Tissue Expander to FInal Implant Questions
Hi Everyone,
I am relatively new to the site and feel a bit overwhelmed by the amount of information here. So, I am hopeful my questions can be answered by the wonderful people here and their experiences. I had a bilateral mastectomy in June of this year with TE's placed at time of surgery. I am approaching the end of the filling process due to a number of reasons (per my PS) - 1) My tissue is about as stretched as it wants to go (one side is more "tight" than the other) and 2) The TE's that were placed hold about 350cc's. My size prior to surgery was a small A cup (on a good day!). I believe I am at about 325-350cc's at this point and I don't know how that translates to a "cup" size...
My questions and concerns are -
What should I be asking my PS at this point in regards to the final implants? Size? Style? Or do I just let him make the decision based on my physical frame?
Is my PS correct in saying my tissue has reached it's maximum stretch? Is that due to the limitations of the chest muscle and how lean my body is?
What is the process in determining the final implant and style? Is this left up the PS to determine and when is it decided? Before surgery or during?
I would appreciate any help and insight which can be shared! I feel a bit like I am working in the dark...

I had a bilat mastectomy in June also and just had my exchange surgery 3 weeks ago (3 weeks ago tomorrow, actually).
I am / was exactly the same size as you. I was also a small A before the mastectomy. And, I too had tissue expanders that were ultimately filled to 350 cc on one side & 320 cc on the other side. My right side requires a nipple reconstruction b/c cancer was found in that nipple so it was taken off. The PS uses skin for that reconstruction, which is why that one was filled more.
I know that my skin was stretched extraordinarily tight. My PS actually showed me by turning off the overhead lights and shining a light through the skin, and it was practically transarent. Anyway, he said that was definitely a sign that we were done with the filling. Honestly, particularly at that time, at 350 cc, it was really bigger than the size I wanted anyway, but I knew he was overfilling the one side for the nipple reconstruction.
Ultimately, I told him I wanted to be about a Size B. I actually also told him I liked the size at about 320 cc. Ultimately, I ended up with 300 cc implants. I would have liked them to be maybe a little bit bigger than they are, but other days, I think if they were too big, I would need new clothes in some outfits, so all in all, I am good with it. I know in my case, he placed several different sizes in during the surgery and his PA and the BS (who was there for the nipple removal) gave their input as well, based on the size I expressed to them I wanted to be and how they looked. As he said, how they look is very dependent on the person's frame. B/c I have a very small frame, the size he implanted look like very solid to large B on me, but would look small on others.
I know whipplemom on this site will also help you with trying to determine the best implant size for the look you are trying to achieve.
Good luck with the exchange! It is a cake walk compared to the BMX. I was at home the same night and out and about the next day with absolutely no pain at all.

Amy, thank you so much for sharing your story. Your story and information was so very helpful considering there were so many similarities! Could I ask you if you got the "anatomically" shaped implants and were they silicone? My PS is suggesting these and also with a higher profile to help with the step down from the upper chest area to where the implant might start. Due to lack of much body fat on me, there is a noticeable step where the breast tissue was removed. I am not sure if the implant alone will fix all of that, but I guess we will see. I too have little body fat...more now than before surgery unfortantely, but I digress.. My TEs were only 275cc and filled to 350cc. My surgeon thought that was enough for my skin. He had mentioned 370cc for exchange, I liked how my TEs looked and he said my implants would be close but a little smaller. But in surgery he actually ended up using a 425cc Sientra Shaped implant. I guess after he tried the others he decided he had some room so tried the bigger ones and the general consensus was the 425cc looked better. I too was an A on a good day before surger and now a C and sometimes a D depending on the bra. But I look like a small C in clothes. I think the implants look pretty close to the projection of my TEs...but look and feel much better!
Are you wanting to go bigger than your TEs? To get the projection close to your TEs your implant should be about 75-100cc bigger than your TE. Since the implants are soft compared to the TEs the PS can usually go up at least 75cc bigger than your TE. Now if you have had radiation this might not be true..I didn't have radiation so I don't know much about that. Except the skin loses elasticity.
I wonder sometimes how women are filled to 800 cc's do it? Does the muscle really stretch that much?

Thanks "Mischeif" for your thoughts and suggestions. I so appreciate you sharing all your details and how your PS handled your surgery and implants. It will help in my next conversation with him. No, I don't really want to go bigger than my TE's currently are. I think I am pretty much fine as they are. My only concern is that I have read that the final implants end up being smaller than what the TE's are. Not sure if this is true anymore with all the implant possibilities that exist. My PS mentioned he would take measurements at the time of surgery and decide on the final implant at that point and he is currently leaning towards an implant that is anatomically shaped with a higher profile to take it up higher on my chest. When he talks about the profile is that the same as the projection?
I did not have to have radiation but one side seems to have more skin flexibility than the other. Could be due to the amount of skin they took during the BMX.
Do I need to contact Whippetmom directly to get her input?

well, my only suggestion is if you like the projection of your TEs and would like to be the same size after the exchange, to make sure your PS knows that. ( from what I understand profile is the height of the implant, how high up the chest wall it goes and projection is how far it projects out.) He will need to go up in CCs if you like where you are. My 425cc look a lot like my TEs at 350ccs. The implants are soft so they conform more to your chest wall than the hard TEs. I too had a side that was always tighter. If you have 350cc expanders and get 350cc implants you will notice the difference in size.
You can just post on the Breast Implant Sizing 101 thread and Whippetmom will get back to you with suggestions. She is really good and knows her stuff. Just start your post with her screen name. She will need to know your height, weight, and TEs model and your measurement of rib cage under your TEs. If you give her all that she will let you know what would be the best implants for your size and shape.

Frances, I am just as confused at you are! I am meeting with my PS tomorrow, and am scheduled for switch on 11/22. ( I haven't written to whippetmom because I dont know what type of TEs I have.)
The PS gave me lots of reading material re types in implants, - but I have to think that he knows better than I do what would be best.
I was a small B before bmx, and would like to be at least that after all of this!
I have had only 3 or 4 fills. The expanders are very hard, seem very far apart in front, and not much 'projection'. I did have radiation on one side, years ago, so that is limiting the amount of fill. The skin on that side is very tight.
It will be interesting to see what the PS tells me tomorrow! Will let you know!

Truthfully, sizing implants is both a science and an art. Using your former cup size, or deciding you want to become a certain cup size, kind of goes out the window with breast reconstruction.
For the first time, your sizing will be determined by SO many factors. Please be sure to read the ENTIRE heading at the top of whippetmom's thread.... and then send her all the information she requests.
The majority of us have received the exact same implants, in the exact same amount of ccs as she recommended.
For others, it was a good starting point for asking your PS the right questions.

anotherNYCgirl, my TEs were so far apart I felt like you could have landed a small place between them...It seemed they were under my arms more than on my chest. But all of that gets fixed at exchange. It's nice to go from rock hard TEs to soft(er) implants.

I am back from PS. He said that he will use anatomically shaped implant, as opposed to round, due to the amount of tissue removed during my bmx. Round won't lay as well on top.
He will try to reduce space in middle (and told me to remind him that day. I hope that I remember to mention it!!)
He'll have various sizes by Mentor and Sientra, and will make the final decision during surgery. There is concern that the radiated side will not handle it as well as the other side, and prepared me to need follow up fat transfer and/or alloderm. Of course, I had to sign all sorts of waivers, confirming that I am aware of all the risks. ugh
Fingers crossed for easy, uneventful switch for all of us!!
