Infection in breast, losing implant?

Hi ladies. I've been through a lot since my diagnosis (as we all have been). The most recent, I was hospitalized for a couple days due to an infection in my right breast. I had BMx jan 29, exchange June 2, TAH/BSO July 1 and lots of procedures in between. My ps wants to try to keep implant, so she put me on vancomycin I'V antibiotic. I was on it in hospital and now at home. She also took me to the OR to open up incision and clean it out with antibiotic. There has been one spot on the base of the incision that wouldn't heal. It would scab, scab would fall off and I'd be Left with a hole and you could see the implant! Each time ps would cut away unhealthy skin around the hole and stitch it up, glue it etc.
I see ps on Tuesday to see what she thinks about my healing... My question here is if I need to have implant removed what is the procedure, recovery, future reconstruction options etc.
Anyone who has had similar experiences please respond. Thanks!!!

Hi Mommyathome, I am interested to hear similar experiences too.
I had an incision infection like you, and you could see implant in the hole. I had the same surgery you had, but I'm on ciproflaxicin for 14 days. I'm going to the docs today to have the stitches out.

Good luck at your appointment. Let me know how it goes!! My ps always uses dissolvable stitches... This time she must have stitched under the skin and just glued top of skin- I see no stitches...I go tomorrow for follow up

I have had a couple of infections. Mine have not affected the incision though. I had my implant removed a few weeks ago. We haven't really discussed the next step but in looking at my chest I will have to go through the whole expansion process again to try again.

I have had an implant removed in May due to healing issues without any infection, but not on the main incision - my incisions are IMF and this was an incision for areas of non-viable skin up on the front of the breast. This was after two surgical tries to close the skin and keep the implant in place - the second surgery the PS even placed a slightly smaller implant hoping not to stress the skin as much - didn't work. He felt that because the implant is a foreign body stretching the skin it would never close, and these areas had been open (two dime sized holes in the skin with the alloderm showing) since the first week of March. Once he removed the implant they healed within a week. I have to have the implant out for 90 days and then he will place an expander and start over, then exchange. I also had this issue with the original TE - several surgical tries to save it and then removal, this area of skin had healing issues due to necrosis from the BMX surgery in 2010.

Ladies thanks for sharing! My opening was about a pinky finger around. It just wouldn't heal. 4 attempts later, here I am. With a picc line in to do I'V antibiotic at home waiting to see if after this last attempt, it'll work. My ps said if not we will take it out. I'll be flat for 3-4 months then we start over with Tes and exchange.
Another question I have, some people insist on telling me rest in order to heal others suggest moving around, going for walks etc. which is best?? I honestly have no energy. I hardly sleep at night and I'm exhausted during the day. I'd rather just lay around but is that doing more harm?!
I'm very nervous and anxious because I've been out of work since January ( I teach) and Ivan supposed to start back on August 25. I am hoping to get into my class August 18 to start setting up, doing lessons etc. it's already August 11th!! I have 1 week!!!!

Stitches out and all looks good:) thank you for thinking of me.Stitches pulled, replaced by a strip of brown plaster over the incision which will (I am told) fall off when ready and all will be well. But it could flare up again when I am off the antibiotics.
Sorry to hear that SpecialK, it's a lot of surgeries for your body to take. And over such a long time span too, it must be very tiring for you. I'm glad to hear that your infection has gone, but sorry you lost another implant. I'm counting the days down with you til you get another one. Hopefully the next one will be trouble free.
Mommyathome, I know what you mean when some say rest and some say keep moving! I really don't know what to do either. Are you able to take a walk every day? That helps me with the sleep. Probably a mad idea, but you could have a run through at home for your lesson structure - sort of like a mock exam. Then you would know how much you can and can't do. Unless you are a PE teacher, then that would be very different! I've got a feeling you'll be ready in a week.

What a lovely age group to teach..I have gone into my local school and done singalong sessions for 5 - 6 year olds. Their little faces! Shiny eyes all big looking at you with awe. Did the Gruffalo song, wheels on the bus, and of course twinkle twinkle little star hee hee

I think that walking is good because it oxygenates, moving around seems to be something that can worsen a seroma or exacerbate fluid production for drains, but for healing oxygen is desirable. My PS has always encouraged me to get out and walk. That is the theory behind using hyperbaric chambers for wound healing. So, if you can generate some energy, I think walking and moving around is good. Eating 100g of protein is also good for healing as well as eating pineapple for the bromelain and taking some zinc.
songbird - sounds like you are on the right path, hoping things are going well for you. Yes, it has been a long journey for me, and I am not done yet! But, I am good and thankful for your good wishes, so don't be worried! I have never had any infections - just weakened skin, which has proved to be problematic.

Hi SpecialK, thank you for the advice - I will try extra protein, nuts and avocado should be good. I had not thought about adding pineapple and did not know that it contained bromelain, so that is on the shopping list for tomorrow:)
Looking at your signature, Long journey doesn't quite capture the last four years of your life in terms of operations and procedures. You are amazing. May I ask was there something that caused the weakened skin? Was it SE of chemo?
I had to google search the hyperbaric chambers. Sounds scary! I think I got a similar effect from the strong wind today while walking my dogs - we got the tail end of Bertha for the next few days.

