Reconstruction Insurance coverage

I am sure people will come along who know more about this than I do, but I believe there is a federal law (if you're in the US) that says insurance has to cover surgery/other reconstruction for purposes of overall reconstruction or symmetry (as in lumpectomy).

The federal law covers only reconstruction due to mastectomy.  There are some state laws that extend that coverage for lumpectomy, but it is not universal.  You would need to check with your state and with your own insurer.  Here are some links:
