Break through bleeding on Tamoxifen

I have been on Tamoxifen since 2012. My periods went very light and I have usually only had about 6 to 8 weeks and they usually only last a few days. My last period started on November 4th. Lasted until the 8th. Imagine my surprise when I went to the bathroom today and I am bleeding again? It is light but there is definite cramping. Trying doctor Google and it ranges from call your doctor, to this normal.

Anybody have this? Thoughts?

I haven't had bleeding like that, but my MO has a very low threshold to investigate unusual gyno symptoms, and I've had my share. (being on tamox, we have a slightly higher risk of endometrial cancer; being BC survivors means we have a slightly higher risk of ovarian cancer. My MO's approach is to mitigate the risk; i.e. check out every symptom to rule it out or, at a minimum, catch problems as early as possible.) So, if it was me, I'd call my gyno. It may be nothing or she may want to do an ultrasound or endometrial biopsy to be sure. FWIW, my gyno said that fibroids are more common when we're on tamox, so that could be a possibility. (I've never had one, so I don't know the range of symptoms, but I know they can cause bleeding) I hope it's nothing but better safe than sorry.

Thank you for your response. You answered the other question I had, whether to call BS or gyno. I will start there, as this is very unusual for me.

Glad it was helpful. Good luck with this. Even though I'm glad to have my symptoms checked-out, I find these episodes to be stressful. So I hope you're able to do something nice for yourself too, in the midst of this. take care.

hi, I'm a little late to this thread. I have been on tamoxifen for nearly eight months. Still having periods but with longer intervals in between. I started seeing a new Gyno at the recommendation of my oncologists (I had not seen a Gyno for probably ten years since my pelvic exams were done by my regular primary doc). Anyway, my new Gyno told me that I need to notify her right away if I ever experience breakthrough bleeding or spotting while I'm on tamoxifen, just to err on the side of caution.

Please keep us posted, and I hope everything works out uneventfully. :-)

    Hi Everyone,

I'm not on this drug, but my be a possibility that i be put on it, I'm very concern about this drug, and got a lot of questions for my oncologist on Tuesday. I'm seeing too many negative side effects in my opinion. Not sure what's better a hysterectomy or doing these drugs.

I was 46 at the time of my BMX surgery two years ago, and my periods stopped almost right after surgery.  I took tamoxifen for about a year and a half tolerating it pretty well.  After a hormone level blood test indicated that I was menopausal, my oncologist switched me to letrozole (femara).  I also tolerated it pretty well.

After 2 months on letrozole, I had a couple days worth of spotting.  Went to my OB/GYN who did a saline infusion sonohysterography to take a look at my uterus.Found two new polyps: one on my cervix, another on in the uterus, and it's off to a D&C for me for further investigation.  He said he has seen lots of polyps with tamoxifen, majority benign, but wants to be careful.

Hate the thought of another procedure under the general anesthesia.  I was sick as a dog after the BMX.  Also am nervous -- I hope they will use not use the arm where lymph nodes were removed to monitor blood pressure, don't know if there is anything to be worried about wrt letrozole and surgery.

Hi Dense, so sorry to hear you have to deal with this. Maybe the anesthesia for this procedure might not be as intense as with your bmx?  Also, you can definitely insist on no needles or BP cuff on your lymph node arm. They should be able to provide some sort of wristband indicating to stay off that arm.

my oncologist explained  while on tamoxifen 25% continue with regular periods, 50% have irregular periods, and 25% stop periods altogether.  Mine became very light and very irregular, ranging anywhere from 20-60 days inbetween. He said the spotting is very common and nothing to worry about, to only be concerned if the bleeding was very heavy. (they actually refused me a transvaginal US due to lack of heavy bleeding).

dense-you need to tell the anesthesiologist about the problems you've had in the past. It't his/her job to mitigate these issues.

Thanks a lot for your advice, ladies.

Just had a call with the nurse, she promised they would get the records from the hospital where I previously had anesthesia to figure out what to do. So hopefully that is sorted.

Thank you gemini4 for the wristband advice.  I'll check on that too.

Now .. a new development.  I thought I was just getting a polyp remove, but the surgery nurse just told me that my OB/GYN plans to do also do "morcellation".  No idea why my doctor planned to do morcellation during D&C. I have
one fibroid. Had it for a while (before BC diagnosis), it has not
grown and has not bothered me.  I wonder if he is thinking that maybe while he is in there, he'll get rid of the fibroid as well... Arrgh... there was just an article in the WSJ about the risk of fibroid
morcellation spreading cancer (if the fibroid happens to be malignant).  I'll have to investigate further.

    Thanks again.

It's been a bit since I posted anything, but I have a couple of questions that maybe someone could shed some light on for me

I have been on tamoxifen for almost two years, my cancer was estrogen positive, as soon as I started chemo I stopped my period, and I have had two break through periods since, one happening currently.  Is this normal?  I had a biopsy done to check for cancer in my ovaries a few months ago when I had my other one and it came back benign.  I feel okay, just bloated and crampy like a period would be.

    Any ideas? suggestions? concerns?
