HI all, I am so disappointed myself...just had consult with PS, not only am I a lousy candidate (38DD at least, but pitosis is extreme.) I'd not want to have two surgeries with the good chance of necrosis and losing the nipple, to keep it.

anniealso - So sorry....hugs to you! By two surgeries you mean like a reduction/lift before the NSM? Ugh, nothing about BC is easy

Theodore Tsangaris, MD at Johns Hopkins in Baltimore. He offered nipple sparing despite my cancer diagnosis. Clear margins post-op and I got to keep my nipples. Excellent surgeon.

Dr. Tsangaris at johns Hopkins performed mine as well, april 2010. I agree, excellent surgeon. Dr. Manahan performed a bmx with DIEP. Despite the challenging journey I have faced, I was blessed with their skills and experience.

I had BMX but nipple sparing only on one side. Cancer too close to nipple on other side. I went to Lima Linda hospital in Redlands CA. -

I ended up being treated elsewhere for reasons unrelated to Dr. Kakkis, but Jane has an excellent reputation and I was very impressed with her.

I am confused. I thought with a double mastectomy rebuild that nipple sparing was a good idea. My surgeon advised against it today saying that you have to preserve it with breast tissue and that puts you at some risk. Have you all heard the same thing? Is it worth sparing nipples?

It is worth sparing nipples if the cancer is far enough away. Safe as well as for a prophylatic mastectomy.

Lisa, retaining the nipples adds a minute risk, approx 2-5% the most, according to research. Very personal decisions. It was definitely worth keeping mine, wouldnt have it any other way, unless I had no choice. Remember, the choice is yours and your alone, if u r a candidate. Many docs who do not perform these highly technical procedures will poo poo them as well. If u want nipple sparing, find an experienced surgeon who has performed many NSM's. Good luck!

Kd6blk - Unfortunately, NSM is not offered in my local area. Reason, I am told, is that the breast surgeons are willing, but we don't have vascular microsurgeons here that do breast reconstruction like DIEP, etc. Fortunately, my wonderful local BS knew I was a good candidate even though she couldn't offer NSM herself. So she arranged for me to see a BS/PS team 2 hours away. It's been difficult (had some healing issues), but definitely worth it! I would recommend pursuing NSM even if you have to travel a bit to find it. As LISAMG wrote, many docs who can't or don't offer a procedure will sometimes just not bring it up at all or downplay it.
My BS followed a pretty common routine, which is to core out the tissue behind the nipple for separate scrutiny by the pathologist. My cancers were not near the nipple and I was not very large before MX. I knew if the pathologist saw anything initially during surgery the nipple(s) would not be saved and the possibility existed that one or both would have to be removed after the fact if more extensive pathology showed any cancer in that retro-nipple tissue.
I encourage you to at least schedule a consult with a BS who performs NSM even if you opt not to go that route. I think it will help you to know that you at least checked out all of your options.

I had a nipple sparring bilateral mastectomy with immediate alloderm implants---the plastic surgeon who performed the implants is Dr. Chasan in Del Mar, CA and the general surgeon is Dr. Sherman in La Jolla California.

I had skin/nipple sparing bi-lateral mastectomies with reconstruction on 8/21/12 in Nashville, TN. Dr. Brian Tierney was my excellent PS but without my knowledgeable and skilled BS Dr. Pat Whitworth it would never have happened. I had read about the procedure, but just barely as there were not many write ups about it that I had run across. As soon as Dr. Whitworth named it as one of my options I jumped on it! I had a small malignancy on the far left edge of my left breast and had the right removed for prophylactic reasons (strong family history). I have yet to have my TEs filled (only 4 wks post-op) but am thrilled at keeping my own nipples. Both doctors do excellent work!

Ladies, I am new to this forum, even though my surgery was just over 2 years ago and I'm amazed and disappointed that NSM is not more broadly known at this point. It seems to me that women looking for NSM options need to scan hundreds of posts in this topic and the other NSM thread, unless there is something I'm missing. I have spent a little time pulling some of the information together from this thread and made a very rudimentary website to try to capture everything in one place, and sorted by state. salarie - Tampa Bay Breast care also does NSM. I can also give you a link with some stats and photos from the Univ. of S. Florida on NSM which has some good info, but the photos include both surgical photos (graphic) and "before and after" shots.

Hello,
I am in Ann Arbor Michigan, had a lumpectomy in August by did not get clean margins. I did chemo while I did genetic testing, am just completing chemo and have a variation on the PTEN gene. I am very interested in saving my nipples, for the pleasure as well as the appearance. The U of M doctors do not do NSM.
I am looking for doctors in Michigan/Ohio/Chicago?
For those who've had the NSM, what is the physical sensation in the nipple like?
The Cleveland Clinic website has an article by Dr. Crowe referred to "skelatonizing" the nipple, which I presume means killing the nerves and removing all the tissue. Can anyone explain that?
