Fat Grafting....recovery & results
I will be having a fat grafting procedure done in about 2 weeks. I had a bilateral mastectomy with DIEP Flap reconstruction in Oct 2012. In January I had a revision (lift and shaping). Now I'm ready for the fat graft. My understanding is the PS will Lipo fat from my hip area and fill in my breasts where needed. Has anyone had this procedure done recently. I'm wondering what the recovering time is (I know we are all different) and if you were pleased with the outcome. Any questions I should be sure to ask before the procedure....maybe something I haven't already thought about. Wishing everyone well...........

As a matter of fact, I just had some lipo/grafting done 10 days ago. The first week was a little rough - mostly due to bruising & bandages. The fill spots & lipo sites were painful. The compression garment isn't fun, but is worth it. I have 6 more weeks of that to go. I had some grafting on the upper poles and a divot on the left side (for lack of a better description.
For me, the worst are the night sweats and lack of appetite as the nausea works out of your system. My surgery only took 2 hours. I was home 2 hours after surgery ended. I did have an RX for tramadol, but ended up just taking ibuprofen afterward. The sleeping on the back is annoying, but will get better with time.

Just had my revision and fat grafting last week. You will be sore for a few weeks and yes the binder in a pain. But yes it is worth it. My upper pole is still swollen but looking good no more hollow spot. My surgery was a little under 2 hours, very doable.

Janet, thank you so much for the information. Sounds like I need the grafting in the same places you had it done. Did you have the "divot" in the upper inner chest where they remove a small pieces of bone to do the flap surgery? I have divots on both sides. Although not a picnic sounds like things went pretty well for you....happy to hear that. I keep hearing about this compression garment even after the DIEP surgery. I have never worn one. I wonder why my PS didn't do that. I am going to ask him about it at my pre-op this Monday. Thanks for your kind words. Grafting scheduled for May 30th....will keep you posted. Wishing you well!!!

Wherever they take fat from they will require you to wear a compression garment or binder. If it's belly, they may give you an abdominal binder (similar to those velcro back supports). For me it was belly and inner thighs....my compression garment zipped up both sides....getting it on took an act of congress. It compressed my belly and my upper thighs where the liposuction cannula had been inserted. I've graduated to "spanx" like garments for the last 4 weeks.
Wearing them will prevent fluid from filling in and accumulating where the fat was removed causing "seromas: or fluid pockets.

Hi A...yes it makes sense. Just wondering why I didn't have a compression garment for my stomach after the DIEP Flap surgery? Or is it only used after the Lipo? Mine will be from the hip area.....on both sides where my incision ends.

Flaps take ALL the tissue excised together. Fat grafting either involves large needles or a liposuction cannula that will create "tunnels" that fluid may fill up. That's the BIG difference - How the tissue and fat versus fat alone is removed.

Hi ladies - I just made a post on the nipple reconstruction topic a second ago but wanted to touch base to say my turn is next Thursday (the 1 year eve of bc dx). Fat grafting from tummy to both breasts, side rippling on on left side and both upper poles need to filled in, I have a boney chest and the implants have dropped. While I don't know exactly where PS will take the fat from nurse said common areas are lower pelvic and belly button (ew....sci fi..gross). Also getting my a reduction of right nipple as the left nipple didn't survive bmx, I've been living w/ one projected nipple since last August
aerolas look great; I'll be more than happy w/ flat nipples. PS nurse told me to take more time off of work than I planned, she said I will feel like I did 1,000 sit-ups, pain level will be much, much more than exchange which was about a 5 on the pain scale. Wish me luck! I wish you the best src3acs, keep us posted your f.g. surgery/recovery.

Thanks for the info SA. Pre-op appt with PS tomorrow morning( May 20th). Hi Jill.....I had the DIEP so not much fat left in my belly. My hips bulge some where my incisions end...that's where PS is taking the fat. I have a very boney chest as well so I'm hoping I have enough fat to fill in both upper poles. Good luck on your surgery and recovery. Let me know how it goes. I will be thinking of you on Thursday......my surgery is the following Thursday.

