mastectomy after lumpectomy and radiation with ILC

Hello,

I haven't been active on these discussion boards since shortly after I finsihed my radiation treatments four years ago.  I was diagnosed with microinvasive ILC 1mm on the left breast in May 2009.  I finished 33 radiation treatments the day before my 45 birthday on 9/17/09.  I started on Tamoxifen shortly after that. Since my original diagnosis I have been doing great! A new issue has arisen though and I need some support from people who have been through it.  I have been going for my 2x/year mammos on the left side with yearly on the right. I have also been getting annual MRI's.  On my original biopsy in May 2009 in addition to the ILC it also reported lobular neoplasia.  Last year in August when I went for my left sided mammo they did a stereotatic biopsy the same day.  It came back with lobular neoplasia again.  They recommended watching it in 6 months with a mammo and then in Aug 2013 to have another mammo and MRI.  Well, I went for my mammo on Monday, Aug. 19 and the radiologist again recommended a biopsy.  This time she sugggested a surgical biopsy with a wide excision of the area which is what I had in 2009 when the cancer was found.  She showed my scans to another radiologist in the group (all they do there is read breast films all day).  They both felt "unsettled" by the films.  I immediately went home and got an appointment with a breast surgeon for Friday, Aug 23.  On Wed, Aug 21, I saw my medical oncologist for a routine follow-up appointment.  He told me to have the biospy but not to be worried about it (easy for him to say!) as he wasn't worried.  My husband and I saw the breast surgeon yesterday afternoon. We talked options, including doing the biopsy and proceeding with a "wait and see" approach all the way to having a bilateral mastectomy.  After the appt I went for an MRI.  I am leaning towards the bilateral mastectomy just so I don't have to worry as much (and neither will my family).  The breast surgeon said that with lobular neoplasia it does put me at higher risk for developing another breast cancer in the same breast (either ILC or IDC) and with the original lobular cancer I have a greater risk of it occurring in the right breast.  The surgeon told me that I had already played my "radiation card", so if I do develop another cancer that is out as an option.  She set me up with an appt for a plastic surgeon on Tuesday, Aug 27.  She said because of the radiation on the left breast I cannot have the more common recontruction with saline.  She said I would likely have to go with the DIEP reconstruction.  This is a 9 hour procedure with a 5-7 day hospital stay and a 6 week recovery.

My question is:  Has anyone else ever gone through this procedure after radiation and how was it?  Are there specific questions I should be asking the plastic surgeon?  Any comments/suggestions/personal experiences would be welcome!

Thanks!

Jill

I just went through it in July but I had a BMX and one-step recon, no DIEP. If you search some of my posts (go to search and fill in member name) you'll be able to see how I did (very well, I think).  I'm fully recovered.  You can also PM me for more info.

jillrio-I was diagnosed with ILC and had a lumpectomy, chemo and radiation. 5 months after radiation I had a BMX with SGAP reconstruction. It's similiar to DIEP but done using an artery from your hip/buttock area instead of your abdoman. I have had good results on the side with previous radiation.
