Complications after fat grafting

I posted recently about lumps which developed 10 days or do after fat grafting.  Over 2 weeks later and they are still there.  PS says it will take months to know if they will go away on their own.  Surgical oncologist says they are concerning because of their location along the scar line of SNB, so we can not wait months.  If not gone in a few weeks, he wants me to contact him so he can address them. They feel like cysts, which may be fatty cysts from fat which did not take.  Has anyone had this?  Did they do ultrasound, fine needle aspiration, or surgical removal?  Just trying to get an idea what I'll be on for... Lumps are getting worse, not better.  There are 4 or 5 of them.  This was supposed to have been my last surgery.  I should have been done.  :(

    kbeee - sounds like they could be fatty lipomas, has there been any discussion about that? Or fat necrosis? I have not experienced this but did have fat grafting. Depending on what imaging shows (I believe that ultrasound can distinguish) they have a variety of ways to remove them. I know I have read of others having this happen, I don't think it is that uncommon.

    KBeee - I have had fat grafting twice.  On the first go round, I developed two lumps, along the scar of my BMX, in close proximity to one another.  One was about nickel sized, the other a tad smaller.  I did not have any testing on them.  My PS said we would keep an eye on them.  My MO agreed with PS.  Both felt they were fat necrosis.  They are still there, but feel like they are getting smaller.  Hoping yours are simply fat necrosis as well.  Any new lumps are scary!  Sending you hugs!

    Researching leads me to believe they may be lipomas or fatty filled cysts...or at least that's what I hope!  SO (surgical oncologist) said they were concerning,but not alarming...but if they are concerning to him, then they are alarming to me!  They feel like  a bunch of grapes.  There were 3, then 4, now more.  PS said lumps occur in about 5-10 percent of cases, but not usually this soon.  PS and SO work together often, but they don't always agree 100%.  MO just said to keep him updated on what SO decides.  I hate the waiting game  I am supposed to wait 2 more weeks, but I see PS next Monday.  If they are still there, I am going to call SO and play dumb and pretend that the 3 weeks he told me to wait was 3 weeks from when they appeared...at least maybe I can find out more of what the plan is to address them.  Wait and see versus ultrasound versus needle aspiration versus surgery all have big bearings on my work schedule and summer plans!

    anxious to hear how it goes!  I had fat grafting 2+ weeks ago and just noticed a new lump.  Had a clear mammo just before surgery, so I'm sure it's nothing.  Don't like feeling lumps though!

    SophiaMarie, I'll keep you posted.  I've got about 5 lumps now, and my biggest concern (other than the remote possibility of BC again so soon) is that I will not be able to assess for new lumps while these are here.  Let me know how your lump fares.  I hope it goes away on its own.

    I developed 4 lumps about 2 months after my exchange.  I was told they were consistent with fat necrosis, and have a 6 month follow up ultrasound to check on them.  When I saw my PS a couple weeks after my ultrasound and BS appointment, he said that the location of the lumps is "where the alloderm went in" and that it was possible they would go away by themselves.  As of now - 3 months out - I can no longer feel 2 out of 4.   I did have fat grafting during my exchange, but only on one side, and these lumps appeared bilaterally.

    I see you're having problems with lumps again! Luckily fat necrosis is B9.

    I'm having FN problems with reduction. Will these breasts (original or reconstructed) ever stop bugging us?

    my MO said that it didn't feel like a breast tumor - he said he thought it was fat and was not concerned.  I will just watch it - I see the PS next week and my RO in July.

    SophiaMarie, Great news!  I see my PS on Monday and then have to call BS back later that week or the next week to decide on what action he wants to take.  We'll see.  I am sure they are likely not cancer, but there are a lot of them and they are in the area where cancer would be the most likely to recur, so I will not really know if anything changes.

