How does TE expansion "finished" size equate to implant size?

I hope that subject line makes sense. =). I was just curious if a 400 cc TE means a lessor or greater implant size. My PS said she over fills the TE so I would assume that means my implants will be less than 400 ccs (if that's where I decide to stop). But I seem to recall reading a couple posts that some ladies ended up with larger implants than their TE fills. I plan on asking my PS when I see her next week but thought I'd ask on here from those of you who have made the exchange what your experience was.

    Jaegergirl:  good question;  I had Mentor 450cc TE's, my final fill was 510cc (it took me 5 months to get there avg fill 50cc's).  My permanent implants are Mentor Memory Gel's smooth round 550cc on the left and 600cc on the right.  My size wasn't decided before my surgery, other than I said I wanted to go as big and full as possible.  When PS marked me in pre-op he didn't say my size much depended on what what fit after he broke up a ton of scar tissue, cut out mastecomy incisions skin and assess my overall skin quality; he was prepared to use more Alloderm if needed...I didn't need it. PS said the 600cc didn't fit on my left side but I am totally symetrical.   

    I'm 5'6" 130# with a 30" rib cage.  I am VERY happy with my size, feel, shape and I even have a cleavage now.  Recovery after exchange was a breeze, needed drains but it wasn't like the first time. Back to work after 6 days off, still sleeping on back.

    Jaegergirl - I had very wide TEs - 16 cm, and my saline implants only went up to 14 cm, so the PS had to do a lot of pocket work at exchange.

    Usually, you hear about women getting more ccs in their permanent implants because there is just no way that implants can have the projection that TEs do.

    TEs are like deflated implants, except with a hard backing. All the saline that is put into them pushes outward, against the skin and muscle to create a pocket.

    On the other hand, your permanent implants are squishy all over, so once they are inserted into the pocket your PS has created, they will go through a process called "Drop and Fluff"(don't expect your doc to have heard this term).

    As the swelling from your exchange surgery goes down, the soft implant will gradually "drop" and settle firmly into the pocket created by the TEs.

    At the same time, it will soften somewhat, and confirm to your chest wall...something that the TE could not due because of the hard backs. But the implants also "fluff" out a bit, and look more like a natural breast than a TE, which tends to look like half a coconut shell.

    I think what your PS may be telling you is that by "overfilling" the TEs, she is actually putting in saline in the upper range of acceptable fills. (Different manufacturers have different types, obviously, but most can accept more saline than the actual size they are labeled.

    This does give you some wiggle room to choose a permanent implant that will fill that "footprint" made by your TE. 

    If you haven't done so already, I encourage you to check out the "Breast Implant Sizing 101" thread run by whippetmom. She is our resident Implant Sizing Guru. 

    Be sure to read the header at the top of each page, and give her the information she needs to suggest possible appropriate implant brands and styles for your particular body size and shape. 

    This will give you a great starting point in discussing sizes with your PS.

    My TEs were filled to 620ccs. My permanent implants were Allergan Natrelle High Profile Smooth Saline 68HP-700s, filled exactly to 700 ccs. Love 'em!

    I have learned that some surgeons go bigger than TE's and some go smaller.  My PS filled me to 510cc's and then used a 450 cc Mentor High Profile Smooth implant.  Good question to ask but remember the size you want is up to you.  You have a say in that!

    Thank you, ladies, for the replies. It will be interesting to hear what my PS says when I see her this week. I told my her from the beginning that I was going to look to her for "guidance" as to what size I "should" be. I know it's my decision and she is on board with that, but as a girl who was barely an "A" before cancer and one who went her whole life being self-conscious about it, this is my "time" to get the boobs I "always wanted" (ok, I'll be honest, I'd rather NOT have cancer and be flat chested but these were the cards I was delt so....)  I  didn't want to go hog wild and have these crazy big boobs that would look ridiculous on my body and my PS knows that =) but at the same time I only get one shot at this so i want to make sure I make the best decision. I'll keep you posted. Thanks again. =)

    My PS is overfilling my TE's about "30 to 50%, so you'll have a nice bounce when we exchange them with the implants" so...I am larger now with the TE's than what I'll end up being.

