Ladies, do you like your anatomical implants? If not, why?

I am currently one month post PBMX surgery.  I have tissue expanders that are wide, flat, and very firm.  I have 250ccs in each so far.  

When I started this process, I was SO sure that I would want anatomicals.  However, from lots of reading and researching, I am getting the idea that women are finding the anatomicals way too firm and without enough projection.  I am wondering which implants out there that smaller framed women really love.  I have a 31 inch ribcage with 13 BWD.  

Please let me know your thoughts on this.

    Originally my PS recommended anatomicals, however, when it came time to measure me for them just before my exchange, he decided they were too wide for my chest.  We went with Mentor high profile rounds instead.  I think they give me good projection, they are nice and soft and look natural.  Very happy with them, overall.

    Thanks Sarahjane.  Did you have any fat grafting done to enhance the results?

    Yes, a little bit.  He used belly fat to smooth the transition between implant and ribs on my right side.  It did not add volume, just softened the edge a little.

    Up until a short time before my exchange, we were going with anatomicals.  Since then, I've learned that the TEs are specific to the type of implant - did not know that at the time.  I don't know what I had for TEs, but can only assume they were the ones for the anatomicals.  So I guess my PS would have had to do some pocket work to make way for the rounds.  Whatever he did, I think it looks good.

    FLgrl32 I am about 8 wks out and am having the same question you have.  Thanks for starting this thread.  My PS also said to use the shaped.  I was ok with it till like you I read about the issues with it being firm and not comfortable.  I have 280 cc in my TEs.  I saw your posts on 101 as well.  When are you looking to exchange?  Mine is end of May.

    Has anyone else had an issue with them flipping?

    I have Allergan 410 gummy bears and just decided this morning looking in the mirror that I do like them. I had a BMX with one step (direct to implant) on 3/19 so they will be three weeks old tomorrow.  At first they looked really smushed and flat, but yesterday I noticed the left one was starting to look pretty good. Today the right one looks like it's catching up and they both are starting to look "normal" which is nice!  

    I was actually hoping to go a little bigger than I was, but didn't have the patience to go the TE route once I found out I was a candidate for one step.  I knew I'd be swollen after surgery and the first time I looked at them I was afraid I'd end up even smaller than I was before after all the swelling went down!  They looked like big pancakes.  And then I started reading through some threads here and saw it mentioned a few times that the gummies don't drop and fluff like other Implants; what you see is what you get.  No wonder I was starting to feel depressed.  lol  BUT they really are continuing to change for the better as my muscles stretch to accommodate the implants.  And they are definitely "fuller" than my real breasts were, so that is a plus!

    Thanks ucfmom!  Do you remember which model you got i.e.  High profile, moderate, high or low height? Do they feel real firm? My TEs are so wide and flat.  

    KGSN - My PS put 250ccs in at surgery.  He waits until 6 weeks to do a fill.  Then only every two weeks.  I am sure he will want to wait awhile after the last fill, too.  Which puts my exchange at um... Never.  Hahaha. No worries, just looking to get this show on the road.

    They are full height, full projection.  They do feel pretty firm but to me they feel squishier than they did at first.  I still have one drain left in but hope to get it out tomorrow if I'm lucky.  My DH said we'd have to do a test feel while we're there and compare the implant sample in the exam room to me and see what the difference is.  He thinks mine will continue to soften up a bit.  I'm just happy I'm starting to feel more comfortable with them.  There really isn't a whole lot of projection.  It's starting to look like I'll have more than I did with my real ones, though, and I have cleavage now, which I only had before with special bras.  lol

    I just had reconstuctive surgery yesterday. 

    My left breast is the mastecmastectomy side. I have a silicon implant. At the moment the breast looks a bit flatter than I expected. Wondering if the shape willpossibly round out w/ any more projection as time goes on?

    Mine don't have a great deal of projection, but they are still changing.  Tomorrow will be three weeks and as of this week, I've started noticing changes for the better.  They are looking a lot less flat and definitely fuller.

    Thanks, ucfmom! The width of my breast fits my old bra but the cup has a bit of room. I am hoping for some of the changes you are seeing.

    FLgrl32 Mine put in 160cc and 200cc to start but I only had to wait a week to do a fill.  Then every two to three weeks but he evened them out over the first two fills.

    hi,

    I had a unilateral mastectomy 10 years ago using the lat flap procedure. At the time the implant used was a sientra round gel. Last August I underwent a prophylactic mast on the non cancer side and had the old implant exchanged. I have the anatomical 410 and I love them. On me the result is much more natural looking. They are firm but so was the cohesive gel I had before. They definitely do not have the give and feel on my body of real breast and I know from experience they never will. However, you will grow less aware of this as time goes by.

