Hi, this is my first time on this site. I am 64 had a breast reduction at which time the tissue was bx and adh was found. My plastic surgeon sent me to a breast cancer surgeon . She had her hospital review the slides of my path report as they were from another hospital. Breast surgeon called today and told me that the path. From her hospital says they don t see any atypia which the first report did see. She still wants me to have genetic counseling because my 2 aunts had bc and my brother has colorectal cancer which could indicate family gene mutation. My question is this. Because of the difference of path report findings, should I get a third opinion on the pathology. I do have an appt. for genetic counseling. I am happy about the second opinion but at the same time scared.the first report was reviewed by two pathologist, don t know about the second. Any thoughts would be helpful.

Hmm... well, certainly if one pathology report was ADH, and another pathology report was DCIS, I'd personally want a 2nd opinion on pathology, because they treat / can treat ADH differently than DCIS.

I think its really important to get a 2nd pathology opinion if they were going to treat you differently depending on their results. (Some breast surgeons/oncologists offer patients with ADH tamoxifen treatment.) Your treatment options may change if genetic testing shows you have a deleterious BRCA mutation. Your genetic counselor should be able to advise you about your potential risk for a deleterious BRCA mutation.

Part of your decision may be whether or not your insurance will pay for 2nd pathology opinion and/or genetic counseling/testing.

FYI: I have LCIS and ALH, and I ended up getting a 2nd opinion on the pathology. I paid for the 2nd opinion out of pocket, but it ended up that my insurance paid for the 2nd opinion. (My diagnosis only changed from classic LCIS with cells with characteristics of ALH to (on the 2nd opinion) classic LCIS with ALH.) So my 2nd pathology opinion didn't change my treatment course.

I am going through the same think. I had a lumpectomy two months ago- and at the hospital where the surgury was done adh was seen. At that time my BS sent my slides for a second look. This time FEA was found. Which from talking to my doctors means my risk is only minimally increased. Went to an oncologist and he sent them to a third pathogist. Where no atypia was seen. The 2nd a d third were from breast pathologist so from what I am told they r the experts. I go back next month to talk to the MO I think just closer monitoring is what I am going to be told. Sure hope so. A third opinion doesn't hurt.

This was very much my situation-- first needle biopsy yielded a dx of DCIS. I switched hospitals because the BS I wanted to use was at a second institution, they reanalyzed the slides, and the result was ADH. My oncologist was at yet a third hospital and wanted the slides looked at a third time-- ADH dx again. Decided to go with the best 2 out of 3, altho all my doctors insist that the tx would have been the same regardless (it was all removed by the needle biopsy, so tiny sized lesion.)

Thank you all for your posts. I saw a genetic counselor today. She feels that I m not at high risk for any family risks but my Gail score was still 1.7. She said that is a little high and says I meet the requirements for preventive treatment. I also suggested another pathologist review and she thought that was a good idea. I see my breast surgeon soon and will question her about that. I feel better that the counselor doesn't t feel I need any testing but am concerned that they are only suggesting a follow up mammo in a year.