So she took off bandage and applied steri strips... Said no promises but we will give it our all to save implant. I'm on I'V antibiotic until like August 21 or do. Then she's going to take picc line out and wait and see if infection comes back. If it does I lose implant. If it doesn't I'm good.
But she had noticed that now after the revision she did in the hospital my right breast ( the problem one) sits higher and is firmer than the left. It resembles the te. She said its because of how she had to place the muscle.... ? She told the pa that they'll have to fat graft at some point. I was like, let me get through this! I'm not signing up for any more surgery unless absolutely necessary!!

songbird - the weakened skin was actually a SE of the BMX procedure itself - I was one of the unfortunate 20% that develop skin necrosis after skin sparing mastectomies. It is skin death that occurs when the blood supply from the muscle to the skin is patchy. I have extremely thin pectoral muscles - my PS said he has only seen one other case of that degree of thinness in his years of practice. So, my skin did not form a good connection to the muscle and those areas of skin on the left breast were compromised, so they were excised. This has continued to be an issue going forward - clearly! I never would have thought I would have recurrent issues with this in the surgery in March though (it was a replacement of the allograft material) because it had been so long since that area had healed. It tore during that surgery as the skin is paper thin there. The repeated attempts to excise and re-stitch did not work, so the implant had to be removed. I do not heal well in those areas with a foreign body underneath - be it expander or implant. In both cases, I had easy and complete healing once the foreign body was removed. Greek yogurt and protein powder smoothies also pack a good protein wallop! I use vegan protein powder to steer clear of soy and dairy.
mommyathome - FWIW - fat grafting was one of the best things I did - easy outpatient surgery for me, worst part was wearing compression garments and being barred from the gym for 8 weeks. Something to think about for the future, but I hear you on just getting through the immediate problem!

I don't understand why it would be higher and firmer either. Perhaps it is the way it is healing. I've heard they "drop and fluff" after a couple or few months, but I'm not sure what that really means.
I feel the same as you about not wanting to sign up for any more surgery. Fighting an infection really brings it home just how serious any kind of surgery is - especially if it goes wrong.
You've got some serious potent antibiotics working for you. The "van" part is so called because it vanquishes (thanks Dr Google). How are you coping having a picc line?

Songbird- my husband has been doing the I'V treatments for me which causes some conflict with his work schedule but I'm too stressed right now to do it myself. Sounds crazy I know, but it's true. I'm someone who hated to take Tylenol for a migraine. Now I've been on all these meds!!!!! Freaks me out! When they said we r putting in a picc line so you can do I'V antibiotic at home I laughed.... Until I realized they were really expecting me to do this at home!! Lol nurse comes tomorrow to take blood and hook me up.
I think the change in how the implant feels is because she moved the allied em and muscle a bit when she went in to excise. Actually, I like how this breast feels than the left. But not sure I'll sign up for fat graphing, nipples etc. I just want to be done!! I have to go back to work and my kids want their mama back

I developed some sort of infect and had to have capsulectomy. I'm five days post-op. Surgical site is fine, draining okay and basically pain free, but I pulled a rib muscle sneezing while waiting for my BS and PS.

I'm five days out from a capsulectomy. I developed an infection underneath the implant and was stage 4 in capsular contracture. I still have a drain and my output is still red.
I would imagine recovery period would be the same as the initial mastectomy.

It's the removal of the implant and scar tissue that formed around the implant.

mommyathome - FWIW - for what it's worth. You could always do 3D tattoos instead of nips - much quicker and no downtime.

Mommyathome, I would be just as squeamish about doing my own picc line too. I am happy I am on tablet form. No sign of infection so far. I hope it doesn't come back when I finish the antibiotics. I've heard that can happen - eek!

songbird - expander placement in October, expansion slowly, then exchange. I have a 650cc implant on the right that remains in place and trying to match it with a prosthetic is a pain, so I do want to try again. I had no issues when I went through this process the last time, so I am hopeful. I just won't mess with success again. If I had it to do over I would not have done the allograft repair back in March and just left well enough alone, but hindsight is always 20/20!

I'm a little late in the conversation, but I bring another perspective/experience, should they have to remove the implant.I had my BMX with TE placement in Nov, 2011. Fills beginning Feb, 2012. Exchange July, 2012. I had an allergic reaction to the post op antibiotic so they discontinued it. 9 days post op I had a fever of 104* and infection in the left breast. I was hospitalized on IV antibiotics for 7 or 8 days. Then they sent me home with a PICC line. I had a great dinner, showered and my right breast simply would not dry off. It was leaking! Back to the ER. My right side had developed a seroma (fluid pocket) that had burst my incision and exposed the implant. The implant was not ruptured, just exposed. They put me back in the hospital and removed my right implant the next day. They did not want to risk infection in the open wound, and felt my thin skin would not heal over the implant.
I went from July, 2012 to May 2013 with one implant and one skin sagging right breast. They left the sagging skin, so as to have that skin for furture reconstruction. My PS decided that my skin was too damaged from the TE, implant, seroma, etc... that a new implant would not hold. So, May 2013 I had phase I DIEP. March, 2014 I had a minor scar revision where the incision from May, 2013 had not completely healed! June 2014 I had phase II DIEP.
Because of my weak, thin skin, my PS waits a long time between each procedure.
I am satisfied with the results of the DIEP. I definitely would have preferred if the implants had not been lost, and this was all over. But......................

SpecialK there was every chance the allograft could have fixed it. I think it was worth a try. I am glad you won't have to wait long til October. I hope the expansion process is not too long after all you've been through and that you come out the other side looking fantastic and balanced (did I say that right?)
shechirple I'm so glad the DIEP gave good results.
Your leaking seroma sounded traumatic. I had leaking from my incision, but more like a gently dripping faucet, so I can only imagine how nasty yours was.

Songbird, I don't know how long it will take before my PS is able to do an implant. I'm only six days post-op and still waiting for the results. My PS told me on Tuesday that I won't need TE which is good. I'm not in a hurry at all.
