My wife had Perjeta for neoadjuvant chemo and will stay on it after her BMX. Our primary onc was wary of this as the benefit/risk factor for Perjeta in the adjuvant setting is largely unknown at this time. However, we got a second opinion from another onc at MD Anderson who specializes in Her2+, and he told us to stay on Perjeta as long as possible -- even if it's only a few more times after surgery. With his expert recommendation, our primary onc agreed to it. It will also likely fly with our insurance company as they initially approved a one year combo of Herceptin and Perjeta.

However, at this time, you still need to think of Perjeta after surgery as risky. This is simply because there is not enough data to illustrate the efficacy of Perjeta or the potential side effects of longer term use. There are actually clinical trials underway right now to determine just this. As for our treatment plan and because my wife is only 26, we've accepted the potential unknown risks. Save for radiation (which we're still not sure about), we're throwing everything we can at it. We feel further empowered with our decision as my wife has already had an outstanding reaction from Herpceptin and Perjeta in the neoadjuvant setting. So, because of our unique circumstances, we feel comfortable moving forward with Perjeta. Because our bodies, cancers, and circumstances our all different, Perjeta after surgery may not be the best route to pursue or it may be worth fighting for. At very least, it's worth talking to your treatment team about.

    Hope this help somewhat.

thanks Ben.  I hope your wife continues to have nothing but great results with perjeta and herceptin together.

It's worth the risk for me at this time. I have already had chemo and herceptin almost 5 years ago and I recurred in some residual breast tissue. :(.
