rephresh (which I don't like because it contains parabens) and coconut oil externally. My problem is not really external though, it is internal, so I'm a bit frustrated having not such great options. Also because of all the discharge I wouldn't call it dryness. I may try the Emerita personal cream recommended above, but if anyone else has suggestions, I would be interested.
