A really long wait.......

More than 10 years ago I was dx'd with stage IIIC in my left breast. I opted to have both breasts removed and begin chemo right away (because I was young and had a one year old, my oncologist told me that he wanted to be very aggressive with his treatment). At the time of my mastectomy, the PS put in expanders. Within just a couple days we realized that the fluid in my drains was becoming cloudy and was increasing instead of decreasing. My chest began to feel hot and look red. I had an obvious infection. After days and days of antibiotics it was clear to all of us that the expanders would have to be removed. A month or so after stopping chemo, the expanders were put back in. The infection I developed this time was so bad I ended up in the hospital for a week. Again the expanders had to be removed. We all assumed that I would try again after waiting 6 months or so. That never happened (life gets in the way sometimes). Now here we are and I'm trying again. The thing is I have quite a bit of scar tissue, not just from putting in and removing expanders but also from radiation and because I went back later and had some skin removed from the area (it was rubbing terribly and causing significant pain). My new PS has said that he will use alloderm to help with the scar tissue and that he would like to put in allergan gummy bear implants after we've expanded.
I go in to see the PS tomorrow for pre-op exam (my surgery will be next week) and I have written down some questions I have but I hope you can perhaps give me any others you think I should ask. Though it has been 13 years, I haven't been thinking about this until now. Literally, I mentioned the possibility of doing this and within a week or so I was scheduled for surgery. Here are the questions I have (and maybe you might have the answers)
1. Will my muscles need to be cut and will the expanders / implants go over or under the muscle? I really don't remember what they did last time, I was kind of in shock from the whole thing.
2. Will the alloderm be used when the expanders are put in or the implants or both?
3. Does the alloderm come from donors or from me?
4. How many CC's will be put in at the time of insert of expanders? Will it likely be very little due to scar tissue??
5. After removing the drains (which I really hope we can actually accomplish this time) what are my restrictions? Will I have to wear a bra? Can I exercise (I like to take 2 hour morning speed walks / jogs)?
6. What about height / projection, moderate / full? Not sure I truly understand all this
7. How will weight loss affect expanders / implants? I will definitely be losing weight and potentially (hopefully) a lot of it.
Thank you all for your assistance with this. I was feeling fine until the last day or so. Now I've begun to get a bit apprehensive.

Congratulations on your ten year mark! So sorry the previous attempts at reconstruction failed. I hope third time is the charm.
They do not cut the muscles, expander goes under the muscle. As far as I know The brand name alloderm comes from human cadavers. I thought there was a pig skin option too. Usually the surgeon makes the pocket at the time expanders go in but I am not sure. My drains came out after one week but I had to wait to go back to gym. Walking is fine at any point, might have to hold off jogging until your PS approves. I did not have to wear any bra, but some women have been bandaged or special bras, every surgeon is different. The size and type of implants depends how large you have the expanders filled and your anatomy. You will not have any fat in your reconstructed breasts if you choose implants, so their size will remain unchanged through weightloss.
I think it's cool that you are doing reconstruction even though so many years have passed. Good luck!

Congrats on the 10 year mark, and on your upcoming reconstruction! A lot has changed in the last 13 years, so it's good that you're going in armed with your questions and concerns. The good thing is, lots of advances have been made in the field, so fingers crossed you won't run into the same issues you had when you first attempted it.
You'll get lots of great responses from our community on this subject, so we just wanted to again say welcome, and wish you luck on your appointment! Keep us posted on how everything goes!

I just saw my surgeon and he did tell me that
1. The expanders / implants will go under the muscle
2. Alloderm will be used with the expanders
3. It is donated via cadavers
4. He will attempt to put at least 450 cc upon insertion of the expanders
5. There is no need to wear a bra with the expanders (after removal of the chest wrap). I don't have to wear one after the exchange, however he does recommend that I do. I can walk as much as I want as soon as I feel up to it. I can even lift weights if I'd like. But I am not to do anything that will bounce or strain my chest area (such as running / jogging). No chest exercises at all until at least 2 months or so after surgery.
6. He didn't go too much into height / projection and all that. He basically said that this is really going to depend greatly on how much stretch we get with the expanders. He'd like to wait to see before making any decisions about that.
7. Weight loss will affect the implants some due to him wanting to use fat grafting with the implants.
He didn't measure too much like I thought he would. He just measured my width across the areas where the expanders would be inserted. Said that one side was 22 and the other 20. He casually mentioned that I was "wide" and would need a wide expander. Any thoughts on this?? What does that mean for me when it comes down to what these are going to look like when all is said and done?
After the appt with him I had to stop by the hospital for pre-op info. The nurse there went on and on about this surgeon. Talking about how wonderful he really is. Said that she saw one of his 70 year old patients who now looked like she had the breasts of a 17 year old. Well if I can have that .......

Ok just 3 more days until my surgery. I'm starting to feel a little sick to my stomach (nerves I'm sure). I just do not want to go through what I went through before. All the infections and having to be admitted to the hospital (being away from my son was agony, he was barely one year old at the time).
I have another concern and I won't get to see the ps again until the morning of surgery. He mentioned that we could pick between round and anatomical after the expanders were put in and we see how well I'm stretching. I've been reading a lot on this board and I'm sure that I read someone say that this decision must be made BEFORE putting in the expanders as a different type of expander will be used if we choose anatomical. Plus it can affect the alloderm as well which he will be using.
This surgery is not going to be inexpensive for us, though we have insurance our deductible is fairly large as well as our co-pay. I just worry about things like rippling and having to do this yet again. I think this may be my one shot and I need to get it right (which I know is impossible to predict).

The information on Allergan 410 anatomicals say that the implants should be 0.5 cm to 1 cm wider than the expanders. The anatomicals need a snug fit so that they don't rotate. I asked my PS about this and he wasn't in complete agreement. I think they like to keep the options open as the process unfolds. For me, I feel as though my previous PS used expanders that were too wide for my body- we never discussed sizing like I wish we had, but I was facing a cancer dx and just wanted to move things along. If I could go back in time, I would drill him with questions about sizing. Good luck.
