Dr seems to keep wanting to do consultation over phone

I feel like she doesn't want to talk to me or maybe doesn't want to guide me at all.

Dx 1 week ago with ILC ER/PR + Her2-. Have not seen MD since Biopsy. Have talked to MD on phone twice to get results. I told her today I was thinking about bilateral mastectomy. Her response was that was maybe more than was needed and referred me to talk to a plastic surgeon before I finally have consult with her.

This seems quite backwards to me. Also seems like she is not being aggressive enough. Am I crazy?!?

Is the MD you mentioned a breast surgeon?  Generally, they are the ones who advise you on if you are a candidate for a lumpectomy or if your individual case warrants a mastectomy or bilateral.  Talking to a plastic surgeon is also a good idea because they can tell you what options you have for reconstruction, whether you go lump. or mast.  So if your MD that diagnosed you is not a breast surgeon, if I was you I would seek one out as my next step. Sorry you've had to join us in breast cancer land.

surgeon seems to think that either I can read her mind or I can figure out the best course of action with no input from her.

It doesn't strike me as backwards to talk to a PS before consulting with the BS.  It will save time for you.  It may be that after you hear about reconstruction, you will wish for a less aggressive surgery to avoid reconstruction.  Or maybe reconstruction possibilities will suit you just fine.

I met with my BS first who was rather discouraging about doing a double mastectomy and immediate reconstruction.  But he would do it if I wanted to.  However, I didn't really know if I wanted it or not because I hadn't talked to a PS.  So I followed the BS's first recommendation.  I could have done it the other way, and I think I would have been more informed.

Maybe the breast surgeon wanted to consult plastics in order to more fully present your options.  There are many types of reconstruction, some of which can be done at the same time as the mastectomy.

My advice is to see an oncologist before surgery, too.

Run. Find another surgeon. I don't even know where to start with that situation.

I also agree that a consult with a plastic surgeon is not unwarranted before consulting with the breast or general surgeon.  If, for example, the plastic surgeon tells the patient that he can "match" the new breast to the old, but then reminds her of something like "of course, when your existing breast begins to sag, or you gain/lose weight, it will change - but the reconstructed breast won't" - that might help a person decide lumpectomy or mastectomy.  Or maybe he will talk about a "faux" nipple reconstruction, etc.  I guess, as I type this, it still does seem that consult could come after the general/breast surgeon - but at least before making a decision.  We know that the general/breast surgeon needs to know if he/she is performing skin saving surgery, for example.  I decided on a mastectomy without a plastic  surgeon consult at all.  And I can't really recall my surgeon appt. providing me with too much info - though I'm sure it did.  Bottom line is - if you're feeling uncomfortable about your surgeon, that's a reason to shop around. But I don't think it's unusual to refer someone to see a plastic surgeon to have a talk, before deciding on a double mastectomy. They want you to know all your options and know what you're getting yourself into. Good luck on this tough decision. Best to ya.
