Follow up mammograms
Hello everyone,
I have been having followup mammograms every six months following my initial BC treatments. Every single one comes back as a bi-rads 3. There have been 3 in a row which is very nerve racking. My surgeon sent me to have a breast MRI which came back negative but what about a recurrence of DCIS? MRI isn't the best tool for that is it? However, I am going back for my next 6 month followup mammo in November. My doctor has hinted that if this comes back a bi-rads 3 we may do a biopsy. This breast is already been through the gamet (3 lumpectomies and radiation) and I'm not sure if I even want to continue this path any longer. It seems that there has not been any determination as to whether or not I am actually cancer free or not? Here is questions?
Anyone else been through this same scenario?
If my surgeon recomends a biopsy could I instead go for a simple masectomy instead?

H cinchgirl, I'm heading off this afternoon for my first (6 months post rad) follow up mammogram so was very interested to see this post, although I don't have any experiences to share (yet). I'm also curious to know what the follow-up experience has been like for others who had surgery and radiation to treat DCIS.

I've read that MRI can be more effective for detecting high-grade DCIS, but I'd read of someone on the board in the last 2 months who'd had their intermediate-grade DCIS detected with MRI but not mammogram. I think the additional imaging is a good thing, especially since the MRI has no additional risk like radiation. MRI's can be prone to false-positives, so I think that it's a good sign that nothing was detected in your MRI. Best wishes for clear mammograms!

Im pretty sure mammo picked up my low grade dcis. It was my first mammo

My intermediate grade DCIS showed up only on an MRI, not an ultrasound or a 3d mammo. I had requested an MRI instead of a mammo initially because mammos are extremely painful for me. If I had not initially had that MRI, it would never have been found. My doctor is always going to have me get an MRI as part of my follow-up.

Having the MRI does give me a little more peace of mind although both tests have their limitations...........I have always heard that the Mammo is better for detecting DCIS and the MRI for invasive.
All I can do now is pray and wait on the upcoming mammo and go from there!

I just had a biopsy last month after my annual mammogram picked up abnormalities (or whatever). Thank G*d it was benign. However, I have to do a 6 month followup. I hate them; any test is just scary and nerve wracking. Once you've had BC, I guess you never get over of the fear of it coming back. I can understand your thinking about getting a mx just to put a halt to all of the testing, but remember a mx has its own set of problems.

I do understand that the masectomy comes with its own set of issues. Hopefully, the masectomy wont be an issue and I will finally get a a "clear" mammogram. It's just that there is alot of things to consider when facing the challenges of breast cancer treatment as we all know far too well.

Hi all, just got a call for a follow up ultrasound and possibly another mammogram (after my 6 months follow up mammogram last week). I'm now feeling pretty anxious. Actually make that scared... Luckily I only have to wait until Saturday for the follow up to the follow up.
Mine was a very tiny (3 mm) intermediate grade DCIS found by mammogram, followed by core biopsy, followed by wire guided biopsy/lumpectomy, followed by radiation - 7 months from start to finish. I am hoping not to have to go back down that road again, but regardless I am grateful for the tests that are catching these things.

akmom, did you have a mammogram after your finished treatment? If not, then your breast looks different now, with scar tissue and possibly new calcs that developed from the radiation, and that could be the reason for the call-back. Good luck on Saturday. Hopefully everything looks fine on the next set of films.

akmom, I am so sorry to hear this ... best of luck to you for benign results. I had a "new baseline" mammogram 6 weeks after radiation. I am seriously hoping it reduces biopsy need in my future.
It takes longer than a few months to develop after lumpectomy and radiation - median time is 24 months, according to this paper: http://www.ajronline.org/content/188/2/393.full
... but, it doesn't mean they aren't still associated with other benign changes.

Thanks everyone, and thanks for the link, BL. I like the conclusion: "Newly occurring calcifications in the treated breast are usually benign, and they can be managed conservatively in many cases by using morphology and pattern of distribution as a guide."
The tech told me I was scheduled for mammo only on the treated breast, but since it has been over 12 months since my last bilateral mammogram she went ahead and did both breasts (and hurray for her for taking the initiative and not just blindly following 'orders'). So I'm not even sure whether the area of concern is in the treated breast or not - I hope so, because then as Beesie says it may just be scar tissue. Fingers crossed!

