August 2014 Surgery
Hi everyone! Anyone having surgery in August?
I'm finishing up neo-adjuvant chemo next week and will have a BMX with TE placement on August 18. Then I'm on to rads followed by the exchange surgery and a hysterectomy somewhere in the mix since I'm also BRCA2+. I wasn't nearly as nervous to face chemo as I am this surgery. It's funny, on one hand I can't wait to get rid of my breasts, but I'm also terrified of complications and not knowing what my body will look like when all is said and done. I'd love to hear from others going through surgery next month so we can all support each other through this crazy journey.

Allydp- Greetings from another Ally!! And also an August Surgery Sister! I am so glad you posted as it seemed other months were there but not August. Maybe we will get a group to post here. There are several that are having procedures done in August. Anyway, I had my PBMX with TEs placed on May 5th and am scheduled to have my exchange surgery August 11th!! Yippee!! Congrats on finishing up the chemo and being able to move forward. There is a pic forum that can help and my biggest advice is to talk to Whippetmom (Breast Implant Exchange 101) before your surgery. I did not find her until after my TEs were placed and the type and size of TEs make a difference in the implant that is eventually used. She needs you to give her some information and you can PM her if that helps. The ladies on the June, July and September boards are super helpful!!

Hi fellow Ally! Congrats on your upcoming exchange surgery! It must feel great to know you'll have those TE's out soon! Thanks for the suggestion to speak with Whippetmom. I have a pre-op with my PS on July 30. I'd like to go much smaller than I am now, but have no idea about sizing, implants, etc. I'm hoping she can help me sort some of that out. I will definitely shoot her a PM. I'm already on the pic forum and, I agree, it's been very helpful.
Since my dx, I've pretty much stayed in my own little cocoon over on the TNBC board, but I figured it was time for me to venture out. I'm looking forward to meeting all the other August surgery sisters! I'll have to pop over to the other threads as well.

Hey Ladies I'm just jumping in to give ya'll a helpful list for those getting ready for surgery....
Fill scripts before coming home so you won't have to stop on
the wayMove commonly used items, kitchen tools, etc to low shelves
before surgeryHave plastic cups and plates - you won't want to lift any
glass or platesHave remotes close to recliner or bed so you won't have to
get up or reach too far - I slept in my recliner For a week!!Find NON child-proof caps for meds
Have constipation meds ready
Have pillows for the car-ride home. Have extras handy for
propping up at homeLanyard (like for ID cards) for holding drains and or Fanny
pack for drainsshower chair and shower head extension
biotene for dry mouth right after surgery
Lip balm / lotion
Mp3 / Music player / Tablet
Notebook for managing meds, etc
Phone / have friends' numbers handy
slippers / flipflops
camisoles/tanks which can be stepped into (as opposed to
pulling over-head) buy 1-2 sizes biggerSilky bottoms/PJs to make getting in/out of bed easier
button up shirts
zip up hoodies/ warm ups - pull on, easy pants
Jog Bras (Natori is recommended) or whatever support is
suggested by your surgeon - I didn't have a bra of any type my PS said not to use
them - check with your doctorgrab all the
dressings, bactrcin ointment, alcohol swabs, plastic bucket, etc from the
hospital that you canYou can also scan trough this thread for more

Allydp, thanks for starting this topic. Fantastic news that you are finishing up with the chemo next week. I did not have to go down that road and always thought that would be the most difficult part of this process. You are a trooper and are going to do great through surgery! We will be here to cheer you on!
Ally2345, I'll be right after you with my exchange. I don't know about you but im ready to get these coconuts out and move over to the squishy side! I had my dx on March 6th and will have my exchange on August 13th.

Allydp, thank you for starting the August surgery thread! I will be having exchange surgery on 8/8. The rest of us here so far have been through the whole BMX thing so don't hesitate to ask any questions! You'll do great. You've been through a lot and made it through chemo so I'm hoping this will be the easier part part for you.

