Just diagnosed 04/03/16
Hello everyone. My name is Michelle. I am 44, a wife and mother of four boys. I was diagnosed with stage 2 ILC, and meet with breast surgeon next week. I am undecided as to single or double mastectomy. I have a mass of 3 cm in right breast, Beyond the obvious, what should I be asking the BS?

Sorry that you had to join the club that no one wants to be a member of but I promise you that you're in good company. You came to the right place. You will get tons of information from great women with the biggest hearts! The beginning is the worst but it moves like lightning speed. First of all, take someone to the appointment with you because you will never remember half of what the doctor says. Or bring a tape recorder. The first thing the doctor usually does is a breast MRI to determine if it has spread to the lymph nodes. Also, ask the doctor if you can take the oncotype test, which will tell you how likely it is the cancer will recurr (if you take hormone therapy). And, ask the doctor what he would advise his own daughter (uni or bilateral). I went with the uni after my doctor suggested it over the bilateral. You will get through this. Thanks so much for your response. It is so much information that I need to learn. I look forward to getting acquainted with you all! I know I am ER/PR positive and HER2 NEG......that is all I really know so far. I have only met with my oncologist once.....but lots of appointments coming up this week.

Something you may not be aware of, chemo doesn't always work so well with ILC. Oncotype DX test really worthwhile.

Sorry you had to join this group. My wife had a very similar diagnosis.
I assume you are in New Mexico?
When I saw that you were just diagnosed, my initial thought was to suggest seeking treatment at the University of Pittsburgh in Pennsylvania since they have the only research team in the US that is studying Lobular. They are conducting the first ever and only US-based ILC clinical trial (which I wrote about here). It's specifically for "newly diagnosed" patients like yourself who haven't had surgery yet. The 3-4 week clinical trial is designed to determine treatment response by testing a few different common breast cancer therapies to identify areas of Lobular drug resistance. Drug resistance is a bit of a challenge and ILC is a chronically understudied, poorly understood subtype that warrants different targeted treatment. The trial hopes to answer some fundamental questions, which are needed to change treatment guidelines that will ultimately improve survival for all ILC patients.

Just a little difference of opinion. You might but will not necessarily be offered an MRI unless you have dense breast tissue &/or your surgeon suspects more areas. You should also ask the surgeon whether (s)he specializes in breast cancer and if so, how many of each type of breast surgery (s)he has done and does. You should ask what grade and the hormone & HER2 status of your tumor are, based on your biopsy path report. (It, as well as size, could change in the post-op report). As to OncotypeDX, it's way too soon to ask. It needs to be done on tumor tissue removed from your surgery, because: that pathology is more definitive than initial biopsy tissue; you need to know your lymph node status, which is usually determined by sentinel-node biopsy, because it isn't ordered in women with 3 or more positive nodes (until recently, test-eligible women had to be node-negative); and generally limited to IDC (another version is for DCIS). ILC because of its shape and growth pattern tends to be bigger than IDC (so don't panic over the size), and it's tougher to get clean margins in ILC with a lumpectomy, which is why mastectomy might be recommended. You might also ask what your reconstruction options are--whether to see a plastic surgeon and if (s)he will be in the OR during your mastectomy or lumpectomy, and if for symmetry's sake your healthy breast will need any modification.

I saw my MO and Breast surgeon today, and they have ordered an MRI for next week. Also, in the next couple of weeks, a sentinel lobe dye test will be done to see if I have any nodes involved. They didn't find anything on the ultrasound and they can't feel anything with examination. I will keep you all posted.

I just joined this group my DCIS of three years ago has returned 3-30-16 Stage 0 but Grade 3 I thought my Lumpectomy and radiation was my cure before. Now I have to make decisions about what kind of surgery and reconstruction meeting with Dr. Next week. How long can I wait for mastectomy
