Disppointed After Exchange- Allergan 410
Hi ladies,
I desperately need some advice. I'm single and in my early forties and feel really disappointed with the look and feel of my reconstructed breasts. I exchanged my TE for 655cc Allergan 410 (FF) implants about 6-7 weeks ago now- I'm 5'10 and medium frame. I'm one of the few women that like the perky ("wonder bra") look of the TE and expressed to my PS that I really wanted smaller C cup breasts with upper pole fullness. The upper pole fullness was and is very important to me.
I originally asked for round implants (though I didn't name a specific type or model) and he was partial to the Allergan 410s saying that they were best for reconstruction and that he could still achieve some upper pole fullness with the shaped implant, as opposed to round. Fast forward to now with the swelling gone and my breast look really mediocre -- very firm implant, some cleavage; very little upper pole fullness. In fairness, they look really natural and some people might think they look good. However, I'm partial to a faker looking breast having had an augmentation before breast cancer hit and I'm really unhappy with these. To make matters worse, I cant even get them to perk up in a bra because the implants are so firm and don't move much.
I want my PS to do a revision but I'm concerned about the following:
PS says the round implants are not great because they will cause too much rippling and not sit right. Do any of you have round implants and did you have a lot of rippling? If so, did you do fat grating and did it help? Fyi, I have some fat for grafting purposes but not a lot to spare so rippling is a concern.
Has anyone regretted doing revision after exchange due to complications or the revision implants looking even worse? In other words, was revision worth the risk?
PS surprised me at post op appt by saying that I would have to do 3d nipple tattoo (as opposed to reconstructing a nipple that can be felt) because I don't have enough skin to build a nipple. I had the option to do a nipple sparring mastectomy but I went with PS suggestion that they construct new ones after reconstruction and then he tells me this is probably not possible. Has anyone else had a nipple built after being told that they didn't have enough skin? I hear that they can graft a nipple from other body parts such as an ear lobe-- has anyone done this and are you happy with the result?
Sorry for so many questions but I'm feeling kind of desperate. Thanks in advance for any help you can provide.

I think you need a second opinion from another PS. You do not need a lot of fat usually for grafting, they do grafting even from very thin women, and there are many ways to create reconstructed nipples if that is important to you, like by removing skin from elsewhere. Also, if he is not confident he can give you a good result with rounds, I would not want him to do a swap to rounds...go to someone else.
I have smooth silicone rounds. My PS is opposite of yours in that he prefers rounds for most reconstructions though he has been using anatomicals for years for both recon and augmentation (we are in Canada so they have been available for a while). I have not had fat grafting. I can see one very slight dent/ripple on outside of one breast depending on how I move my arms or lean over (when not in a bra), and after wearing certain bras it is more visible for a bit (it goes away after the bra is off for a while), but I am very happy with the shape, size, feel, movement, projection, and that ripple, that is usually not visible, does not bother me to point I even really want to do a new surgery to do anything about it though I am sure grafting would hide it quite well. I think it is there as that breast that was a bit larger to start with and the skin/pocket was a bit more loose (I went nipple sparing direct to implant). I know (and have seen the results) of some other members who had some rippling and fat grafting or other revision with great results.
I do think good results can be found with either rounds or anatomicals, but your PS ought to have listened to YOUR preferences for appearance here, I have seen some great anatomicals on women here, but it is not a look I preferred for myself either. I never had augmentation, but I did in my younger years have much fuller and rounder breasts, which took on the look closer to anatomicals in last handful of years and did not like that look as much.
If you have not done so, please go to BREAST IMPLANT SIZING 101, and give Whippetmom the requested info in the header for her input as to what rounds, if any, would work best for you. She may need you to send her some pictures. A big factor in reducing rippling is choosing right implant and size for the pocket. You will need to wait a few months before a revision though, I imagine, and may or may not need pocket work.

Wow DiveCat- thanks so much! Your post has tears welling in my eyes out of appreciation and the possibility of feeling better about my breasts in the future.I thought about a second opinion but my PS has such a nice demeanor and is willing to revise in attempt to make me happy, and I wasn't sure whether insurance would cover a swap/revision by another PS. I think that you are right that, even if PS is the best/nicest guy in the World, he lacks confidence in getting a good result with rounds and may not be the best for the result that I WANT. Getting a second opinion will help put things into perspective to see if what I want can be reasonably achieved-- both with rounds (or a perkier look some other way) and nipple reconstruction.
Congrats on your successful reconstruction by the way-- from your description alone, I have breast envy. Lol ;-) Seriously though, it would be helpful to see other reconstructions gone well in this process since this provides confirmation on what is possible and provides hope for a brighter day.
I've requested access to the picture forum but it may be some time before I'm granted access since I'm a newer poster (I've used the forum for the last 6 months getting through my BMX but when things went wrong did I really go back and study the details in this reconstruction forum.) For some silly reason that I now regret, I deferred to my PS' expertise and figured all would turn out just fine. Oh well, fingers crossed that I can get help to fix them.
I will also re-visit BREAST IMPLANT SIZING 101 tonight, and reach out to Whippetmom who I saw was the breast guru for this purpose but I was somewhat shy in contacting her to this point.

A second opinion does not mean you NEED to go with them or not go back to your first PS, but it just might help you hear a second perspective
Don't regret what is done, you certainly are not alone or wrong for trusting your PS, and you having a different desired cosmetic outcome does not mean he isn't a nice guy, or a talented surgeon, just maybe not the right fit for you (or maybe he is, but I still worry he feels he can't get a good result with rounds!).
I hope you can get access to the picture forum. I do think seeing photos of other women going through this is beneficial for many ways: it helps with realistic expectations of what one can expect after reconstruction (both positives and negatives), it helps one know they are not alone, and it shows that every journey IS unique as there are success stories AND success stories AFTER complications or dissatisfaction.
Don't be shy about whippetmom (Deborah) either, she is great and you can actually PM here if you are not comfortable putting the details on the thread itself.

Sientra promotes upper pole fullness in their round implant. Could be just what you are looking for..

I should have read a little further before posting my new topic. I have full height moderate projection allergen 410 implants. I think they are 600cc. I don't remember exact cc's. I have grade III capsular contracture and the front of the implants are completely flat. I cannot fill out a bra correctly at all. I went to a store that provides service for post mastectomy patients and they fitted me with prosthesis and bras. The prosthesis are for patients who have had lumpectomies or for patients like me who have problems with the reconstruction. I am very thankful for them because with the implants alone, due to the contracture, my clothes did not fit properly and it was very noticeable that the breasts were different sizes and one was higher. I am so disappointed in my reconstruction surgery. I have opted to not follow through with the nipple reconstruction because of the poor outcome.

One thing I will caution you about with a second opinion and plastic surgery that has already been done - or is in the process of being done - some plastic surgeons will not see you for a second opinion unless you tell them that you will NOT be going back to your original surgeon. This type of second opinion is different than one you would get prior to work being done - you are asking for advice/opinions about another doctor's work, and due to the fear of malpractice suits, and providing commentary on another doctor's work, many plastic surgeons will not see you for an appointment unless you indicate that you are NOT currently seeing another surgeon. unless you go out of town for a consult, it is likely the docs know each other. I live in a fairly large metropolitan area and this is the case here.

DebbieNinja- I'm so sorry to hear about the trouble you're having with the CC. I've always heard that CC was less common with the Allegan 410 implant, and textured implants in general. Anyway, it sounds really awful and frustrating and I hope you are able to address it.... my skin has been sticking to my implants a bunch except for the areas where there is a little bit of fat and I've wondered if that could be the beginning of CC-- I have no point of comparison though and so I plan to inquire with my PS as to whether it is normal. So, are you planning on a revision to address the CC? You deserve to be happy just like everyone else.SpecialK- thanks for the second opinion tip as well! I had not idea and would not have thought about that in advance. Appreciate the wisdom.
