Birads 4D but no biopsy recommended

Hello, I have been lurking on these boards for a couple of years and love this community.
I am 37 years old, have extremely dense and busy breasts, and never had children. Mother was diagnosed with IBC at age 65. So I am considered high risk and on the usual 6 month MRI, 6 month mamo/ultrasound watch.
In the past few years since the monitoring started, I have had numerous biopsies (all excisional) that all came back benign.
This time once again I got the call back after mamo and ultrasound. Basically once again they found both breasts are extremely dense, heterogeneous and complex with scar tissues but the DR couldn't pin point any specific area to do a biopsy. The report says no outright sign for malignancy was found but would recommend bilateral prophylactic mastectomy at this point. The report gave me a rating of Birads 4D !
I am wondering if anyone has had a rating of Birads 4D but no biopsy, just bilateral prophylactic mastectomy? My mind is racing once again.

Wow! You have been though a lot. I only had 2 biopsies and that was enough for me.
I can understand your confusion and why you would question the recommendation. They should have explained to you the reason why. I'm a RN but not a cancer specialist or anything. Only info I know came from dealing with my own.
The way I see it is they are concerned that you are having so many biopsies and now so dense can't find the area. Could be that they think its cancer but your breast are to dense to find it. It's a hard decision you have to make. They can't guarantee you that cancer is there without biopsy nor can they rule it out. The odds are high in there view do to your history. Remember machines can only tell so much. Looking at the actual breast tissue under a microscope is much better. I had my first mammogram at 27 and was always clear until February. Once it was found it was stage 3 due to 6 lymphnodes being positive. So basically I thought I was staying on top of it and didn't work out that way.
After finishing my chemo all test said I was cancer free, didn't show up on any test. When I had my MX they found another tumor in the cancerous breast that was the size of a pin point that wouldn't have showed up for years if at all.
Having said that I feel for you. It's hard to make a choice of having breast removed than finding out nothing was there. Than again what if they find something, than it would be worth it. MX isn't like a lumpectomy once the breast is gone its gone.
There is a new machine designed to exam dense breast but not sure when it will start being used. Whatever you do take your time to thing about the plus and minus of each procedure and how it will affect your life.
You aren't alone dealing with this. We are hear to support you and share what we know but the choice will be yours.
