DIEP 2015

New to this group. Saw my new "salvage" plastic surgeon today. My previous doc did a lat flap and due to post op infections it failed. So he referred me on to someone else. Scheduling my DIEP for either May or June.
-
Morwenna, I am so sorry to hear about your problems. I'm thinking of you and sending hugs. Xox
-
Morwenna, I'm also sorry to hear about your clots. Every decision we have to make is so complex. Thinking of you and wishing "thin blood" thoughts!
I'm nine days out from a unilateral DIEP. Surgery was last Monday, took just over eight hours. Went home on Thursday. The surgeon did a stacked DIEP where he used both sides of the stomach to create one breast. I have a larger frame and I didn't want to got a lot smaller, am currently a D on the natural side and would like to finish up around a C/D. I'm a little suprised by how big the reconstruction side is right now, but know that the second surgeries will be tidy things up. Surgery was at Toronto General, nurses and the ward were great.
I still have both abdominal drains in - hoping that one will come out over the weekend. Based on my MX I know that I tend to keep the drains in a bit longer than others, and also that it's really not good to take them out early so I appreciate the surgeon's concern of keeping them in until they are under 20mL. Pain is okay, as of yesterday I went down to only Tylenol. The nausea was getting worse on the Oxycondone and I'd rather have a bit of pain than constant stomach aches and nausea.
One thing that came up on the pre-surgery CT scan is that I currently have a broken rib. Surgeon said if they had know that before they would have postponed the surgery. It was all a rush because of me taking a cancellation spot. I had a rushed bone scan and nothing else showed up so I'm trying to re-assure myself that a single broken rib isn't a lot to worry about. It's below my radiation field and I just assumed it was ongoing costochondritis that I had acutely last January and never really went away. I guess I'll find out more in the next few weeks. Anyone else have a broken rib that turned out to be nothing? It does make me slightly nervous to be off the Tamoxifen though...
-
Just heard from the Ambulatory surgery nurse. My surgery tomorrow is scheduled for 0730, so I must be there at 0545. Fortunately, at that time of the day it is only a 30-45 minute drive. I'm more scared of having to stay in the hospital than the surgery itself. When I had my BMX I not only went home the same day, but walked out the center to my vehicle.
-
Morwenna -- sorry to hear about the clots -- that's a big bump in the road. I'm hoping you are feeling better.Five_cents -- I hope your broken rib nothing serious - glad your bone scan didn't show anything. I had my surgery at Toronto General as well (Dr. Hofer is my PS -- was that who did yours?). I'll have my last surgery with him in just over a month.
-
Morwenna, that sounds incredibly frustrating. So sorry you have to go through this blood clot ordeal. It must make you extra tired on top of the plain-old recovery tired. Thinking good thoughts for you
-
stay strong, thinking of you.

Hi everyone! I'm new to this group, so here's my backstory. I had a BMX with immediate TEs placed in September 2012. Due to poor margins, I had to have radiation, and that's where all the problems started. My first PS said no problem to implants after radiation, so 7 months after rads ended, I had my first exchange. I quickly developed capsular contracture, so 6 months later, he removed the implant, cleaned up the scar tissue and used a smaller implant. I had a lot of discomfort still, and wanted to pursue a flap recon instead. So, I saw a new PS, who is very experienced with DIEP, and was set to have one last September, but the CT scan showed very small blood vessels in my abdomen. We decided to try anatomical implants instead, as I really didn't want a lat flap, and now the skin on my rads side is extremely thin, and the implant is causing discomfort and pain every day. I saw my PS again today, and he said we could attempt the diep. He said he'd remove my right implant, reposition the muscle, and look at the veins in my abdomen to make sure they would be suitable for the surgery. We scheduled a surgery date of May 4. I am willing to give this a try, as my only other option is the lat flap, and I really don't want to lose the muscle from my back. He told me not to lose any weight, or I would not have enough fat for the procedure, which I find funny, as I have quite a lovely stomach pooch going on. So, I'm keeping my fingers crossed that this time I'll be able to have the surgery, and my veins will be large enough for a good blood supply to the flap!
Best wishes to everyone, and sorry to have written a novel for my first post.