Hi Ladies, thank you Sharon of thinking of me. Well the anesthesia wore out about 1 a.m. today and oh boy am I sore! Taking pain meds/ muscle relaxants / anti-biotics (about a 7 on the pain scale). My breasts don't hurt at all (sporting a surgical bra w/ lots of gauze) and wearing a white binder lined with gauze that starts under bra and runs south of my pelvic bone, they had to shave my hair down there. My nipple reduction stitches are internal and then glued shut. I can see very little of my core, but what I can see is bruised dark purple. I see PS in 3 hours, I imagine he'll want to take a peak at everything. Surgery took 3 hours and 1.75 hours to wake up. I'll be able to tell more when I get home, I'll let you know. In the meantime, time for pain meds. Jill

Hi Jill...sorry to hear your pain level is so high. I hope it comes down soon and your recover quickly. I talked to my PS on Monday. Said he plans to only take fat from my hip area and put that in my breasts (is that what we still call them after a MX???). He is also going to remove the right side of my abdominal scar, the scar tissue and repair that. Audrey, I asked him about the compression binder and he says I won't need one and will not have any lumps or dimples where he removes the fat. Anyway, my surgery in this Thursday. Have been looking forward to it...can't stand my boney chest but now I'm getting nervous. Oh, GYN found complex cysts on both ovaries. They are small so we are going to look at them in 6 weeks. She consulted a GYN Onco and was told it was best to look at them again and see if there are any changes....because they are small. Guess I could be making some new decision in the next couple months. Well wishes to everyone!!!

Hi Sharon! Well first off I wish you the best on your grafting this week! That is great you won't have to wear a binder. They are tight. I'm so sorry about your ovarian cysts. One step at a time. I had a recent gyno scare that delayed my fat grating/nipple reduction; had abnormal painful long bleeding; transvaginal U/S found a 1 cm polyp, April 12th had a hysteroscopy/polypectomy/D&C. Polyp benign but what a scare I didn't need so close to bc dx. But that's over w/ and here I am today.
Back from PS appt and I am feeling much better...surgical bra off, I can wear a soft bra and I'm cleared to shower and wear a Spanx. PS prefers me to put the binder on when I'm at home, it's hard to hide under clothes. The grand unveiling was a quite a sight. DH, me and PS all very happy! My flanks/stomach are so flat. No bruising on the belly just on a few areas which will go away. PS says the fat in my breasts will take 6 months - 1 year to settle in and will only look better as time goes on. My right nipple looks great, he didn't completely flatten it out but it will be a super close match once it's healed. I feel like I'm back on track. I put on 12# this last year with all the surgeries but once I'm cleared to exercise/run again, it will be permanent, cuz no more operations (I'm thinking positive which has been a challenge for me!). My flat stomach is motivation to eat healthy and get active again. Let us know how your surgery goes and how you are doing with the gyno issue too. jill

Hi ladies, thought I give you a 5 days report. Off Vicodine for almost 3 days (pain only when I accidently hit or move my gut wrong or pull up my spanx). At my 2nd post op appt today, PS said I look great, am healing well, some of the bruising is turning yellow. Cleared to take Motrin, that will help the swelling go down. Wearing my spanx + binder around the clock, honestly I feel better (protected w/ both on). PS said the hypersensitivity I feel in my gut will feel like this for the next 3-6 months, even w/ the tiny incisions of lipo it's pretty aggressive once inside, hitting way more nerves than a large incision surgery.

Hi Jill...you sound good. Happy to hear the pain has improved. I go in tomorrow morning...surgery at 8am. I'm so glad I read this. I had a lot of hypersensitivity in my stomach after my MX and DEIP. It has gotten much better....was hoping I would have to go through that again. Maybe mine will be different since the fat is coming from my hip area and not my stomach (that was already done). I will let you know how it goes after tomorrow. Wishing you well!!

