Basic exhange questions
I've been researching some basic information on this but I've not found any concrete answers so here I am. You all know more than the Internet anyhow
After a BMX, do they use the same incision for the exchange?
Will I have drains again?

I believe that most plastic surgeons use the same incision, mine did. I did not have drains, but this may vary from person to person. If your plastic surgeon needs to do any additional work on the pocket, etc., you may have a drain. I thought the exchange surgery was easy. Good luck!

I just had my exchange this past Wednesday. Yes, my ps used the same incision for the exchange andTHANK GOODNESS - NO drains....those things suck ....
And IMHO, I thought the exchange surgery, except for anesthesia this time, was a breeze....and my PS told me she did more work in there than she thought she'd have to and I had some fat grafting.... -
My ps used the same incisions but I did have two drains post exchange. Even so, very easy surgery. Had the surgery on Thursday and went back to work on Monday.. The drains seem to vary on PS preference.....Hope yours goes smoothly.

My PS used my original incisions for the exchange, and he cleaned them up quite a bit. Before my exchange the incisions were fairly lumpy and wrinkled. They ended up nice and smooth and pretty! No drains. Surgery this time was a piece of cake. The hardest part was complying with the restrictions, because I felt so good.

ShaneOak, just had my exchange on the 23rd and yes my PS used the same incision, just cleaned everything up. I also had a lift on my healthy breast so I have been a little sore. Overall the exchange seems to be a much easier surgery. It is still a big surgery so you will still have restrictions to follow. Hope this helps.

Thank you so much ladies, I knew you'd come through with the info I needed. Wow, I feel a lot more relieved knowing that I may not have drains and that it won't be like the first surgery.

ShaneOak, I know you only asked about BMX exchange, but I thought I'd weigh in anyway. I had UMX and my exchange surgery is this coming Thursday...Valentine's Day! My original scar is out from the nipple, and the exchange incision will be in the fold under the breast. I will have a drain for a few days. I will not be spending the night in the hospital unless there are complications. I hope my doc isn't painting too "rosey" a picture, but he's almost convinced me that the surgery is "not a big deal". (Ha! I'll let you know!

Steiner18, I was back to work 10 days after my exchange surgery. I worked a part day the first day back, but then was able to work a full 8 hours after that.
Shane Oak, My PS used the same incision,but only about 2 inches of it. My original incisions are about 8 inches long (from underarm to breast bone). I've read that with silicone implants the incision needed to be quite large, but that wasn't my case. My implants were 450cc on the right and 475 cc on the left.
Taylormd, I hope everything goes smoothly with your surgery and you have a boring recovery. This surgery is easier and so it is easy to overdo afterward. Be mindful of your surgeon's instructions and take it easy.

Why did you have to have a mastectomy for DCIS stage 0? Just preventative choice?

I second odie16 on the drains varying by PS preference. Mine used them and I was a "drain-hater" from my 4 week tangle with them after my BMX. They came out in less than a week, though.
Shane Oak, I had the same incisions for silicon implants & they were nice and smooth and no wider than before.
46and single, I had a BMX for DCIS, one side prophylactic, because I didn't want to deal with the wondering and worrying between mammograms- it's easier for me to just get it off my plate because I have a pretty hectic life. Some women chose MX because they have DCIS over larger areas that a lumpectomy wouldn't be a viable choice. Good luck, everyone! -
Can an exchange occur after radiation? My PS won't operate on radiated skin (I start radiation at the end of the month) so I have a choice to make- either get the exchange after 4 TE fills or wait until after radiation to do a lat flap. I have no idea wat to do!

Kelly...my exchange was done after radiation. I had my TE's fully expanded before I started radiation, and then my PS said I had to wait at least 6 months before he would do the exchange. I ended up waiting about 9 months (he really preferred a full year, it was our compromise), and had no trouble. He said I had good elasticity back in my skin, and it would be fine, and it was.
Since you're going to have a bunch of time after radiation anyway, maybe you can look around for a 2nd or 3rd opinion? It really shouldn't be that uncommon - just look for someone experienced.

46andsingle, Honey, NO question is dumb! We've all had to learn far more than we wish we knew about a subject we really didn't care about learning about! BUT it's so good that we have each other to ask and "bounce" things off of. Anyway, I was given a choice of mx or lumpectomy w/ radiation (7 wks, I think dr. said). I just wanted to "get the cancer out" as much as I could and hopefully be done with it. It's such a personal choice! Another reason for my choice is that I had to have quite a bit of radiation as a child for an unrelated condition, and I didn't want to have anymore rads if I could prevent it.

Thank you ladies for weighing in on your exchange experience. I'm really hoping that I can be back to work in less than 2 weeks. I'm ready to get on with life. How long after the exchange do you get to see them and when did you start wearing a bra again....if at all? I think I only have about 3 more fills before they schedule the exchange.

I had an ace wrap on for 4 days, but could shower after 2 days. I saw them on the 2nd day after surgery. I thought they looked fine-just what I had asked for. I wanted to look like I did preBC and they are very close. My PS had me wear a sports bra for about 6 weeks. Now I can wear non-underwired bras. I was never a fan of underwires although there are some very pretty ones.

