    Just found out today that my surgery is scheduled for Wednesday, March 25th. I was treated for 20 weeks of ACT neoadjuvent chemotherapy for triple negative, grade 3, IIIC breast cancer which ended on February 24. I decided on a BMX with reconstruction expanders at the time of surgery. When surgery and expanders are complete I will begin 5 weeks of radiation. After, the normal waiting period after radiation I will have a DIEP flap.

    Thanks for the great suggestion on the anti-nausea drug.

    thanks Divecat. I am not looking forward to it but I know it will be over soon too. I'm preparing better than I did for my first surgery. Dulclax, prunes, Netflix. LOL. Does anyone know the best way to sleep after SNB. I'm a side sleeper and SX will be on left side. My boobs still hurt from original biopsies so sleeping hasn't been great to begin with. My surgeon told me to use hot compresses and massage them. It hasn't helped.

    AudreyB,

    I imagine you would need to be on your back or other side; I would definitely ask your BS as they may have some recommendations for you.

    I did not have SNB but before my BMX/immediate implants I was a dedicated stomach sleeper, with a small amount of side sleeping. Never my back....could not sleep that way in all my 34 years (at that time). After surgery, I had no choice though...so I made a pillow nest with many pillows, and a Snoogle body pillow, and found I could sleep very well that way. I no longer use the pillow nest, but I now regularly go to sleep on my back...go figure. My cat likes to come sleep on my chest most of night though now....so I hope I don't have any unpleasant wake ups in the middle of the night this time around by her snuggling up on my chest

    Hi Audrey, I too am a side sleeper and the last few months have been torture. I'm finally comfortably on my side again, just in time for more surgery. Ack! I didn't think the SNB had very much impact on the pain but I also had BMX at the same time and everything was very numb. My SNB armpit is STILL numb. Hopefully it won't be too uncomfortable for you.

    Thanks for the advice. If I had opted for mastectomy I believe the nodes would have been removed during same surgery but now I have the joy of an underarm incision. I am just so over all the stress of waiting. Waiting for surgery and the effects, waiting for results. My nerves are shot. I just took a Xanax.

    I had my SNB at time of BNX and they still went through underarm incision! That one hurt more and healed slower than Breast incision! So never kno

    Oh, I'm sorry about that Robin! I thought SNB during mastectomy was different. I had lumpectomy in each breast 2/6. I have to say while a very sore procedure (I'm still in a lot of discomfort due to surgeon taking out a lot of tissue, her words) I am even more afraid of the pain from SNB and all the extra complications. I hope I'm lucky and have an easy recovery but that's not how things happen for me.

    One day to go...do you know what time your surgery is at yet?

    Like last time, I won't know until 1:30-3:30 the day before,which is not handy as it is a 2 1/2 hour drive away so I don't know yet if we will need to be up driving at 3:00 am, or can wait until 7:00 am!

    How are you feeling today? I hope you have some time to relax today....and I wish you a smooth and uneventful surgery tomorrow.

    I forgot to ask, and maybe you mentioned it but I missed it.....since you had the LX, and I assume with good margins, how do they know which nodes to take for the SNB? I ask mostly because for those of us who are high risk and have prophys, node removal - even SNB - is NOT standard, and not preferred obviously, but there is a risk of pathology coming back with an undetected cancer and so sometimes at the PBMX women try and preserve ability to get SNB later still if needed....some get clips, but there are some attempts by some surgeons now to use dye, which hopefully lasts. This is what Angelina Jolie's surgeon did. I didn't have anything like this done, and my pathology had a few changes but no cancer fortunately, but unfortunately have seen a few BRCA+ women end up with prophylactic mastectomies that turned out not to be so prophylactic. Anyway, I have seen this concern raised in the high risk community so was just curious. Some women push to get SNBs anyway at time of MX, but obviously there are risks with that.

    Surgery in 1 week. I am on a cleaning spree....even went so far as to vacuum under the refrigerator. It feels awesome! Trying not to think about how everything will be a mess again in a few days.

    Thank you DiveCat and Angemonm41.  Surgery is scheduled for 1pm tomorrow, but I'm waiting for call from the hospital this afternoon to confirm that.

    Dive Cat, My surgeon got clean margins from lumpectomy, MRI showed no other tumors in left or right breast, and nodes looked "clean" in MRI.  At the time of my lumpectomy, I was not diagnosed, excisional biopsy turned into lumpectomy on both sides during that procedure.  However, nodes were not taken at that time as there was no cancer diagnosis. Hence, a 2nd surgery for me now.

    Surgeon said Sentinal Node Biopsy left side.  I didn't question it.  She will use dye, injected after I am under general anasthesia. I am hoping nodes are as clean as she thinks they are and that I don't need chemo.  I will be starting radiation soon I would guess, and tamofixan or some other meds.  She wants me on it 10 years.

    If cancer returns to left breast, I will get a BMX, if it develops in right breast, I might try radiation first, then BMX if it reoccurs in either breast.

    I am not sure if I am making the right decision, but my sister had the same cancer 5 years ago, (IDC), did lumpectomy, chemo and radiation.  This year she is a 5 year survivor.  I go to the Survivor Tent with her every year to get her survivor medal at the Susan G. Koman walk at Jones Beach.  We always have a good cry.  This year, I will be getting my own medal.  I can only imagine the tears that will flow.

    I was on board for the BMX, even went recliner shopping, until the MRI results came in last week.  I think the surgeon was surprised there were no other tumors present and she told me I could go this route and would most likely have a good outcome. I am praying she is right.