Hi Sharon, yes I'm better as the days go by. Returned to work today but was pooped by the end of the day. After my PS showed me my flat tummy I am eating healthier than I have in over 10 years; the 6 # I put on from inflammation post op is gone + 2 extra #'s. I am determined to keep my stomach flat. It helps wearing these eating control devices a.k.a. spanx + binder, my external lap-band
you can only eat so much when your stomach is pressed in. Seriously, horray for you, tomorrow is your day and 8 a.m. surgery is nice and early. I forgot to mention but at my PS office the nurse used on me (and I bought a bottle there) Donnell Super Skin K-Derm Cream for bruise and rosacea control. Recommended after surgery. See if your PS sells it, works great. I'll be thinking of you tomorrow. Yes, looking forward to hear how your surgery goes. best, jill

Hi ladies. Jill I wanted to ask you if you only had lipo for fat graphing or if you had the whole lipo procedure. Im scheduled tomorrow at 845am for exchange with fat graphing and I didnt know if he will take enough fat that i will notice. I thought that only a small amount was taken. Well it would bea blessing if he took alot.

I had my mastectomy with immediate reconstruction surgery (by using my belly fat to create my new right breast) was last week on Friday May 24, 2013. I was scheduled to be at Beth Israel Hospital by 9:30am. Got there on time and they gave me a buzzer and when it goes off that was time for prep. We (my hubby and I) sat in the waiting area until this thing buzzed...then I started crying and they told my husband they will let him in later.
They got me all ready for surgery and my husband was allowed to come in. We said some prayers and I made him do a before photo with all the marker markings made from the plastic surgeon. I was still very scared and nervous. The doctor came in again and spoke to my husband and answered all his last minute questions. I was glad for that. We kissed and I was wheeled into surgery which six hours later it turned out good:happydance:. I woke up and my husband was the first I saw. I was super happy and very scared to look under the covers now.
Then on Sunday May 27 I felt this pain in my right new breast that omg words could't express it the doc was called in and he said it was a blood clot building up and everyone in the room had to step out and he and his team did their work right there. I started out being awake but by time he was done I was knocked out cold. I was so happy he was around at the right time.
I have a follow up appointment with the plastic surgeon tomorrow so, I hope me and hubby are doing things okay. The drainage's are a bit discomforting to do but we get it done. I am still very sore but once things heal it would look better....I hope anyway. I find it hard to look at myself right now. its really not a pretty sight to see...I thought looking at other women will help me but seeing it first hand on my own body is just a shock for me.
Other then the look I am doing good and coming along well.

Hi Melmcbee: Looks I missed your note in time b4 your surgery. I had fat grafting/recon revision, not whole lipo surgery.
Blessings to you & Sharon who are in the OR today. Welcome 35 smiling to this tread, oh my you've been through a lot this last year, your blood clot story is scary, thank God you doctor was near. Immediate recon is a tough recovery dear, stay on top of your pain w/ taking meds and accept all the help you can get from others.

melmcbee - good luck with your surgery today! It is important to understand that because insurance pays for revision surgery (fat grafting to "fix" a problem is coded as a revision) your doctor has to be careful not to take enough fat that it becomes a cosmetic procedure, like full lipo. Your doctor submits a surgical report with the total amount of fat removed - it is measured in cc/ml, not sure - and insurance companies know how much should be removed for this type of surgery. You run the risk of your insurance company denying payment for the surgery if it is looking more like cosmetic lipo than a fat graft. It is also problematic to combine full lipo with a revision because it opens the door to a denial by the insurance company - I specifically asked about my PS about this. I had fat harvested from a number of locations but that was because I didn't have enough in any one place to make enough after centrifuging to fill in the hollows in my upper chest.

