punch skin biopsy

Today I'm having a punch biopsy can this detect ibc.. I'm a wreck..
-
A skin punch can detect IBC IF it is there in the area checked. My IBC was DXd via FN biopsies of an enlarged node under arm and some areas in breast. My skin did not show any abnormalities thus nothing for a skin punch to check.
-
If the punch biopsy comes back negative then they r going to do fine needle biopsy in about 6 different areas because I have such bad discoloration. Pain and swelling.. if u don't mind me asking what were ur symptoms.
-
You will have to ask them what comes next (if anything at this time). I am not a medical professional at all but I would doubt that FNBs of discolored skin would be done after a negative skin punch. There are so MANY possibilities for skin discoloration along with pain and swelling. Based on what I've read/been told, IF IBC is present, it will show up in the biopsy of an area showing what might be 'signs' as the lymphatic system is involved.
I found an enlarged lymph node under arm one night that had not been there the night before. It was about the sze of an almond. My PA was on vacation and as I only wanted to see her, it took me a week to get in. In that week the node greatly enlarged. I saw her at 11 and she had me at Radiology Clinic at 1 for a new mammo which turned into also a sono and biopsies. I left there about 3 and had the path. report at 8 the next morning - as expected IBC. In the next 17 days til I started neoadjuvant chemo, my breast increased, peau d'orange developed and nipple inverted. I never had any pain/discomfort/itchiness or discoloration. We are each so different so there is no exact as to how our body reacts to anything. What my body did is not the same as all do, just as my experiences are not exactly what anyone else has experienced.

Thank u.. I've gone through 4 doctors with 4 different dx until I was referred to a dermatologist Who did blood work to check for a specific infection.. at that time he got on the phone with a breast surgeon .they decided
If my bloowork was negative to do the skin biopsy.. if the skin biopsy is negative then they will do the needle biopsy.. anyway my pain and itching is deep It's not like a top of the skin itch if that makes sense but I've been through 3 rounds of antibiotics and 2 rounds of steroids which did Nothing... I'm just in so much pain and I just want some answers.. It's been rough

Tmess,
There are no hard and fast rules with IBC. I didn't have enough skin involvement to warrant a skin punch though my nipple was inverted, etc.. Mammo and sono showed 3 masses in breast. Yupe, it was IBC with lymph node involvement: at first, needle biopsy showed only 1 axillary node as involved, later, MRI, added 2 more 'shadowy' nodes. Number of nodes seemingly doesn't matter with IBC: anything but axillary lymph node dissection is counter-indicated, though I've been told that the decision as to do an ALND as opposed to sentinel dissection will be a OR decision. I said nope, if I'm going to agree to anything less than what is recommended, I want a damn good reason why.
The point in this meandering post is that Kicks has hit the nail squarely on the head with her remark that we are all so unique. Or, to put it another way, IBC's typical presentation is never typical. And that's why a quick response biopsy is so important. Personally, I'd want more than a skin punch, if only to be clear on what was going on. And I'd refuse to wait on bloodwork: IBC is very invasive and aggressive. IMO, it should be aggressively DXed. Within 3 weeks of my DX, I was getting my first chemo. But that's just my opinion: we really are all so unique (thanks for saying it so well Kicks.)