Hope all goes as well as it could do tomorrow, ME! Enjoy the nice long nap anyway!
Thanks for the commiserations and kind thoughts everyone. Give me a few days to get over the news, and I'll soon get my game face back on.
The plastics surgeon's office called today, but I was at the hospital getting my shot, so I guess I'll talk to them tomorrow for their advice or whatever ....
I go to the anticoagulation clinic Friday, so will get the ongoing plan then.
-
Mefromcc, best wishes for a smooth procedure tomorrow! I love an early surgery time so there is less time to fret! Anyone who walks out after a BMX will be doing laps around the hallways in no time after DIEP!
Five_cents, I hope your rib is on the mend. Also, 9 days out your breast can still be very swollen so maybe it will be closer to the desired size in a few weeks.
Andrea623, I had vessel grafting done when my vessels were poor. Hopefully yours will be just fine as they are.
Morweena, no game-face needed here. Thanks for sharing the situation also. I think sometime folks try to not worry those with upcoming surgeries, but we all benefit from knowing the range of issues that can arise.
I'm 16 days out from my uni-DIEP and am happy with the results so far. I'm nearly off the pain meds but still rather tired. My husband and I go for morning walks and then I spend the day video-binging and snacking. One-handed dishwasher loading wore me out today! I hope to start working at home part-time next week and full-time the week after. Fingers crossed for no complications. The necrosis in my original SGAP breast wasn't noticeable until 3 weeks after Stage 1, so I'm still holding my breath a bit.
-
Thanks everyone for the good wishes. I'm all showered, took my must-have meds, and am ready to leave for the hospital at 0500. LAstar, there is still plenty of time for me to fret-like every hour during the night because I would wake up. At least I will have one long sleep 💤 today.
-
My first conscious thought on coming round was disappointment that my nice long sleep was over. I didn't want to wake up, but the nurses were pretty insistent!
As regards my clots, I hope nobody gets too anxious, because although it is always a post surgical risk, it was particularly high in my case due to previous history and other factors.
Nevertheless, it's a good reminder not to sit around more than you physically have to in that post operative period.
I'm not feeling so miserable today!
-
Hope all goes really well for you today mefromcc.
I had 3 out of 4 drains pulled yesterday (1 week post op DIEP) and my left foob is making a sloshing sound!Anyone have that happen?
-
Sloshing is usually indicative of seroma. Do you still have the drain in on that side?
Seromas can reabsorb or your PS might want to drain it. Either way, I think it'd be a good idea to make your team aware ......
-
Ridley, yes I had Dr. Hofer too. He came very highly recommended and overall I think I'm pleased. We'll see what everything looks like when the swelling is down. He also said he would do his best to match everything up in the next surgery. It's comforting to have theses threads and know that the first surgery is all about getting the fob to live. Helps keep it in perspective.
I think I'll lose one drain tomorrow, it just dried up pretty much overnight.
LAStar - morning walks? I haven't left the house yet! Of course we're having snow and ongoing deep freezes here. My biggest adventure for today was a shower and making some lentil soup. I have to sit down all the time while cooking or my lower back aches from being hunched over.
Neta69, I had a seroma after my mastectomy and delayed too long in reporting it, ended up with an infection that really hurt and delayed my chemo a bit. Let your doctor know and see what they say - it felt soooo good when they drained it -I didn't even mind that they reinserted a drain
-
Had to have MRI biopsy on right side and it came back benign! I will proceed with mastectomy and DIEP on left. My doctor wanted to do Oncotype test on tumor he removed during lumpectomy. I was told today that my insurance does not cover it and if I don't qualify for assistance then it would cost me $4500! Has anyone else had this test run?

Welcome new ladies and nice to hear from you recovering ladies. I have a completely random thought/question: my menstrual cycle is due a few days after my surgery and I tend to be really miserable. Any suggestions for dealing with "aunt flow" during recovery lol??
-
SheriCola, I got that dreaded visit the day of my surgery. I didn't notice my cramps with all the meds, but using a pad was tricky with that catheter. I'll spare you all the details. Best wishes!
-
Can I just say what a great consolation prize the flat tummy is. Now 4 weeks post-op. Still tire easily but feeling better. My hubbie is away this week so I am looking after our 3 year old by myself. I worry that I am not resting enough but friends are giving me breaks here and there. Plus she has school a couple of times a week, so that helps.Faithfully wearing the binder and going to PT. The lymphatic massage they gave me was nice...
-
For those who have already had the surgery, how long did you have the drains? Were you very tight in the stomach area? How long were you off work? I have a million questions! Thanks!
-
ok, so this freaked me out! 5 1/2 weeks post op:
My LE physio asked me this week if I was doing scar massage to my belly scar, and I said yes, but I hadn't done a lot yet to be honest.
So last night it was looking a little dry, so I thought I'd massage with some Glaxalbase cream. As I ran my fingers along the scar I could feel multiple little pea sized lumps along the scar, and as I pressed and rubbed in little circles several of these lumps seemed to "pop"!
I can't seem to find anything about this on here, but on a cosmetic surgery site several people described the same thing, also about 5 weeks post op, and said they were encouraged to keep massaging and popping to get a smooth scar!
Anyone come across this? It's a bit like popping bubble wrap!
-
I haven't but you made me laugh with the bubble wrap reference. If it happens to me, I will have to keep my kids away from it.
-
Morwenna, could it possibly be stitches working their way out?
-
Thanks ladies, the sloshing seems to have stopped but I'm swollen. Do I need to worry about swelling if it's not red or sore?
-
I have been trying since yesterday to tell our group that I made it successfully through surgery! I now am C cup! My natural size was almost B. My sister in law says I will have to start wearing a bra. Time for next foob check.
-
Neta, I would be concerned that there is a seroma that is so full that it no longer sloshes. I had a hip seroma with my SGAP and it had what the ladies call the "waterbed" effect. When touched it would ripple slightly. Do you see anything like that? If it's a seroma, it may need to be aspirated. It's not awful but not fun.
Welcome back, mefromcc! Rest well!
-
Good for you, mefromcc! Sending healing thoughts!
Andrea, I really don't know, but there is no new redness or anything to see on the surface. I'll continue trying to find out. Maybe my PT can tell me. My surgeon is so darn busy and hard to get hold of, but I am due to see him next week.
edit: here's a link to comments on "the bubble wrap effect" on another site: http://messageboards.makemeheal.com/breast-reducti...
It seems to be something that can happen, so whether it is subcutaneous dissolving stitches popping, or early scar tissue breaking, it doesn't seem to be harmful, if a little freaky! So I guess I'll carry on!
Not looking forward to this evening when have to start doing my own daily anticoag injections, until my INR reaches therapeutic level, then continue oral Warfarin ...
-
Good point LAStar. Keeping an eye on it.
-
mfromcc: I am glad you are doing well.
-
Happy to hear things are going well for those coming out of surgery!
I am waiting to hear next week if my surgery will be an immediate reconstruction or delayed. The RO and PS are going to discuss as I will need 28 rounds of rads after my mastectomy, and there is a debate about irradiating transplanted tissue. Personally, I would rather the immediate, one surgery instead of 2, and having all this done before the new year!
take care all!
