anyone diagnosed with ACH and FEA
Okay, I joined this thread two weeks ago when my pathology came back as dcis. That Air Force sent my sample out to another lab per their policy...that was good. When the second path report came back it said no dcis but ACH (atypical columnar hyperplasia) and FEA (flat epithial atypia). I've searched high and low on this site and can't find any info. The surgeon suggested a wait and see approach bringing me back in in six months. I'm okay with that. I have googled and only found a couple of random things that looked like research papers regarding this. Have any of you had this on a path report? For that matter have any of you hada diagnosis of dcis and then a retraction?

Cathy, good news that your pathology has been downgraded from DCIS.
The DIN scale puts FEA as DIN 1A (or sometimes 1B), ADH as DIN 1B, and low grade DCIS as DIN 1C. Intermediate grade DCIS is DIN 2 and high grade DCIS is DIN 3. I believe that ACH would be considered to be similar to FEA - in fact I've read that it's just another name for the same thing. Generally FEA and ADH/ALH are considered to be high risk conditions, but not pre-cancers and certainly not cancer.
FEA, ADH and DCIS can be difficult to distinguish from each other, particularly if there is only a small sample to analyse. From this article: Clinicopathologic Implications of “Flat Epithelial Atypia” in Core Needle Biopsy Specimens of the Breast
"FEA is distinguished from ADH by the absence of the aforementioned complex architectural features. Although FEA shows cells that resemble those seen in low-grade DCIS, it differs from it by the absence of the aforementioned combined complex architectural and cytologic features that involve the duct entirely."
This article may be helpful as well: The diagnosis and management of pre-invasive breast disease: Flat epithelial atypia – classification, pathologic features and clinical significance
Although FEA and ACH is less common than ADH, there are many women on this board who do have FEA, although often it is in combination with ADH. If you use the search button on the left hand side of the screen and input either "FEA" or "flat epithelial atypia", a lot of different discussions threads will come up, mostly in the High Risk for Breast Cancer forum. There are lots of women there also on 6 months follow-ups: Member of the "6 Month Watchful Waiting Club", Unite! -
Thank you Bessie...I'm going to read right now because I don't know if I should be worrying about this wait and see thing or if I should be relieved. Have a nice weekend!