Yay!!! An august surgery thread!!! I am also having neo-adjuvant chemo and am currently in the middle of my post steroids insomnia attack. I came on to start one and was happy to see this!
ALLYDP we seem to be on a very similar schedule. My last chemo is 7/18, my pre-op testing is 7/29 and my surgery is 8/11. Because I have Inflammatory BC, my reconstruction will be 6 months after my last rad. So no TE's for me. My skin was involved so a lot of it will be removed they don't want the remaining skin to be stretched over TE's and hit with rads. Anyone else not getting TE's?
I'm nervous because I feel like 3 weeks is not enough time for my body to heal after chemo and be ready for surgery. But on the flip side since the moment I was diagnosed I've wanted these girls gone.so many mixed emotions!

Iwannaseeyoubebrave and Juliecc - thanks so much. That's what I keep telling myself...hopefully the worst is behind me and surgery will be the easy part. Congrats on your upcoming exchange surgeries! Hopefully the worst is behind you both as well and it's all downhill from here!
Jules - we are definitely on a similar schedule. My last chemo is July 16, post-op July 30, surgery Aug 18. Congrats on getting through chemo! I'm so glad this phase of treatment is coming to a close. I had to laugh when I read your post...I too am in a steroid crash right now. Insomnia, mood swings, the works. Fun, fun. Sounds like you have a great plan waiting 6 months for your skin to heal. I've always wondered if I should be doing the same since I'm doing rads. As far as 3 weeks being enough time for your body to heal, my BS was going to schedule my surgery for sooner, but I've had very low blood counts during the second half of chemo. She pushed the surgery back to 5 weeks out. So I would say as long as your blood counts look good, 3 weeks should be fine. I'm with you on all the mixed emotions too! I'm so ready to get this cancer off my body, but I'm scared of the recovery.
For all the ladies that have been through the mastectomy with TE's, how was the pain when you woke up from surgery? I'm having a nerve block to avoid taking as much morphine. I'm nervous about it doing its job and keeping the pain at bay.

Thatnkfully my counts have been decent. I did have a severe drop in my WBC just before taxol #4 and had to be hospitalized. It's been borderline ever since along with my liver functions. That's why they are waiting 3 weeks so my counts will get better for anesthesia. Will be getting any scans before surgery? In having an MRI next tues.

Congrats, Ninany. Interestingly, I never had any bloodwork or EKG before my BMX surgery.
Allydp, I had very little pain and no nausea when I woke up. Two things, when you meet with the anesthesiologist, ask for an ear patch to put on before surgery to prevent nausea. Also ask for anything that can be given with the anesthesia. I told mine that I get carsick very easily. Between the ear patch and whatever she gave me, I was totally nausea free. For pain, my plastic surgeon uses this stuff called exparel, that she injects into the incisions at the end of surgery. It provides 3 days of pain relief. I went home the same day as my BMX but that isn't the norm. I felt good. I woke up feeling happy and "high" LOL. Unfortunately, many nerves are cut so you will probably have very little incision pain because the chest area will probably be numb. Most of my pain came from the armpits and drains near the armpits. I was still able to switch to tylenol after about 3 days if I'm remembering right.

Allydp- Most of the ladies will tell you to stay ahead of the pain...take your meds as soon as you can, even if it means setting an alarm. If you do that, it is uncomfortable at times but not awful. I agree with Juliecc, the most painful area for me was one of my drain sites. It is still sore and sensitive two months later. I did not have the exparel but those that did, rave about it so do check into it. I was on extra strength tylenol by the 3rd day.
Ninany- so jealous you get to do your blood work and stuff today. It makes it seem closer when you are actually taking those steps!! I am counting down the days til my pre-op meeting with my PS...8 days! Woop, woop!!
Jules_NY- Another August 11th sister! Juliecc is going to blaze a path for us to the squishy side on the 8th!