Thank u all skin punch was negative as I suspected.. now timing havinfection a needle Biopsy.. I won't Stop until I have some answers I'm very swollen under my arm and in my ribs along the bottom of my breast.. I've been so frustrated because I feel deep down this is ibc but No one wanted to listen.. now finally they r thank goodness.. I'll keep posting
-
I pray that this is not IBC. But if it is, we're all with you.
-
I feel for you. I'm having a hard time getting diagnosed as well. Had a couple days of itching. Sent to surgeon who said it wasn't ibc but sent me for screening mammo. Screening mammo came back with area of concern. Meanwhile, all my lymph nodes start getting huge and breast gets more swollen. Diagnostic mammo and ultrasound came back with "dense tissue". That was two weeks ago. Meanwhile I still have huge lymph nodes, pain in chest wall, and breast. Swollen lump nodes causing tingling and numbness in arm. Went back to primary care physician, who is sending me back to surgeon. He said to insist on a biopsy of at least the lymph node in arm pit but also on the "area of concern" on breast. 2nd appointment with surgeon is tomorrow. Wishing biopsy was tomorrow too but im assumjng it won't happen until next week. Meanwhile, I've been trying to get someone to listen to me about IBC fors month.
-
Thanks for the support and I know how u feel.. I've been at this for two months don't give up.. Why didn't ur primary order the biopsy.. I'll keep u posted
-
So pissed No biopsy because I don't have a visible knot on ultrasound.. these radiologists r so damn irritating. Do they not know that u don't always have visible tumors for there to be a problem... Ugggghhhh
-
Tmess,
Without a target, it is impossible to do a meaningful biopsy. If your docs have no positive indication that it isn't IBC, I would hope they would do a lymph node work up. Of course, since my IBC presented with no pain or signs of node involvement, I was pretty freaked out that there was such involvement.
Still hoping you don't have IBC,

OK surgeon called today to basically tell me to deal with the pain and swelling. He wasn't doing anything else until I developed a mass or symptoms got much worse so wtf do I do now.. I thought doctors were supposed to listen I know there r other tests besides a biopsy.. I just want to know what the underlying Cause is.. I'm so damn upset.. I guess I just wait and see.. they all say It's an inflammatory problem but none of the inflammation drugs work so what does that say..help please
-
Tmess,
Why are you dealing with a surgeon rather than an oc who specializes in imagery such as mammos and us? If I was as certain as you are that it is IBC and was getting no useful info from my radiologist, I'd find a medical oncologist. IBC is so rare and weird that, personally, I'd want more than a surgeon's DX. But I also had palpable masses in my lymph node and breast so getting a DX was fairly straight forward. Like many of us, I had never heard of IBC -- given that out of 250K new breast cancers reported yearly just in the USA, only 12,000 are IBC and I had no particular reason to suspect I had it. I absolutely understand your fears and hope you can get a solid DX soon. Keep us posted.

I have been through my pcf, a dermatologist, an infectous disease doctor and a two surgeons.. my only mammogram was the first week this started and both us consisted of one spot on my breast.. the radiologist said both we OK to recheck in 6 months.. my thing is something is causing this severe pain swelling and bruis
-
Finally getting an MRI hopefully this will tell me something... I just want some answers..
-
OK MRI showed a large spot skin thickening and a small spot they say It's all benign and all this swelling and pain will clear up on It's own.So iI'm still like wtf...I just want some relief.. I now have two more sores that are about the size of a dime.. my breast is black in spots and my stretch marks r a deep purple.. right now I'll take any advice I can get.
-
skin thickening? That is something that should be checked out. Are you seeing breast specialists?
-
I'm trying.. the radiologist says It's all OK but there is skin thickening on MRI.. I'm going to see a oncologist hopefully next week that specialized in this.. she treated my mother in law and I trust her opinion.. I have a large something on the MRI so hopefully I'll have someone willing to listen fin
-
I would definitely see a "breast" specialist..
-
Can u tell me what ur symptoms were
-
I had a red breast and skin thickening on MRI. My doctors found a 3.6 cm IDC tumor in my breast, but I also had cancer in the skin lymphatic system.
There is really no way to know. You just need to get it all checked by a specialist. I have also had 3 scares on the opposite breast that all turned out benign and were suspicious on MRI. So, you just don't know, but don't let it go. I persisted with the red breast when some doctors easily sent me on my way.
-
I need some advice I have a 8 cm cluster and a 2. Spot with marked skin thickening and they want to Rescan in 1 month.. again WTF.. Who do they think they r playing with.. do I worry now or later
-
Tmess,
You have an appt. with an oncologist in a few days, yes? I'd see what she says before I'd accept a rescan in a month. IMHO, everyone needs a medical oc and I'm relieved you're seeing one.
sending best wishes,
bride
