I am very confused about the latest development in my DCIS treatment, My story is in an earlier thread - I participated in a clinical trial for neoadjuvant treatment of DCIS with letrozole. My 4cm lesion was removed in a January lumpectomy. The pathology slides read during surgery were clear of DCIS, but the final read showed .5cm of residual DCIS. I was off letrozole for almost two months while the team determined next steps. My radiation was delayed due to slow wound healing, but I recently had a pre-radiation mammogram.

The shocker is this: the mammogram showed two areas of amorphous calcifications which are suspicious. The radiation oncologist notified the team, and all deferred to my surgeon who said a biopsy is not necessary at this time. I will proceed with radiation next week, and I will be taking letrozole for the next 4.5 years.

How could I have additional calcifications? Do I insist on a biopsy? The radiation oncologist says it is not clear whether these are new or old calcifications, but my December mammogram results did not indicate other calcifications beyond my diagnosed DCIS area.

Has anyone here had this type of finding? I would appreciate any insight.

I don't ever remember anyone doing a trial with an AI such as yours (although I was the person who took black cohosh for 19 days prior to lumpectomy and my DCIS was smaller than they thought from the films at pathology so not sure if it shrunk or not) so am not sure if anyone has had this experience. I personally would want a biopsy. Just my take on it. Your December mammo could have missed something and now a better picture of your breast showed these areas of calcifications? That most definitely has happened before. Mammograms miss things all the time unfortunately. Maybe a second opinion is in order at the very least?

I hope everything turns out ok. Keep us posted.