    I do think our life now is always watching and waiting and worrying.  I went to PS today and he agreed that the lumps seem a little bigger and/or more numerous than last time.  He told me to contact BS today and not wait the extra week (BS had said call back in 3 weeks if they're not gone) so that he can set up an ultrasound to double check them.  I called at 9 am this morning.  Here it is 3:30 in the afternoon and no phone call back from nurse.  That is unfortunately the norm, but I know from making a second call once prior, the nurse will have a hissy fit and yell if I call again. This surgeon is the best around here; I just wish he had a nicer nurse with a better track record for returning calls.  I hate this waiting and wondering if my message ever got through.

    I went through this and the lumps were oil cysts from necrosis that developed after reconstruction.  They seem to be resolving on their own after about 6 months.  It's great to have the imaging interpreted by someone who understands reconstruction.  Best wishes!

    They do kind of feel like cysts, so I am thinking (and hoping) that's what they are.  I just wish my doctors would quit messing around and just order an ultrasound to confirm it.  I wish I had never had the fat grafting done because the lumps are right where the greatest concern would be for a recurrence and I am so worried about not being able to find new lumps.  I am so glad to hear that yours resolved on their own!

    LAstar, Do you remember how long after fat grafting your lumps developed?  My PS seemed surprised that I had so many lumps develop so soon after surgery, all in this one area.  It also makes me wonder if it is just a bunch of swollen lymph nodes.

    My cysts popped up a year after surgery.  In fact, there were several women who'd had reconstructive surgery done about the same time as me that also had oil cysts develop at the same time.  My BS ordered an MRI to determine what mine were.   Three weeks post-BMX is exactly the same time that I noticed my necrosis though.

    ugh.  I didn't realize this came with it... So along with the excitement of getting repaired comes cause for more anxiety.  I hope it's nothing scary Karen!

    BS did a fine needle aspiration biopsy yesterday (first they were going to wait another month and I told them not a chance...I was getting a second opinion).  I should have the results tomorrow.  Hoping and praying for confirmation of fat necrosis.  I really just want to know exactly what it is so that I can know when to expect it to go away.  I do not like the stress of extra lumps and bumps.

    Good for you!  Yes, lumps should not be left unchecked -- month, schmonth!  Fingers crossed for great news (tomorrow!  not Tuesday!) and a relaxing holiday weekend for you.

    I finally called. Rotten nurse who is so mean had left for the day, so I talked with a nicer nurse, though not one who knew anything about me. First she told me the results weren't in; I asked her to look again. They were there, just with a wrong date on them. Pathology said benign, with no abnormalities noted. There were a lot of lymphocytes. She did not know what they meant other than maybe inflammation. It was not fat necrosis, not a fluid-filled cyst, not a fatty cyst, not a lymph node, so I do not know what it is. I do know what it isn't - cancer. Perhaps just inflammation. There are several lumps and he just sampled one, but I am confident they are all the same. I see him in a month and can ask more; I also asked the nurse to send me the results so that I can dissect them further myself.  I wish I had a specific answer as to what the lump is, but I am relieved about what it isn't!

    I was hoping to find out that the lumps were oil cysts, fat necrosis, or traditional cysts...or maybe swollen lymph nodes...no, no, no, and no.  At least they are not cancer.  I am anxious to get my paws on the pathology report!

    I saw him about setting up the next surgery - I actually forgot about the lump.  Until last night when I felt it again and now hope the mo was right.  I don't like that it's so close to the old tumor site.  Anxious to know what your path report says!

    Hi,

    I would like to hear from others who have had fat grafting.  I had it done on Friday and I had no idea that my breasts would be so swollen!  Has anyone else experienced this?  How long did it take to go away?  It is very uncomfortable and sometimes painful.  I expected my stomach to be swollen but it is not too bad.

    Julieped, My breasts were swollen for about 10-14 days.  My lumps appeared around day 7...most people do not get lumps, so hopefully your swelling will decrease here in the next week or so, and you'll be more comfortable soon.  I was more bruised and swollen than I expected.
