Pain in the ovaries

I'm not really sure where to post this.

I had a hx 28 years ago - but still have part of both ovaries.  I would get ovulation pain regularly until I went into menopause - about 10 years ago.

Lately I've been having what feels like quite strong ovulation pain on the left side. I've had a u/s and both ovaries look normal as do my kidneys. I had a colonoscopy in December last year and that was fine - just some benign polyps found which were removed.

I'm on Aromasin, and one of the suggestions I had a while ago was that ovaries shrivelling up could cause pain.  My dr isn't convinced of that, but wouldn't rule it out.  Any further tests would be invasive, and I'm not quite ready for that yet.  But the pain does seem to be getting worse.  I've had it for around 3 months now.

I had both ILC as well as IDC.

Any suggestions?  Anyone else had anything similar?

    I totally understand except my pain is on the right side.. Joked with my ob/gyn that it must be the ovary as my appendix should have ruptured by now.... I have had two transvaginal ultrasounds and she is now recommending an exploratory laparoscopy. I too am not ready for any more surgery but am concerned about what the Tamoxifen may be doing.

    Thanks for your reply odie, it's a worry when there's pain, but everything else seems normal.  I'm sure it can't be ovulation pain -  it's all the time, not just certain times of the month.  And I guess if it was ovulation pain, I'd be in big trouble lol - and the Aromasin wouldn't be working.

    I know it isn't my appendix either - it would have ruptured by now too lol. 

    I did have quite severe enometriosis - which is why I had the hx, I wonder if it could be scar tissue.

    Wish I had an answer but can only commisserate. I was premenopausal but it didn't correlate with ovulation either. Sometimes would be almost unbearable for a few days then ease off to simply annoying most days. Now I seem to have stopped having cycles (since June) but the pain is better...just not sure what it means but hope it is just minor discomfort from the lack of estrogen...



    Hopefully your pain will get better soon.

    Trisha and Odie, I don't want to scare you or be a drama queen but I am a firm believer in you know your body better than anyone. Last summer I was having a very similar pain. I felt cramps like I was about to start my period but chemo threw me into instant chemopause. In about the location of my ovaries I was having a throbbing pain. The problem was the pain was not constant. I ended up with mets to the pelvis, ovaries, tubes, uterus, etc. On ultrasound one ovary was a little enlarged but appeared normal for my age (46) and being menopausal.



    Trisha, ILC can be sneaky and not show up like it should. This past week I had an MRI that didn't show the same thing as the previous scan. The doctor said that ILC is more of a thickening than a distinct lesion. Again, I am not trying to scare you but rather to encourage you to pursue anything that lasts for weeks or consistently becomes worse.

    Oh Samayofl, I must admit that was at the back of my mind just a little tiny bit.  I know ILC can be pretty sneaky and doesn't show up as you would expect.

    My dr is saying the next step would be invasive though - should I have a explorative laparoscopy?  I know you can't answer that, I'll have to make up my mind.

    So was your pain like ovulation pain? Mine seems to stop for a few days and then come back for days on end, then just stop again. 

    I'm just so over procedures and tests!

    Trisha-Anne, while it's true that exploratory laparoscopy is "invasive", it's not that difficult (I had it years ago after repeated miscarriages). A very small incicion is made and the tube with laparoscopic camera is inserted so docs can see all your pelvic organs. It's done under general anesthesia. My incision was very small - 3 stitches was all that was needed - and only hurt if I pressed on it. The gas that's pumped in to inflate the area is very uncomfortable and sometimes painful for some people but usually dissipates within a day.

    Trish, which doctor is monitoring this symptom? Is it your oncologist or gynaecologist? Maybe both should be involved.



    Have you had blood tests for hormone and ovarian cancer markers?



    Could it possibly be cysts?



    I hope there is a benign explanation and that your doctors can work together to get a correct diagnosis.

    Thanks for your responses, Leah, yes I've had 3 or 4 laparoscopies and a couple of laparotomies before the hyster, so know that it's not too bad.  I'm just reluctant to go down that path until I have no other choice.  I'm making sure first it's not problems with my back (I have arthritis in my back) that is refering the pain to my pelvic area.  

    I don't seem to have any pain in the mornings, it comes on gradually and is worse in the afternoons.  That could be from a back problem, after sitting all day at the computer.  But I seem to recall having pain on days when I'm not at work.  I'm going to start a "pain diary" to try to pinpoint when it happens.

    Racy - at the moment I'm only seeing my GP.  I haven't had a gynie since the hyster 28 years ago lol haven't needed one. I don't see my onc until Jan next year, although I can see him earlier if I need to.  I had an internal U/S a couple of weeks ago and no cysts were seen, ovaries looked "normal" although she did say they were tender.  Tender was an understatement - the internal U/S was horrible, everytime she probed towards the ovaries, I was in agony.  It wasn't just the left side (which is where the pain is) but the right side as well.

    No blood tests yet, maybe that's a next step.

    Trisha-Anne, you mention that you've had a hyster as well as several ex laps over the years. Is it possible you have adhesions? I've heard that adhesions don't necessarily show up on scans so the only way to know for sure is surgery. Unfortunately surgery is what causes adhesions. And surgery is they only way to remove the adhesions. But therapeutic surgery can cause even more adhesions. It's an awful cycle. I hope you get some answers and feel better soon.

    Racy, I'll make sure it's not back problems first. Didn't have any pain yesterday at all, so it's not constant.



    Lekker yes I wondered about adhesions too. I know I have them as they caused problems in the few years after the hyster, but then seemed to die down.



    I've decided to keep a pain diary over the next month and then go see my gp.

    Trish, remember that your ILC was very small, and you had chemo, so hopefully your symptoms aren't cancer related.



    Over to you but I still suggest following up sooner rather than later.

    HI there, I had for years what felt like pain in my ovaries. Did all the tests you did, they were negative, thank goodness. I ended up seeing a back doctor and when I explained some of my pain, he talked about the sacroyliac  joint.  I ended up getting an injection in my sacroiliac joint and the pelvic pain, which I thought was ovary pain, went away. Perhaps this could be your issue too? Worth a check...
