Anyone do the Tram and then Latissimus Dorsi later on?

I'm a little scared. This is 2 years now-

I had neoadjuvant chemo > BMX with Tram flap recon in 2012. My lymph nodes are all gone in the right side- it caused nasty nerve damage and lymphedema symptoms. Then I had radiation and it literally melted my breast to my chest wall.
Fat grafting didn't work to fix it. Now my PS gave me the choice of the Dorsi surgery or just leave it. I want it fixed, but I'm a little scared because the first surgery really wrecked my arm permanently. I worked on softening the scar tissue with castor oil packs... (it worked too!) I am an artist and work with my hands and I kinda need my right arm to work and function. This is my biggest concern. I have not had any expanders at all.
I'm tired of all of this, but I want the radiated side to look normal and not all hard and shrunk. I have an excellent PS from Fox Chase in Philly so I'm not worried as to his abilities, just curious of the recovery since I have to go through it all over again. Any help or advice would be most awesome <3 thank you :)