    I have 410s and they look ok.  My problem is the expanders looked better because when the doctor did the exchange my nipple somehow got moved to the side and up so now the are very uneven.  I wonder if this is a result of the skin adhering wrong or the scar pulling.  The doctor said I was to critical.  I wear a 38d and with clothes they look fine.  I think the reason it looks bad besides not match, like it did with expanders, is that the nipple is not on the point but is on the flat side so it seems to really point outwards.

    I am in the filling process now but I've had anatomical implants before, prior to my mastectomy. I did't like my anatomicals at all because they projected too much. They felt huge, and everything I had heard about a nice slope and natural droop definitely didn't  apply to me because they were pointing straight out with no droop at all. And I really want natural looking breasts. Actually I think it depends on how ones ribcage is shaped and whether the inframammary fold is located high or low etc. In my case, I have a quite long torso, and the breast folds quite high up, so on me everything probably looks like I have my boobs under my chin. 

    Anyway I did some studying and found that the anatomicals project more than ordinary rounds, in relation to the diameter of the implant. So now I'm hoping for rounds with a fairly low profile, and a rather wide diameter, I think that will make some nice buns:)

    FLgrl ~ I can say only that my PS said that almost every patient of his who had anatomicals ended up switching them out for rounds.  I have Mentor High Profile rounds, and I'm very, very pleased with them.

    I have had both types of implants. Originally, I chose smooth round for my Left skin & nipple sparing mastectomy. Nothing done to the right side. I got a 339cc Allergan Naturelle 15 implant. No anatomicals were available outside a clinical trial at the time so not an option. In my case the shape of the implant was so different than the other side that it never looked right. Just too round, too much upper pole, too flat and noticeably different under workout tops unless I put a push-up pad in the 'good' side. Even with fat grafting you could see the edge of the implant so depending on how low cut the top was, it was a problem. Also, the rippling and distortion was a huge factor that you could see in my spaghetti strap work out tops. I favor halter top bikinis and the roundness on the one side was noticeable. And kind of fake looking. Naked - definitely a huge difference. There would be no fooling anyone in a locker room. And I hated it.

    Revision surgery: Switched to a cohesive implant - 330cc Sientra moderate profile. Definitely more symmetrical, even naked. Both BS and MO commented on improved result when they examined me. Better contour, closer to my natural shape but projects a bit more than natural breast due to a bit of ptosis so naked it's not a 100% perfect match. In a bra, perfect. Not as much upper pole fullness and no visible line at the top of the implant. Almost no rippling, and I would have to wear a very low cut workout top or bathing suit to see any of the much lessened distortion. But it's a firmer implant. I feel it more working out . 

    Even if I had a BMX, I would still choose the anatomical implant. My PS likes the Sientra because they are softer than the 410s. He too has removed the 410s because of the firmness issue. The smooth round implant looked unnatural on me and rippled far too much. Even with a BMX, the rounds just would've looked fake on me

    I am only 2 weeks out from my surgery but have Allergan smooth silicone rounds. My ribcage is 29.5", my former breast size was 32DD. While I am still healing, and "dropping", I really, really like how they are looking so far. I went a bit smaller than I was before - will end up around a D, I think - but have more "fullness". They have softened up - and expect they will do some more softening yet - I would not want them any firmer. At their firmest, right after surgery, they would have been too firm for me.

    Both PSs I saw preferred not to use the gummies in MX patients because of firmness, shape, and texture. Both reported having done many revisions to silicone rounds on MX patients unhappy with the gummies, They did use them in augmentation patients regularly though and really preferred them for that purpose. However, I know some who had gummies used in their reconstruction and they love them. So who knows?

    hi. Are you still happy with the 410's? I am getting them the end of April and still feel so unsure. Do they soften over time?

    Well, I guess looking at my last post, I need to do an update and backtrack a bit....

    I ended up switching to 410s on March 12, 2015. So almost 11 months post-BMX/direct to implant (silicone rounds). The reason was to try and correct some rippling but...I am really, really liking the gummies. They really are NOT noticeably firmer whe I poke and prod them as I was led to believe (this may be due to the tightness my rounds were secured in there) and I like the shape much better on my frame (I am thin, athletic, and these seem to just suit me better). I don't experience ANY discomfort from their firmness. I expect that they are not going to be as "moveable" as the rounds were because these are supposed to adhere to my tissues but that is not a big deal for me honestly.