SpecialK. --my PS gets prior approval for liposuction and Fatgrafting. At one surgery, they called it cosmetic--but when I called-they quickly reversed themselves. Was good to have that prior approval letter in hand. Eileen

eileen - the way my insurance works is the same - I need to have a referral and approval prior to any surgery, including an emergency one (which I had shortly after BMX!). I agree that it is nice to know that everything will be covered! The only thing in my treatment that was denied was Mammaprint testing - for two reasons - the BS office failed to get the referral in advance, and my insurance considers it "experimental" even though it is FDA approved. If you are not familiar with it - it is similar to Oncotype Dx, but tests a wider range of genes. Because it was an error on the part of my BS office staff the test cost ($5,000 - and yes, I choked when I opened that letter!) was absorbed by either the BS or Agendia, or both. All I know is that it wasn't by me!

SpecialK--I relate to your issues. Over these many years of BC treatment, I have death with many financial issues--often errors. At times, when I attempted to double check billing issues, they were annoyed with my inquiries. Errors were still made. This has been an added stress in my BC treatment. Eileen

eileen - every insurance issues I have had all stems back to the scheduler in my BS office - I ended up doing her job for her right after I was diagnosed, on the Mammaprint and the referral for the PS that was being specifically requested by the BS that she worked for! I found out that there was no referral in place 5 days before my BMX! I went and sat outside of someone's office until it was done, then had my PS consult the Friday prior to my Monday surgery - sheesh! The good news is that I have the personality of a bulldog with these matters - that is why I didn't pay that $5,000 Mammaprint bill, they did! You are so correct that we end up even more stressed because of all this stuff on top of a new diagnosis!

Hi ladies. I have not taken the compression or bra off yet to see the results but I did post some pics of my pretty bruises and swelling on my abdomen on the pic forum. I will try to get pics without them tomorrow but I cant remember what I was told about when I can shower. I dont want to mess anything up. I havent slept yet so I hope the pain wont b bad when I wake up. Healing hugs to all

Hello Everyone....had my fat grafting done yesterday. The fat came from my hip and I think partial back area...that's where I am sore and bruised. The PS said he took enough fat to double my size and fill in all of the craters (as I called them). I haven't taken the surgical bra off yet but I can see a difference. Hope it's not all swelling and I just hope it holds. I hear 35%-40% for the fat can be reabsorbed by the body. I also did not know he was going to go through my lower back to do the lipo. This is only day one....and I feel like someone beat with a bat from hip to hip around my back. Pain is not too bad if I am sitting still or even standing. It's the motion of getting in and out of my recliner that hurts. Also, the lower part of my rib cage hurts in the front. I have a low tolerance for pain. I wasn't supposed to get a compression binder but I had a lot of bleeding every time I stood up. So they added a lot of gauze and a binder to compress that area. Bleeding has stopped. I am still wearing the binder....it makes me feel protected. Pain meds and sleep this weekend. Hoping to feel better next week. Best wishes to everyone. Keeping you all in my thought a prayers.

Sharon Im glad you checked in. I feel the same way. As long as Im standing or sitting im ok. Its the moving up and down that hurts. I also have a stretching pain on the top of my breast where the fat was put in. So im trying to keep ice on it. I hope you feel better

melmcbee - Please check with your PS about using ice on the graft area. You really want the newly grafted fat to find a blood supply and ice may interfere with that.

Hi ladies - I had fat taken from my belly and grafted to my implants yesterday. The PS says it will probably take two more attempts to get it aesthetically pleasing/perfect (well, in his eyes). God, I'm not sure I want to go through this all again. Definitely sore in my belly area and like some of you mentioned, getting up from a sitting position is worse. You know, I didn't ask him to take more fat than necessary and he certainly didn't offer. After reading SpecialKs thread, I guess there is a reason why? Anyway, I will probably go in and get more done, but in some ways, I feel real silly doing this. What's the point....except for vanity, I suppose. The "foobs" look good in clothes and isn't that all that matters?? Yes, I have some rippling and ridges, especially on the breast cancer side, but I'm the only one who sees it. My hubby certainly doesn't care.....eh, I guess its like childbirth.....you forget about the pain after awhile and then go for it again, and sometimes again! Have a great weekend ladies!

I had some filling in with the fat grafting on April - was surprised at how quickly the bruises went away and pain went away! Hope you all have same!
