Radial Scar/ Complex Sclerosing Lesion
First things - I have searched the forum for other threads about radial scars. Since most of the responses are over 6 months old, I wanted to throw the topic back into the mix again. I've got a 2cm radial scar, making it a complex sclerosing lesion. I had a biopsy a couple of weeks ago, where the samples were b9. My pre-surgery appointment is in 2 weeks with the surgery approximately 2 weeks from then. I know it's a good sign that they're not pushing a faster pace, but...I wanted to ask if any members here had radial scars, and what your outcomes were. I've done my due diligence of research with medical journals. The one question I can't find an answer to is whether the size of the radial scar is correlative to a malignant outcome or not. Any shared experience would be so welcome! Thanks!

I came up with the exact same thing one month ago. Did they see calcifications on the Mamogram? I had biopsies and mine turned out to be ADH. I hope for better for you! Keep us posted

They saw some calcifications on the diagnostic mammo & on the u/s. Thanks for the kind words, Ddw79. I am keeping my fingers crossed, but am realistic with the possible outcomes as well. I'll have a better idea of what to expect with the surgery after my appointment in two weeks, including how much they're thinking of taking out. The radiologist suggested they would excise the lesion as well as a margin around it because of things like DCIS & LCIS hiding in the area. It's all a waiting game. Would love to know what plan they have for you, Ddw!

sounds so much like me Bella. They told me the same thing. Diagnostic Mammogram was birads 4. In my case the radia scar and ADH kind of came together and the Microcalcifications pointed to a problem. I had the excisional biopsy within one week of the Sterotactic. Like you they said the same, looking for DCIS or other cancer. There may be no margin to excise Bella. They will take out the adenosis and look to see if there is anything lurking . My guess from my case which sounds pretty similar, is AdH or ALH if anything and let's hope you get an all clear

We are just formulating a plan now. I first saw the MO yesterday and now I have a second opinion coming up in June .

Mine showed complex sclerosing lesion, DCIS and ADH, from two different regions of the biopsied breast. This was back in March, which led to BMX for me last week. IT started with calcifications on the mammograms. Once pathology examined the other side from the MX, it showed ALH, which wasn't caught on mammograms, ultrasounds or MRIs.

Lusciousleaves - thanks for your information. I'm so sorry for your diagnosis and what you've already gone through. What I'm taking from your note is that I can't try to "control" my situation by overanalyzing any of the information I'm finding or from my own reports. We can't know what's actually there until they finish doing their exploration and evaluations. For what it's worth, that gives me a little bit of peace. Thank you! May you have continued healing.

Glad I could help, Bella. I'm not sorry for doing the BMX. I'm grateful this was caught so early allowing me to pursue this path, and that the final reports came back with only one other precancerous lesion. Thank you for the kind words and thoughts of healing. What type of surgery is planned for you?

At this point, I'm still waiting for my excision surgery to take out the 2cm lesion. I do have my pathology report from my core needle biopsy. The report only says that the samples were p63 & ck5/6 positive - both of those most likely mean no DCIS (but of course, only in those samples) because DCIS is almost always ck5/6 negative. Those stains helped them differentiate that it was a complex sclerosing lesion rather than a malignant tumor. I'll know more after talking to the surgeon on the 12th. And...I know that trying to interpret the pathology report is pretty useless for me to do since I'm not the trained doctor. I'm really trying to let go, but the long waits in between appointments are starting to get to me a bit.

do you know if a sclerosing lesion is treated differently than just adenosis? my stereotactic biopsy was done and the path report showed "focal area of fibrocystic changes including adenosis". with dystrophic calcification and no evidence of atypia or malignancy
I met with a general surgeon who told me I don't have DCIS after reviewing the path report (which is why I had the stereotactic biopsy with birads 4 and micro calcifications incidentally in a linear dot/dash pattern suggestive of DCIS)
I am questioning whether I need to get a second opinion by an actual breast specialist and perhaps at a breast centre like at Princess Margaret in Toronto and perhaps a whole lot was missed by my biopsy. (took 5 samples).
not feeling very reassured. my initial mag. view diagnostic mammogram suggested sclerosing adenosis. Can't help but think something is either lurking in the area or may end up being a breast ca picture in my future

You can always get a second read on the path report. I had radial scar/ sclerosing ad enosis ANd Atypia. I would be thrilled not to have the latter so I think you are good but I understand about not trusting i

Hi dolfin - At this point, I just have the general diagnosis of the radial scar/CSL based on the biopsy pathology. They won't give me more details like adenosis and/or atypia until after I have the surgical excision. At this point, the typical treatment for a CSL is excision. But knowing other designations like adenosis won't be given to me for like another month. And like Ddw79 said, you can always get a second opinion.

*sigh* I have no idea why I didn't read my initial screening mammo report before. I thought they hadn't seen any micro calcifications until the diagnostic mammo & u/s. Turns out, they saw them in the first mammo as well as the lesion, which started this journey. In terms of association with carcinoma & RS/CSL, there's a higher association between the two if one or both were viewed in the mammo. Considering I have "dense & busy breasts", I have to say I feel a bit thankful that they saw them. However, I know it increases my chances for them finding something more. Perhaps I can break the percentages!

Hi, I did have a CSL 6 months ago (around 2 cm) that was removed and biopsy came back negative. Hopefully you will be fine as well. As you I was worried before my surgery and I did a lot of research to be prepared just in case .. waiting is the worst part. My doctor always told me although there was a chance of hidden DCIS it was really low and she was doing the procedure just to be on the safe side. Good luck!

Met with the BS this week, and was lucky to snag a surgery slot in two weeks due to a cancellation! When I asked the BS how much they were going to take out, he said between a golf ball and racquetball-sized section. I'm interested to see how that will change the shape of my breast if it does at all. Anyway, so glad that there's only a little more waiting!

Had my surgery this week, and the results came back quickly (thanks to my BS making sure due to our overseas military move being held up because of this). Benign!!!!