    As the 410s are a bit wider than my rounds were (to make a tight fit in the pockets) I also have nicer cleavage, IMO, than I did in the rounds. They look much more natural (and that was in fact the first comment my husband made when he saw them). I am as I said, quite thin, so the rounds were a little more, well, round, on me. Best part of course for me is that my rippling seems to be, at least so far, dramatically reduced. With the rounds, I was having it even while standing straight on the one side (the side my tissues are thinner). Now it looks like I get a bit on the same side with some distortion, or leaning over, but only very slight and because I am looking actively for it where I know it was before.

    Keep in mind I am only 11 days or so out, but they were pretty well placed from the outset (not the same amount of drop and fluffing to do). I ended up a 30E with my rounds, but expect I might need to bump to a shallow 30F with these ones to get the proper fitting underwire as they are slightly wider (projection is only about 0.3cm more though). The rounds were 435cc, the 410s are 450g.

    I'm six weeks post op and not really liking these anatomicals! Someone said there's no drop and fluff with these? My PS had no clue what I was talking about. I never get any feedback from her.

    I guess it all depends on how they were placed. I have definitely noticed some relaxing and better shaping in only two weeks, but I do not expect a dramatic drop and fluff to be as dramatic as with my rounds as 1. They are in a shaped shell and 2. my muscles are not as tight off the bat this time around so they started off a bit looser (not squeezing implant up and flat) and 3. they are textured and are meant to adhere to the tissues (to prevent rotation and displacement) and this happens within weeks

    In any event, they definitely do look different, the bottom is fuller, upper pole more sloping, than I was a few days out of surgery.

    I've looked at a few augmentation forums, and women with anatomicals do definitely experience changes over time (months, or even a year, not weeks). However, this is due to muscles relaxing and letting the shape of the implant itself show, not due to the silicone itself shifting in shell more to bottom as with rounds (which is what is usually meant by drop and fluff...so there ARE changes but the process is different.

    Many PSs do not know what drop and fluff is or means, that is more online lingo on my experience, and many do not see their patients day to day to see the changes.

    I recommend taking regular photos, as you will be able to see changes in photos you might not otherwise.

    I had my anatomicals placed in August and I don't care for them at all.  To me, they look droop too much.  They are fuller on the bottom and slope on the top kind of like real ones.  Not to be ugly, but in my clothes, they look like old lady boobs.  I was expecting fuller and more projection.

    I go back to see my PS in April.  I guess we are going to look at some revisions.

    That was funny! I'm glad you posted.  I am 56 and wouldn't mind that old lady look. I don't have that look yet. They made me a B cup. How long was it before you got the old lady look? 

    Did yours ever soften?

    I am sorry you are not happy with how they look ml. I think a lot of it has to do with their height, etc? Mine are moderate height, moderate projection, and don't look old lady like at all! They do look more athletic than my rounds, perkier than my natural breasts were, and I have much better cleavage, even naked, than I ever had with either my natural breast or round implants (though I am near the same size...natural was 30E, rounds were 30E, I likely will need a 30F when I am back in bras now as these are a bit wider, though only have 0.3cm more projection than my rounds). I've actually been a bit self conscious of my cleavage in some of my lower cut tops....don't need to give my co coworkers that kind of show!

    I hope you and your PS can figure something out that will get you the cosmetic results you are hoping for :)

    My 410s are now 13 months old and I love them. They now feel pretty soft and I think they look great. Just as importantly, so does my husband. :) They look like my real ones did, only fuller and slightly bigger. I definitely have cleavage now that I didn't have before. Very natural looking but not droopy!

    My 410's are almost eight months old and not firm at all.  They look natural and you wouldn't know I had surgery.  I love them!

    it may be a tad early for me to weigh in here as I just went through exchange last Friday. However, so far my expectations are exceeded. My real breasts were ~750 cc's. I had BMX 1.29.14. 500cc TE expanded to 520.

    My ps and I reviewed the literature on what size round vs anatomical would fit with my Te's and in the end I trusted him on what he would do. Interestingly, the recommendation for the silicone round that was much higher cc than the anatomical.

    In the end, I had naturelle 410 MF 525cc and so far I am very pleased. Definitely more slope and projection at the bottom. More projection than I had with the TE but not as much upper pole. He did a great job with cleavage, though. My husband even admits that they look much more natural, with great cleavage. I don't feel like I have old lady foobs at all. Just the opposite. I am 52 and my non perky 34 dds made me feel older! Actually, I put a sports bra on today and the ones I wore before still fit. Keeping fingers crossed it only gets better from here
