just got my denial letter for fat grafting

I fought like heck to get medicaid, the breast cancer, cervical cancer program and so far have only had one issue with them not wanting to pay, oddly my first visit with a family Dr after my BC diagnosis. Today I received a letter in the mail saying my Dr's request for fat grafting to finish my DEIP flap is denied. They recommend implants and said that fat grafting would be more costly. I have a new lump on the edge of the new flap where the cancer was and a large lump on my sternum that has to be biopsied. Rather than go through the pain of the core needle biopsy my DR's agreed to do the biopsies during my 2nd stage of DEIP flap which is scheduled for April 15th. Now the denial..... Has anyone had any experience with Medicaid and the appeal process? Any advice for me?
