Baltimore area PS for a reconstruction redo

I had a BLM with implant reconstruction 5 years ago.  I've never been thrilled with the results.  I think the surgeon placed the implants a bit low and I have a lot of upper pole flatness.  I also have very thin skin and have a good bit of rippling on the sides.  In addition, I have "indents" towards my armpits.  Does anyone have a recommendation for a PS in the Baltimore area who does outstanding implant reconstructions?  Maybe someone who uses a lot of fat grafting to fill out around the implants?  Any names would be much appreciated!

I would also love to hear from ladies who have had fat grafting on top of implants.  Is it smooth? Lumpy?