    Well, I saw my PS this week and while she told me i the beginning that she typically over fills (and why), I wasn't clear if she was doing that with me.She told me that I was "done" (meaning in her opinion I was a nice proportion for my body type) but if I wanted more I can keep going. Aftet this last fill, I have been in so much pain, to where I can hardly breath and can't sleep that I may just be done. Just feel so pressured (and it's all self-induced) that I should have more because this is my only chance to get the boobs I always wanted but at the same time I don't want to look ridicolous. Decisision, decisions.  I did find out that I have Mentory Style 7200 350 ML expanders and if I have counted correctly, I'm at at 300. I will definitely check out Breast Implant Sizing again. I've read it so many time already but never really read it that closely. Thanks for your input, you ladies are the best.

    I had fills to 350 cc (one @ 200 w/ surgery , and one more at 150 cc. OUCH!). My ps put in 421cc mid profile. I love love love them. No nips. But I do the temp nip tattoos when I'm feelng frisky. The expanders hurt like crazy whenever I overdid it. However, it is sort like childbirth, once it is over you kind of forget about the pain. Whippet mom was awesome!

    I am just trying to process all of the recent info about TE'S and implants so need some help.  I keep reading that these te's are soo painful, but my PS showed me the old kind and a new kind they are using-the new models look/feel like silicone gel implants, not hard at all.  Has anyone experienced this type?  I had been warned that having prior radiation(21/2 years ago)would make skin streching impossible for me, however my PS says not true at all.  He said he has the ability to test the muscle during my first surgery(Bi-mX/TE) and that filling would not take all that long.  I have been reading about people taking 6 months and more then waiting 2 extra months for them to relax!! yikes that is forever.  I have size ddd/F now and he said having all this skin will make everything easier/faster.  He wants to use the newest thick siliclone/gummy implant with the teardrop shape(not sure of brand)  He says alloderm is an option as is using my own skin if i choose a bi mx. The healthy breast would provide the tissue for the slings instead of alloderm. He also does lots of fat grafting and may even do some at the first surgery, as it gives a better outcome and helps prevent the contractue issues, more of my tissue the better.  I need to hear form others so i can calm down a bit and prepare for this surgery at mayo in rochester.

    Well, my big day is tomorrow. 3 months and 3 days after my BMX, I am saying goodbye to these cannonballs wrapped in barbed wire and am having my exhange surgery. I'm scared, nervous, anxious and excited. Greencowgirl, my TEs are nothing like silicon. These suckers are hard as rocks (well almost) and do not move AT ALL. I cannot wait to get rid of them. My PS expanded me every week for 8 weeks and then I had to wait 4 weeks to let them "settle" before she'd do the exchange. I was a flat chested barely A before BC and I wanted to try to go as big as I could but at my last fill (4 weeks ago) my PS was concerned about my skin and that, along with being uncomfortable and at times, in quite a bit of pain, she and I decided it was time to stop. I figure I had no boobs before BC so any size will be an improvement. She wasn't sure which implants she was going to use. she was trying to decide between the anatomically correct gummies or traditional silicone. So I won't know until I wake up tomorrow to find out what I ended up with. My PS did used alloderm when she put the TEs in. I wonder if she uses more during the exchange or not. It will be nice to not have to think about another surgery for 6 months (when I have fat grafting and my flipples created) assuming all goes well with this one.  Fingers and toes crossed.

    Best wishes for an easy surgery and boring recovery.  Don't be tempted to do too much post-op when you feel so much better.  Hope your PS is as great as mine.