Yes, Ally2345! 25 days to go. I'm excited I am getting this over with. A couple of my drain holes are still sore when I am rubbing lotion on them. My PS said I have a 50/50 chance of drains this time. She will also do fat grafting from my stomach to fix a dent in my lower cleavage area.

Mods- Could you post our list toward the top of the thread? I noticed other months have it pop right up and it is easy to review and see who is coming up for surgery.

Jules - so glad to hear your counts are stable, but sorry you had to be hospitalized. I had a couple trips to the ER for count issues as well. No fun. I won't be getting any scans prior to surgery and, in fact, I won't have any scans post treatment either. My onc was just telling me the other day that she only runs scans if I present with symptoms. That makes me a little nervous, but I'm sure I'll adjust. I did have a mammo/us half way through chemo and they couldn't see my tumor at all, so I'm hoping they don't find any cancer at surgery either. When is your MRI?
Ninany - welcome! Yay for getting rid of those expanders soon!
Juliecc - thanks so much for the suggestion to use Exparel! I googled it and it definitely sounds like an easy thing to do to help control the pain. I'm going to request it from my PS. I also plan to ask for the anti-nausea meds. When I had surgery to place my port and sentinel node biopsy they gave me something that worked really well. I hope you're able to skip the drains this time around!
Ally2345 - thanks so much for the suggestion to get with Whippetmom! She was SO helpful! It's awesome she takes out so much of her time to help everyone here.

Allydp- Whiippetmom is the best!! I am researching Exparel as well. I had never heard of it before some of the posts on these forums. Thanks for trying to get all of our dates on your initial post. It will be nice to add people as they find us.

I don't think everyone uses Exparel because it costs about $3000. My kaiser insurance covers it. It is definitely worth asking about.

Allydp- my MRI was today, I'll get the results on Friday when I meet with the BS. The borders of my tumor were never really visible on anything other than the MRI, IBC grows in sheets kind of. It didn't show up at all on mammo actually. The goal is a smaller tumor but like you I'm hoping for no cancer cells!!!! Glad you mammo/sono 1/2 way through was normal that's awesome!!!
To all- Just curious to know when is/was your first follow up after MX? My surgery is 8/11 and my follow up is 8/28???? I'm going to ask when I meet with the BS on Friday. I think it was an error

Jules_NY- I met with my PS on Thursday after my surgery on Monday. I saw the medical oncologist about 2 weeks after. I have not seen my BS since the surgery. I have no idea if this is typical or not. I was a reconstruction with TEs BLMX and did not have to have chemo or radiation so my doctor visits may be different. With my exchange they have me coming in to see the PS two days after. It seems that over 2 weeks is a bit long because you can probably get rid of a drain(s) after a few days. However, after reading the many posts, the one thing I have learned is every doc is different. That being said, I would still ask.

Wow...a lot of exchanges in August! Since some already went down the BM path, fire away any questions you may have!! Mere provided a great list of stuff to have on hand...I had my drains for almost 3 weeks, but I just safety pinned them to my surgical bra--very easy...I had the little pouch thing but only used it when I got out of the shower--it wasn't comfortable to wear all day. The little alcohol pads worked best for me on the drains....Thanks for the warm welcome!

Juliecc - wow, that's pretty expensive. I'm assuming my insurance would cover it though under the surgery. I hope so at least.
Jules - I will be thinking of you as you wait for the results of your scan. My first appointment is 9 days post BMX with my PS. I'm thinking that's quite a long time too. I'll have to ask about this at my pre-op on the 30th.

Here's what my PS said about Exparel: Make sure your anesthesiologist is experienced in administering it. She said that if it's improperly administered, you can suffer permanent nerve damage. It is used frequently, so it's not like it's experimental or anything but when you have your powwow with your anesthesiologist make sure you ask if they are comfortable with it. I didn't use it, my anesthesiologist wasn't experienced enough but the general anesthesia was fine for me, no nausea etc.
