A lot of side-stitch pain and rib tenderness
Long winded post. Apologies…
For the last month I have had pain on my right hand side – very sore intercostal muscles and stitch-like pain in my upper abdominal area and on my side. It hurts when I bend over and I have referred pain in my right shoulder and twinges of pain in the upper left abdominal area too. I've had heaps of stomach gurgling and have been belching quite a bit.
I've had pain in my gallbladder region since having it removed over a year ago. I have always put the niggles down to scar tissue from surgery. This feels different.
I complained about my symptoms to my onc last week and she did a manual feel of my abdomen and said my liver is not enlarged. My bloods are all normal with the exception of elevated bilirubin (which I have had since diagnosis in 2012). All of my specialists think I have Gilberts Syndrome (a hepatic disorder) which is meant to be asymptomatic.
My rib cage is really sore to touch. My whole thoracic region feels tight. I do Pilates and wonder if I may have pulled something. Or maybe it's Femara which I moved to 7 weeks ago after Tamoxifen for 15 months? It's the same side as the cancer was on which makes me even more anxious. I am wondering if it could be swollen lymph nodes?
I don't want to be a Nervous Nelly and panic, but am contemplating seeing my regular GP. I had a MRI of my spine back in August which was all clear as I had persistent sciatica that I couldn't shake. Would that have showed up liver problems? It's so close to Christmas that I don't want the scanxiety or results to ruin the season.
Has anyone else had rib and stitch-like pain from liver mets? What's the rule of thumb on waiting for pain to subside before seeing your doctor? Seeing my chiro tomorrow and hoping she can alleviate some of my fear. I hate this perpetual fear that breast cancer causes and I miss not being able to be flippant about my health.

Hi Jubby, I have been exleriencing a little of the same stuff as you. I've had the tightness and pain in the same area. It's hard to tell if it's rib pain or chest tissue. I've even thought lung. It doesn't increase with touch and it hasn't gotten worse. It's just there. They have done full blood panels and told me all liver and pancreas functions are fine. They also told me no enlarged liver. My pain is more on the side like under my arm going down but a little in the front. I do have slight truncal Lymphedema so that could be part of it. They also have done the percussion test on my abdomen and lungs, and listen to my lungs and say all is well. It's hard though to trust them sometimes especially since we know our bodies the best. I have an appointment with my PCP on the 29th and I'm going to ask her to at least order an ultrasound and possibly a CT scan. I'm also having other GI issues and gynecological stuff so perhaps a chest and abdominal CT would be the best route. I'm trying to convince myself that a lot of it's just stress and my body still recovering from chemo but I'm tired of worrying about it. I'm not on any hormonal treatment because my Cancer was Triple Negative so I can't comment on any of those being a culprit. Some say to follow the two week rule regarding pain but my Onc says it's just a guideline. If your pain gets increasingly worse or just doesn't go away you should definitely get it checked out. Sorry to hear you are having a similar issue. I rang my onc yesterday and was almost in tears after seeing my chiro. My chiropractor got me so worried after she said she didn't think it was muscular skeletal but instead 'organic'. ie. something internal that was causing my body to lock up. I had convinced myself iI had liver mets. So today I had an ultrasound of my liver, kidneys, pancreas and bile duct. All clear. The mystery continues.... I am having more bloods done to rule out inflammation of the liver as they think I have Gilbert's syndrome (I always have elevated bilirubin) and I have been taking a huge dose of curcumin which may be aggravating it. If the bloods don't show up anything and the pain persists, I'll be heading for a CT scan of my whole torso. It's not my gallbladder as I've had it removed but it still hurts like hell and feels similar to the GB pain. When I press on the lower ribs the pain radiates into my right shoulder. But tonight I decided to google rib pain and Femara after my onc suggested that the Femara might be playing a role in things. And, BINGO, it seems other women on Femara have had the same thing at almost the same length of time into their Femara stint (2 months). But another BCO person talks about nerve damage after mastectomy and another thing called costochondritis. Maybe see if you can find the thread even though you are not on hormone therapy. I'll keep you posted and please keep me posted too. Here's hoping 2015 brings a smoother pain-free year. xx

- Jubby at least your ultrasounds were clear. That's encouraging! It sounds like the Femara may well be the culprit. Let me know what else you find out. Thanks for updating

I'm sorry you're going through this worry.
Until Monday, so was I.
In brief, I had the side-stich pain and right rib tenderness - and I could feel a lump just under the rib.
GP ordered blood tests, sent me to see a consultant and on to an ultrasound a few weeks later. Blood normal.
The consultant said he thought I had a floating 12th rib with a pointy end. That's what the lump was!
Hoping that's what it is for you.

About the gallbladder surgery--5% continue to have at least one more gallstone. I've had five or six episodes of stones or sand or gravel or sludge in the 25 years since my gallbladder surgery. A few months ago, I saw my onc because of rib and upper right quadrant discomfort/pain. He sent me to PT, which fixed the rib, but the abdomen stayed a bit tender. I ignored it. Two weeks ago, I had a searing pain just below my sternum, went to the ER with a gallstone that must have been the size of Rhode Island. Big deal pain, liver enzymes sky high, and, on day three, back to the ER with a high fever and a feeling like an icepick in my bile duct, especially when I took a deep breath. Lots of scans, all negative. A week on antibiotics got me through it, but I guess I'll need a visit with my gastroenterologist (blood tests today, seeing my pcp tomorrow).Jubby, I strongly suggest you see either your primary or your GI doc.

I had a similar pain on the left side of my ribs and abdomen. it was tender to touch and I couldn't take a deep breath, cough or sneeze without excruciating pain. Had x-rays done to rule out pleural effusion and rib fractures. Turns out it was an intercostal strain. My oncologist put me on celebrex for a month and now the pain is gone! I also had my ribs taped for extra support the first week I started on the celebrex.

Well, saw the gastroenterologist. I'll have an ERCP (tube down throat, through stomach, and up into bile duct for a good look-see) next week. I've had a couple of these before--no big deal, and they'll be able to see if I have stones or sludge or whatever poised to cause more trouble.

Thanks for the kind words ladies. Ultrasound of liver gallbladder, pancreas etc all clear. The pain has significantly improved so who knows what it was. I am pretty certain that it was related to my bile duct even though nothing showed up as it was so similar to the pain I had with a gallbladder attack. How was your ERCP BrooksideVT?

Thanks for asking! My ERCP was fine. Bile duct completely open, no strictures or scarring or whatever. I continue to be a medical mystery. Like you, my ultrasound was fine, also x-ray of lungs and abdomen, and abdominal CT as well, but something, most likely a gallstone, was building for about six months, then wreaked havoc for a few hours. It took a whole month for my liver enzymes to get back to normal. Next time I have that upper right quadrant tenderness, I guess maybe I'll go see my gastroentrologist sooner rather than later.
Today I'll call both my pcp and gi for followup.
i'm glad you're feeling better and that whatever it was seems to have run its course. Incidentally, if it were not that I have been through this very particular bunch of symptoms rather regularly over the past couple of decades, I'd have been absolutely terrified.