    One of my goals over the next two weeks is to teach my mom some tech savvy skills so she can get online too. In the meantime. My mom is scheduled for uni MX on March 9th with reconstruction. Some how she got the PS to agree to do reconstruction with a one step procedure. I wasn't even aware that was an option. I am headed down south this weekend and plan to stay two weeks. Hope there are no surprises for us along this journey.

    AudreyB, good luck tomorrow!!  Is this an out-patient procedure?  Let us know how you did, when you can.  We'll be thinking about you and praying for clear nodes.

    Jade645 - my surgery of April 24, 2014 was a double mastectomy with direct to implant reconstruction. No expanders :) I am still going for a revision, so a "one step" is not always "one and done" but I have no regrets about having it and honestly, a revision is totally just for small cosmetic stuff. Not everyone is a candidate for it, and not all PSs are experienced in it, so it is still not as common as TEs!

    Please PM me if you/your mom have any questions about that particular surgery - such as what to expect after, recovery, etc.

    Thanks hummingbird lover. I'm sitting here waiting for the hospital to call with arrival time. I will be outpatient. Same hospital where lumpectomies were performed so I will be well taken care of. Even though outpatient upon arrival I'm given a real hospital room complete with bed and tv. After surgery back to that room to recover and even get fed. Very professional nursing staff. All in all a very comfortable experience considering why I'm there

    Wow - its been a very long time since I have been to this site.. I had Breast Cancer in 2005 at age 40. For the past 9 years I have been cancer free. October 2014 my MRI showed an area of concern in my other breast. Turned out to be ADH but required a lumpectomy. I am now choosing to have bilateral Mastectomies with tram flap reconstruction. My surgery date is March 16th.. I am getting quite anxious - the reason I am here looking for information.. Good luck to all of you headed into surgery!

    Winding shores....please please ask for Emend! It truly saved me as I had post anesthesia problems in the past. It's one pill you take with a sip of water two hours prior to surgery.

    I met with my surgical oncologist on Friday and plastic surgeon today. I had great response to chemotherapy and my tumor has shrunk from nearly 4 cm to 8mm.

    The docs wanted me to consider lumpectomy but I just couldn't come to peace with anything other than bilateral mastectomy.

    I don't have a date yet but at least I think I have a plan! I will have BMX with Lat Flap and tissue expanded. That will be followed by fills. Then radiation. Then six months of waiting before exchange to implants. Long haul...

    Patty101014, wow, that's so awesome that your tumor shrunk so much!! I don't blame you for choosing BMX, I did too, although the Lx was not an option for me. I am just now starting reconstruction and told my husband the oher night that by the time I'm done, barring complications, it will be an entire year of my life. There are several ladies in the March group who have already endured chemo. ((Hugs to all of you))

    Jilly, will the hospital have Emend? Thanks for the suggestion. I am also going to ask about a paravertebral block.

    i have mixed feelings about my planned surgery this Friday. The surgeon called me today because she had heard I had questions and said if I wanted a bilateral we would have to change the date. So I am going to do the unilateral and try to get the other breast done at another time.

    I was all set for a bilateral- that's what I always thought I would do- but am in my 60's now with an autoimmune disorder, sensitivities to meds, and some neuro issues too. When I met with the surgeon the first time, she was clearly nervous about the double procedure and told me it would be extra time under anesthesia and more stressful healing for me physically. She recommended a single mastectomy and deciding later on the other.

    However, if I called and said, "Look I want a bilateral no matter what" she would do that.

    By asking her questions and deferring to her experience, I am making her responsible for the choice, which means liability if the bilateral goes wrong. On the other hand, I have never had surgery and never experienced a mastectomy and don't know how my body will react, so it would seem reasonable to want a medical opinion. I am not sure how to reconcile our different agendas.

    I think my surgeon is pushing unilateral to avoid legal problems from suggesting a bilateral in case of infection etc. Again, if I demanded it, she would not be liable and would have agreed.

    I really don't like this situation and feel that I should have been able to have a different discussion.

    I guess I would be fine with doing one at a time but my left breast (like my right, where the cancer is) has been painful for about 6 weeks now (well before the lump was found), with nipple sensitivity and burning pain. The mammogram was clear but I don't feel certain at all that it is okay.

    This site has been helpful and is appreciated. I am all ready to go regardless. Bought some stuff to make a comfy nest for rest and have rides lined up and laundry is done!

    Congrats on such a great response to chemo! Even if you are still opting for a BMX over the LX, those results must have been great to hear. Let me know when you have a date and I will add it in. I will add your surgery details in now anyway :)

    You are just having a MX correct, no reconstruction? Given your other medical issues, I do think your surgeon is being cautious, but that is not a bad thing. These days surgeons do strive more for breast conservation in the first place, and if you are at a higher risk of infection and other medical issues, including neuro issues, it is no wonder she might be nervous about it. Anyway, recovery with a MX/no recon is generally a lot smoother going than with recon, so I don't think you would need to wait TOO long to have a second MX if you decided that was still the best choice for you. If you really, really want the BMX though, then please do not hesitate to contact her and say you want to go ahead with the BMX, even if it means rescheduling the date.

    Hi everyone. Home from the hospital. Feeling very loopy from Percocet they gave me for the ride home. Not in much pain but did not let Percocet. Wear off either. Just took a tamadrol.

    Windyshore...ask your doctor for the Emend prescription. Get it filled and take two hours before surgery with a sip of water but run all this by PS. It's crazy because it's only one pill and my co pay was twenty dollars but it was worth it!

    As far as having bilateral versus just one, go with your instincts, and of course the feedback from your physician.

    You will do great!