Turns out the area of concern is in my 'good' breast. The ultrasound tech spent a very long time looking at the spot. Then had another (diagnostic) mammo on that side. Now the waiting.

Interesting thread. I'm due for a mammogram soon on my remaining breast. My initial DCIS was diagnosed by mammo, but not visible on ultrasound. I started out with lumpectomies (no rads) and ended up having MX.

I too, am due for my mammogram in February and am concerned about the calcifications on my non-affected breast. Beesie, I never had a mammogram post treatment. Is this the general rule? I am also worried about the actual mammogram on my affected breast too, as it is pretty sore and tender after the lumpectomy and rads.

Keep us updated on the mammos and ultrasound findings ladies.........Since I started this thread I will give an update on my mammogram situation.
I was diagnosed 2 years ago and ever since then I have been unable to get a clear mammo. By clear I mean a bi-rads 1 or 2. It was always a bi-rads 3 that kept me returning every 6 months. This was starting to become very discerning to me after the 3rd one. So, yesterday low and behold, I ended up with my first clear bi-rads 1 mammogram in 2 years. It was a complete suprise at this point. My surgeon had already mentioned biopsy if a 4rth bi-rads 3 was given. I guess what I am trying to say is to not get discouraged (easier said than done). Whew, I feel like I dodged a bullet on this one. The radiologist actually said that what he was seeing on previous scans was mostly gone. I feel good in that he had so many to compare with in making that assessment.
For now I have a reprieve and I'm going to enjoy it while it lasts......the future is never certain for any of us. If something shows up later down the road I will have to comfort myself with the fact that I am following the guidelines set forth and doing all that I can do to keep myself recurrence free and healthy.

Mamglam, I was worried about the mammo also as my breast is still tender (finished rads May 1) but it was actually not particularly painful - I've had much worse during past screenings. Hope yours will be okay.
Hi gumshoe, I've been lurking on the Canadian threads and keeping an eye on you! Sounds like you've been busy at work, me too. Why the heck can't life just give us one thing at a time, instead of piling everything on all at once???
Hope everyone gets good results, I am going to keep watching for your posts.

Not yet, I think I'll have to make a phone call tomorrow and follow up myself. It's probably a case of "no news is good news" but I don't take anything for granted anymore!

Hi gumshoe, my doctor's receptionist said he had "signed off" on the results and she wasn't told to call me in to review them. Hmmmm. In the circumstances, you'd think they would know I would want to hear what the ultrasound and diagnostic mammo showed... But I'll give him the benefit of the doubt - he usually goes too far the other way and calls me in for nothing
I'm sure it will be a false alarm, anyway. Thanks for asking!

Akmom, sure, call them and tell them you want to talk to the doctor about the results.
On the subject of mammograms, I had a clear mammogram and ultrasound, but I insisted something was wrong with my breast because a teeny droplet of clear liquid came out of the nipple every other week, it was merely a stain on my nightgown...The doc tor wanted to do a needle biopsy to appease me because I wouldn't back down. I am a very nervous person...and I asked for a surgical biopsy figuring that might find the problem.
The results came back my breast was filled with DCIS, I lost my skin, breast, nipple...everything. Probably because the mammogram and ultrasound failed me.
The MRI did show the cancer...and several doctors said I had the DCIS "quite a while" (years) to get to this point. It was very frustrating, and I never missed a mammo in 25 years. Thinking i was safe....

Wow!! In 2007, I had clear discharge from my right nipple and had several mammograms and a failed ductogram (that was a crazy experience). Come to find out it was a papilloma that was benign that I finally had removed in 2010 because the discharge turned colors.
This year, I over slept and miss my annual mammo. I had decided to just wait until next year because I had not felt anything and neither did my gyn during my annual exam. Well my hubby strongly encouraged me to re-schedule. After much protesting I finally had it done...suspicious calcification, MRI, biopsy, DCIS 5 cm, High Grade with necrosis.
I ended up with a MX because of how large it was and the fact that it was high grade and age of diagnosis. As much as I hate mammo's...I will never consider putting one off again.

I have been weaning myself away from the boards here, and haven't visited for weeks. Apologies for not posting my results earlier.
My new baseline mammogram, 4 months after the end of my rads and 6 months after my last surgery, was BIRADS 2. I am immensely relieved, as I had no trust that they got it all in April.