    Thank you, Julianna. I absolutely love my PS and everyone at the hospital just raves about her so i was in good hands! Surgery went well. It took an hour longer because my PS wanted to make the foobs perfect (what she told my hubby when she came out of surgery). I won't know how they look until Monday when I go for my follow up and she removes the ace bandage. My PS ended up going with the traditional silicone rounds because she wanted to give me the most natural look possible. The anatomical gummy bears, while she had two different sizes with her, the bases were too wide for my frame. I am ok with that. While I don't hurt as much as I did with the BMX, I am still really sore. Looks like its the recliner for me, couch for my hubby, Percocet and ice packs for a couple of days. My son was so excited when he came home from school because he thought he was finally going to be able to give me a "Real" hug but I told him it would be just a couple more weeks. Thats ok, the cannonballs are gone, and I finally have my armpits back!!!! Btw, in response to the original question I posted, I was filled to approx 300/320 ccs each side and my expanders were replaced with high profile 495 ccs each.

    My PS did not overfill my TEs. I was filled to 420cc and had 430cc implants on both sides. :)

    Hello everyone have a question although I can't find out what size my expanders are my Drs are enjoying screwing with my head apparently. Even asked for the card and had no idea what I'm talking about. Sorry just Very aggravated. Anyhow I'm filled to 420 cc on both Dr is only doing 1 more possible to 450cc. I weight 152 pounds, was a 38B ribcage measures 38 1/2 I'm 5'7. Any advice he's saying I'll be a C cup won't go bigger because of 2 previous failed back surgeries. So any suggestions would be much appreciated. Thanks and sorry for seeming so grumpy.

    lynne - you may want to approach the hospital medical records department and get a copy of the surgical report - it should contain the information you are looking for.  Tissue expanders are medical devices with unique serial numbers, and you are entitled to have that information - the hospital should have the medical device information in their medical record, but it should also be in your plastic surgeon's record.  When (or if?) you get the info I would recommend going to this thread and relaying it there to get your questions answered.

    Lynne - I have a card for my final implants, but I don't have one for my expanders. I have a 39" ribcage and was a B-C before. I used my path report to use the grams removed at the mx for roughly the ccs for my final fill. They took 385 grams out at mx, I was filled to 350 cc, but she was able to put in 421s but they are moderate profile, so definitely more B than C, which is good with me. If you haven't already check out the breast implant sizing thread. Whippetmom is a rock star. She helped me immensely as she so graciously helps so many other ladies here. BTW my expanders were 14 inches across (I think). Good luck with your exchange!

    hi ladies, i have question and sorry if this has already been asked, but the TE that we have in us, when does the doctor decide you are done? These things are rock hard. I had the TE put in after by bilaterial I had cc injected then, and so far two cc injections. I never know how much, my stupid fault for not asking, but I was told she wants to get me to 500 cc's total. I don't want as big as I was before, Lord I was a 40D before and I don't want to be that big again....she keeps telling me I was a large breasted women and she won't take me back to that size, but how do you know when enough is enough. Also, the bigger you go, does the scar get wider? And I was told when I get to the final cc I have to wait 4 mths for the implant surgery, then the nipple surgery will be after that in a few months.....what is that like? Advice please! Much appreciated ladies!

    Hi funnthesun - I decided I was done after my first fill of 150 (200 in OR). You are in the driver's seat so be sure to make your opinion known. Again Whippetmom can help with suggested sizing. If you haven't already, read through her thread, you'll learn so much. She knows her stuff! Special K linked it above, My mx scar is across the top part of my breast and for the exchange my PS only used about half of the original scar (I can tell because the other half is much more faded). I waited four months after the final fill for the exchange too. Later I decided on 3-D nipple tattoos from Vinnie Myer instead of nipple surgery. Here is a link to a thread about Vinnie's tattoos.

    How long did you girls keep taking the vallum? I have trmadol and vallum and it says every 6hrs. Everytime i try to cut back i feel super "tight" . Today i tried to do half every 4hrs instead of a whole every 6? Not working. The tramodal i only take before bed nnow. Help, what so u girls do or take for relief? Ive had 3 fills. Every Monday, 30cc, 40cc and last Monday 50cc. I just dont want to be tored all the time! Advice?

    I used Tylenol only for pain after a few days post Mx though if I did any physical activity that engaged my pecs it hurt like crazy. I stuck those self heating sticky pain pads to the base of my breasts, used bengay, hot tub and such. I did need Xanax for anxiety for about a month and a half, weaned off too early then starting having panic attacks. Not fun. Those were tough days.
