Switching Plastic Surgeons Midway through Two-Step Recon.

I am about 4 months post PBMX surgery with tissue expanders. I have been filled completely (maybe even a little past ideal). Dealing with my PS has been somewhat of a challenge. Let me first say that his staff is amazing and they have kept me hanging on so far. It's just that every time I deal with him, it is not a positive experience.

He is a very well known Board Certified PS who is known for doing great work.  That is why I chose him.  He has very specific ideas about everything and is not very interested in hearing about how I want to look.  I feel that he can be condescending on the border of rude, he rushes me out, and does not answer all of my questions.  I am just not sure that I want to keep dealing with him.  He is set on using an anatomical implants that may look just fine, but most ladies on this board (thanks girls!) do not seem to like the way the implant feels in their bodies.  He dismisses that as ridiculous.  He wants to put me in implants that are much bigger than I am comfortable going and says that my body shape requires it.  I really hoped that this process would be much more give and take than it is.  I am feeling really frustrated.

However, I am not sure how it will work with the insurance company as my second surgery is already pre-approved. Also, he does do very good work.  How would you guys deal with this?

Any opinions appreciated and thanks in advance.

    I changed PS midway through my reconstruction.  I was not happy with my first PS bed side manner.  The PS was condescending, did not listen to my concerns, and even walked out on me one day because I cried.  Last straw for me.  I switched PS and it was the best decision I ever made.  Remember that the PS works for you.  You are the one living in your body for the rest of your life.  You need to be the one calling the shots.

    I think above all it's important that you are comfortable with your PS, and it sounds like you aren't.  In my case, I also have a well known, board certified PS who is known for doing great work.  He was recommended by my BS.   I met with him not really knowing what I wanted - it was the least of my worries at the time.  I told him approximately what I wanted to do as far as size and he recommended anatomical silicone implants.  He said they were a lot more natural looking. 

    Shortly before my exchange, he was measuring me for the implants and getting kind of aggravated because all the anatomicals were very wide.  Too wide, he thought, for my chest.  We ended up going with rounds instead.  I am so happy the anatomicals were too wide.  I am not a fan of the shape or the fullness of the upper part of them.  I think that on me, that would look ridiculous.  The rounds much more closely match what I had before as far as shape and projection - quite a bit bigger, but that's what I asked for.  If I want the upper fullness, I can get that with a push up bra.   I guess where I am going with this is that I had a lot of confidence in his vision for me and his ability to see it through.  I felt like he was the one who went to medical school, and unless something really didn't sit right or make sense to me, I was going to defer to his judgment.   I went into this not knowing much (if anything really) about reconstruction.  I've learned a bit since, mostly from hanging around here, and if I could do it over I might ask some additional questions to get clarification about why things were done a certain way, but I don't think I would change anything.

    If you aren't comfortable with what your PS is telling you, try a second opinion with someone else, just to hear their thoughts.  You don't have to tell the original guy you are doing this unless you decide to switch.  I'm sure the insurance company can deal with switching the surgeon, if needed.  You have some time - the TEs can stay in as long as it takes to get to the point where you feel good about the direction you're headed in.  If you haven't already checked out the thread Breast Implant Sizing 101, do so.  Whippetmom is the expert on all things implant related and could probably give you some very good information.

    Just a warning - if you are currently in treatment with one PS - and it would be obvious you are if you have expenders in place, you may find reluctance on the part of a new PS to give you a "second opinion" unless you actually fire the original PS before you see a new one.

    I also switched PS after my fills and am having another do the actual implant surgery.  I was not comfortable with the old PS because of his practice of going into the OR with only one size implant.  He does not use sizers.  He just told me to pick which size I wanted and that would be what he would use!  I went to another PS in town and found myself "selling" my case to her.  She really did not want to take me away from another doc.  I had to assure her that I had had several discussions with him about my concerns etc. etc. I finally got her to take me as a patient but not without a hard sell on my part.
