Do MRI results correlate with final node biopsy?
Hi all, Just diagnosed last week and had MRI today. Good news! No new tumors found and she said my lymph nodes 'look good' Did anyone have a negative MRI then a surprise with the final SNB result. Scheduled for a lumpectomy 5/15.I know there is know way to truly know until all is done but I was just curious how they correlate. I did see one study that looked promising for correlation.

I had excellent care. My breast surgeon is a breast cancer survivor and head of the Caldwell Breast Center at Lutheran General. Unfortunately my MRI results (node wise) did not correlate. I ended up with 7/19 nodes when there was every reason to expect no positive nodes. My nodes were unusually tiny. The MRI did reveal another tumor spot which was not seen on mastectomy or ultrasound. Fortunately final pathology showed this was not all one large tumor but smaller areas. Nevertheless - stage iiia.
I'm pretty much the exception to the rule. You will most likely be the rule :-) that's what I told my roommate from college in January and that's what she was - the rule!!!

My MRI did show a satellite tumor that did not show on mammo very close to the 4.5cm mass, however my lymph nodes showed no sign of CA. A BX was positive pre surgery and that node did not uptake the blue dye so during surgery, it was left behind. 6 mo later I returned to that surgeon (who did not do my surgery) and asked her to check to see if the node was still there. She found it via us so I had to have another surgery to remove it.
I decided I don't want a diag. mammo anymore and think an MRI is more appropriate in my case but my onc said too many false positives with MRI so she will not RX it. I guess all the false negatives I got with the mammo is better? I don't get it.

I had false negatives with mammo, MRI, and previously on pelvic US. Mammo did not see my palpable 2cm tumor, MRI did not see the two positive nodes, and the pelvic US did not the 3cm pre-malignant ovarian mass. Surprise! I do not image well and this is what tipped me toward a BMX - I had ADH and ALH in the prophy breast on post-op pathology - so correct decision even in light of the reconstructive nightmare that followed.

Thanks SpecialK. The BS said the MRI gave him more info and helped with staging. I don't understand how you can stage without final BX from SNB. I read all my prior rad reports going back to 2007 and all said "heterogeneously dense" I asked the Rad in Boston yesterday who read my MRI and she said she didn't think they were dense at all. go figure. Sorry you had such an ordeal!

I assume it is helping him with "clinical staging" if he is not seeing glaringly obvious nodal activity, but your stage will not be definitive until your SNB is looked at in the lab. Mine was negative in the OR, positive in the lab, I had a ALND surgery five weeks later and they found a much larger positive node .5cm, further up. I seem to always be the outlier, lol! My issues are related to recon at this point, but I regard it as a nuisance because I am NED, so I can deal and be patient. In regards to density, it is disconcerting that different docs looking at the same imaging draw different conclusions - makes it hard to be the patient, right?

I had the opposite happen in that I had 2 lymph nodes that lit up on the MRI and were suspicious for malignancy. But final path after surgery showed no lymph involvement.

Yes relieved! I also find it interesting reading others stories, it shows how much bc and everything involved (testing etc) varies so much for each individual.

I had an excisional biopsy on 12/17/14; MRI on 12/31/14. MRI showed enlarged lymph nodes so breast surgeon added SLNB to my reexcision surgery. I was so scared even though they told me enlargement is common after surgery. Pathology came back negative from the lymph node biopsy. The MRI tech said less than 10% come back positive if MRI shows nodes look good. Don't know if that is true or not. Didn't really want to have the lymph node surgery after reading about it but glad I did as it at least put those fears to rest. Here's to speedy recovery after your lumpectomy.

My MRI didn't show any signs of lymph node activity either. I had a lumpectomy and sentinel node biopsy today. I'll keep you posted on the results.

At diagnosis, I had three lymph nodes that were kind of clumped together and surgeon said they looked "matted". After chemo, they looked normal on the MRI (i.e. were back to normal size and no longer matted together). However, in surgery on Friday, the pathologist said they looked suspicious. I don't have the final path report yet but it does sound like they may have still had some cancer going on.
Edited to add: During surgery, they actually removed 10 nodes and 5 were positive.

Mamogram didn't catch my 1.3 cm palpable tumor most likely due to I have dense breasts. Screening MRI caught it and saw no abnormalities in the lymph nodes. Had a lumpectomy and sentinel node biopsy on Friday and got the pathology results today. Tumor was actually 1.5 cm in size with 0/3 lymph nodes showing any Cancer.

In my case, MRI done a week before surgery did not show anything in the lymph nodes. Pathology results from surgery were very different - see below.

Muska, that is a difference! Just curious, was your post op pathology grade different than your biopsy grade. ???