Itsjustme- Your PS wanted to wait a year before exchanging? I can't imagine going that long with the TE's in. They are so uncomfortable...I have had 3 fills and plan on one more before rads. My PS said he could do the exchange about a month after rads if I was happy with the size or I could do the lat flap if I wanted to go bigger. But now I'm nervous that maybe a month is too soon??? I really wanted it done before summer rolls around because I can't imagine the TE's look natural in a bathing suit. Am I unrealistic here? Thanks!

Kelly~I'm already thinking about summer dresses and how tops will look when the weather gets nice and I can come out of hibernation!
Julia, how would you compare the recovery with the exchange to the MX?

I didn't end up having chemo or radiation but my PS had said waits a minimum of 3 months after chemo and/or radiation is completed before doing exchange due to immunocompromise.

ShaneOak, My exchange recovery was a piece of cake. The only problem was I was feeling so good and used to do so much with my TEs, I would overdo on occasion. It was hard to go back to lifting/weight restrictions when I felt so good. Pain pills for maybe 2 days, then Tylenol. I had the surgery on the Friday before Thanksgiving and went back to work on the Monday afterward.

I like hearing that it was "a piece of cake", this makes me super excited for the exchange and life afterwards. I just read something that I didn't know about the exchange. It was about whether or not the PS put the implant under the muscle or over it?? I thought there was only one place for it to go (under) where the expander is?? I'm so confused now and not sure if I just misread the article. I need to dig it up again. How long before you can lift like normal? I'm dying to be able to pick up my baby

Kelly, my PS said a minimum of one month per one week of rads before exchange. So if you have, say, 30 treatments, that would be a 6 month wait. Due to a trip I'd like to take this summer, I'm not having my exchange till probably the end of July, beginning of August, which will be 7-8 months after rads. Due to possible complications with healing, they like to wait. However, we'll see how things look at that time before going forward.
I had all my fills done before I started rads, which seems to be the way it's generally done. Yes, the TEs are not the most comfortable things, but as time goes on, they don't feel as bad as they did at first. Good luck! -
okay, my sweet bc sisters! Tomorrow is my exchange surgery! This morning my DH woke me up with the declaration that "Today is our Valentine's Day!" He has made it a wonderful day already, and now we are about to go eat at my favorite restaurant! I will be one happy and well-fed lady when I'm wheeled back to OR tomorrow morning!
I'm sooooo looking forward to getting this TE "outa here!"

Kelly...my PS said 6 months was the minimum he would wait - he said a year was ideal, but was willing to do it at 6 months - we just couldn't work out the date, so the 9 months was when my schedule and his worked together.
I wouldn't do it in less than 6 months - as a matter of fact, if he's pushing more surgery than you want, I would look around and get other opinions - 1 month iscompletely irresponsible for him to even suggest - your outer layer of skin might be healed (ad probably won't even be), but there is no way for you or I to determine if all the layers have healed. He flat out told me that anything under 6 months after radiation, and there is a good chance it'll fail. Radiated skin heals differently than normal skin.
Please get another opinion.
Oh - you'll be able to wear a bathing suit, it'll look OK, not perfect - like the ridge with the tubes, a string bikini top won't work for you, but other than that, you should be fine.
Your doctors will let you know how much sun exposure will be OK that soon after rads ending.

ShaneOak, I would say you would be able to "free" lift your baby after a month. Of course, every PS is different and you certainly don't want to jeopardize your recovery. My granddaughter was 7 months old when I had my exchange, my daughter would hand her to me so I could hold her. A couple weeks after surgery, I could get on her level and pull her to me, keeping my elbows tucked to my sides. That method didn't seem to cause any issues. The best feeling was when she snuggled into my chest and fell asleep, something that wasn't possible with the TEs.

Eek! No sun exposure after radiation too?! I am a teacher so I'm off for summers and live at the shore...last summer I couldn't go to the beach because I was on chemo. This is brutal! Thanks for letting me know that I should be waiting longer before doing the exchange. I am definitely going to be brininging this up to him when I see him on Monday. I do really like him and I hope I don't need to find a new PS. I guess we'll see what he says.

LOL Kelly...you can have sun exposure - just remember sun block for the first summer after rads - especially on the radiated side. I was out in blazing sun in a tank top (no bra - the best part of TE's -no bra), and my radiated side ended up pink - lI was like "really?" - even called the radiation phone # - they said it was normal to have happened. Supposedly the sensitivity goes away after that - I'll let you know!!

Hi, Everyone!
Well, I had the old TE removed on the 14th! Can't tell you how happy I am to get that "booger" out of there! Even in the Recovery Room I could tell how much more comfortable the implant is than the TE. Surgery started at 10:10 AM and I was in our car heading home at 2:30 PM! I have been amazed at how good I feel! I have a drain and will probably have it taken out this coming Tues. I haven't needed any pain meds since 36 hrs. after surgery! I had some nausea last night and this morning, but took Phenergan and got rid of that! I definitely have that "healing feeling"! Take heart, you youngsters! I'm 71 years old and can tell you that the exchange surgery is nothing to fear!

I just had my exchange on feb 15th I to noticed the difference ...post op day to quit sore and itchy..but am glad to have the expanders out...
