This is my first post, though I have been following this thread for some time.  I had BMX July 2011 with TEs placed and exchanged for Mentor smooth round silicone implants in October 2011.  I'm moderately happy with them, but I have no upper pole fullness and my sternum is very prominent and very visible in my clothes.  My PS says I am the perfect candidate for exchange to Allergan 410 Natrelle implants and that will give me a more natural shape.  Has anyone done this for these reasons?  Are you glad you did it?  Hoping to see before and after pictures at some point.

    Hi PinkWalker,

I had a nipple sparing BMX in June of 2012 with silicone round implants.  They looked pretty good, but I had some rippling and asked my PS to see what he could do to fix them.  So in March of 2014 I had my implants swapped to the Allergan Natrelle 410 Highly Cohesive anatomically shaped silicone breast implant.  They look so much better and I am happy that I went through with this second surgery.  I have really nice cleavage in tank tops.  The negative is that they are firmer than my previous implants and I need to get used to that, but strangely they are more comfortable than the silicone rounds.  Good luck with your decision and I hope all goes well!

I switched. Originally, I chose a smooth round 339cc Allergan Naturelle 15 for my Left skin & nipple sparing mastectomy. Nothing done to the right side. No anatomicals were available outside a clinical trial at the time so not an option. In my case, the shape of the implant was so different than the natural side that it never looked right. Just too round, too much upper pole, too flat and noticeably different under workout tops unless I put a push-up pad in the 'good' side. Even with fat grafting you could see the top edge of the implant so depending on how low cut the top was, it was a problem. Also, the rippling and distortion was a huge factor that you could see in my spaghetti strap work out tops. I favor halter top bikinis and the roundness on the one side was noticeable. And kind of fake looking. Naked - definitely a huge difference. There would be no fooling anyone in a locker room. And I hated it.

Revision surgery: Switched to a cohesive implant - 330cc Sientra moderate profile. Definitely more symmetrical, even naked. Both BS and MO commented on improved result when they examined me. Better contour, closer to my natural shape but projects a bit more than natural breast due to a bit of ptosis so naked it's not a 100% perfect match. In a bra, perfect. it slopes so no edge and more natural upper pole fullness at the top of the implant. Almost no rippling, and I would have to wear a very low cut workout top or bathing suit to see any of the much lessened distortion. But it's a definitely a firmer implant. I feel it more working out which I'm definitely not as crazy about.

Even if I had a BMX, I would still choose the anatomical implant. My PS likes the Sientra because they are softer than the 410s. He said has removed the 410s because of the firmness issue. The smooth round implant looked unnatural on me and rippled far too much. Even if I had a BMX, the rounds just would've looked fake on me

NHGirl and blanviper, thanks so much for your replies!  I was starting to get discouraged about not getting any feed back.  It's good to know that you are both happy with your decision to undergo a second exchange.  I'm very torn.  On one hand, I'm pretty sure I would be happy with the cosmetic result and on the other hand, I don't want to tempt fate, as I have had absolutely no complications, and pretty much breezed through all of my surgeries, despite concerns about previous radiation.  My PS is very confident that this is a good option for me and has no concerns about the outcome.  At this point, I'm leaning strongly toward going for it.  I'm not thrilled about having drains again, but they weren't all that bad, so it's a minor concern.   I had inquired about the anatomicals at the time of my first surgery but they were not available to my PS at that time.  He says now he uses them for 90% of his surgeries.  Thanks again for your input.  I'm so appreciative!

I have 410s - I had BMX direct to implants on 3/19.  Since I have no experience with the rounds, I can't make a comparison, but I really like them.  I didn't know what to think of them at first, but they really have a nice shape now and the projection is actually better than I thought it would be.  Tomorrow will be 11 weeks and I've seen some changes even in the past couple weeks and they've softened up quite a bit.  They don't seem too firm to me because I had such dense, lumpy breasts before that the implants feel pretty good and are comfortable.  I really wish I'd taken weekly pictures for comparison purposes.  I have a lot more upper pole fullness than I ever did with my real breasts and the implants are larger, which is what I wanted.  Some people have asked me if I went a little smaller, but that's only because I no longer need to wear a padded bra like I did for the last 30 years.  lol  I'm glad my PS suggested the 410s; I'm happy with them.

ufcmom, thank you for your input.  I don't think the firmness would bother me for the same reason you mentioned.  Are you able to wear a regular bra?  My rounds don't really fit into a cone-shaped bra so I still have to wear padding to make me look "normal".  I'm not a youngster by any stretch of the imagination, and my DH of 45 years loves me in any shape or form, but I still want to look and feel good in my clothes.  Thanks again!

I went bra shopping the day before I went back to work and it was less than 4 weeks after surgery.  I found one that fit kind of OK and bought it in 2 colors.  I wore it the first day and I felt like it was cutting me I'm half.  And I only worked 4 hours that day!  I couldn't wait to get it off and I haven't even tried it on since.  It's very soft so it was just me, not the bra.  All I wear now are Genie bras during the day and sports bras to bed.  I'm going to wait two or three more months before I attempt this whole bra thing and will go somewhere to be fitted. Last weekend I went braless both days (first time for an extended period) and loved it.  So I will likely only wear one when I have to.  I'm still more comfortable wearing a sports bra when I sleep though.

    Hi PinkWalker,

When I had my implant swap in March 2014, I did not have any drains put in as I did with the BMX in 2012.  I had the swap surgery on a Friday and was able to go back to work on Monday.  My PS required that I wear a sports bra 24/7 for the first two months.  Now I wear a regular bra during the day and no bra at night.  I could go braless but I've never done that before but may give that a try.  But just wanted to let you know that for me, the implant swap was so much easier than the BMX.

    Hi ufcmom,

I think one of my biggest surprises was that bra-fitting would be an issue.  Love the idea of going braless and just wearing a soft cami.  Thanks!

    Hi NHGirl,

My implant exchange from TEs was pretty easy.  No drains, little pain, no big deal.  My PS has said that the worst thing about doing another exchange is that I will have to have drains again.  Not fun but do-able.  I'm probably going to go forward with this, but am having a hard time committing to a specific date because of other scheduling conflicts.  Wow!  You did really well to go back to work 3 days post-op!  Don't know that I did that well!
