Janny99 - did your doctor perform microfat grafting? Did you use the BRAVA system?
I'm sorry most of you fat reabsorbed.
If performed correctly fat grafting has up to an 85% retention rate so you are not wrong to have been disappointed.
lisagwa, 1 1/2 years seem incredibly long to finish the full grafting. Mine should be complete in 6 months.

Lizdehart- are you doing brava? If I decide on FG I would not do brava because my PS doesnt use it. My understanding is that the FG would take several treatments and I have to wait in between to heal before each treatment. Plz let me know what you were told and about the 6 months.

I am doing Brava. I believe the consensus is 4 weeks after the mastectomy before one can begin BRAVA. It will be 10 weeks for me due to logistics and then I will wear the BRAVA for 4 weeks. I will be having my first grafting the end of May and another about 2 months later. I was not TOLD 6 months but my impression is (I will check with PS to be sure) one can have a grafting about every 8 weeks. If it takes 3 surgeries (one grafft done at mastectomy + 2 subsequent fat grafts) the process should take about 6 months.
Would your PS be willing to train on the BRAVA system?

My PS knows about Brava but he hasn't expressed interest in using it. I know he has created a single breast without using Brava and I believe he is in the middle of creating double using FG only.

My PS had an interesting question that maybe you all will know. I had mentioned to him that a lot of docs note on their websites that their micro fat grafting retention rate is 80%.
When I saw him yesterday as a followup to the grafting I had done around my implants to improve the area of thinner skin and increase fullness in the upper poles, we were talking about the retention. So far mine is really good.
He wondered what docs use to measure the retention to know it was 80% (or any other percent for that matter). Said they would have to somehow measure before you had it done and then keep measuring somehow for a while.
What do you think?
I can tell you, that mine is a little bit less than it was a month ago (although some of that could have been swelling from the procedure), but I would have no idea how to determine a percentage.

fluffqueen, that is an interesting question. I have read quite a few articles on FG, I have not seen any data that quantifies retention rates. Not even animal studies.
I think most PSs just guestimate. I think I lost 50% with my first round and 4 weeks out from 2nd round I have maybe lost 20%, but expect I may loose more. And as you note, some loss could be reduced edema too.

MIrcrofatgrafting--on April 18. Surgery went great. Went from a 410 to a 213cc implant. Most of my issues are resolved. Definitely the correct decision. Minimal recovery. Eileen

It's exciting to hear that your issues were resolved and you feel already that it was the right decision, Eileen! My insurance company finally (just last week) okayed the procedure and I'm set for next Monday. Good to hear the recovery went well.

the smaller implant with fatgrafting has improved my situation by 90%. will still be going to total fatgrafting with no implant.

Regarding measuring of fat retention, in my case (as part of the original government clinical trial by Dr. Khouri) it was done by MRI. I had a baseline MRI and at least two others...maybe after six months and a year, so it was able to be scientifically measured. Wish I would have asked my exact %, but didn't think to at the time.
In checking out other trials, one compilation of studies with a study pool of 820 subjects indicated measuring by 3D interferometry (not sure what that is). It cited 30 to 40% fat might be reabsorbed, but sounds like it involved many approaches...not sure if any used BRAVA. Seems indications are quite clear that BRAVA use yields better volume and retention results....though sorry, I don't have specific study to cite....have just heard this cited many times from various sources. The results can also be very technique dependent (per Dr. Khouri and anecdotal)..
In my case, I know I lost maybe about 15% (questimating) on basis of my bras still fitting almost exactly the same as about 3 months after completing fat grafting. It's three years out for me now.
Was nice to see in the compliation study, that the satisfaction rate was 50% very good; 40% good and 10% moderately good. There was very minimal complications and no suspicious masses noted with follow up mammograms.

Doesa your Dr utliize the same type of canullas for liposuction and than cenerfuge the fat using Dr Khouris made centerfuge and then reinject using his mapping system of microfatgrafting? I am wondeing if any changes are made to this procedure would it decrease fat retention? For sure BRAVA helps but it seems Dr Khouri also recommends that the collection and type of centerfuging is also important. What do you all think?

I had a chance to watch Dr Khouris presentation on his procedure you recommended and referenced at the bottom. It seems to me that his fat retention rate is due to his centerfudgng process and the mapping he uses with small droplets to be injected are just as important as BRAVA expansion...what do you think? I have heard of a surgeon who would do the procedure but lipo 2L instead of 3-4 L as Dr Khouri and then this means he would only inject that which he lipoed. Also he would not use Dr KJhouris centerfuge process or his valve and am wondering if this should affect the result and fat retention? Would it be worth to see a Dr who does it this way? Just wanted your thoughts?

Honestly Mina, I don't know! I know he is very careful with how he harvests fat, b/c if it isn't injured during the harvesting then of course more of it will live and be retained. I also know that injecting it in microdroplets is critical so that it doesn't all amass together in a glob (if that happens, you are more likely to not get enough blood to the fat and thus it is more likely to die). That is the extent of my knowledge on it, though. I don't know if that helps?

I was just diagnosed with a recurrence (after 9 years!) and am told I must have a UMX. I am trying to do research before meeting with a surgeon. I am thinking of UMX with immediate DIEP. I think micro fat grafting would be good. I am just getting started thinking about all of this. Does anyone know of a good PS in the Los Angeles/Santa Monica area who does DIEP and micro fat grafting?

Hello everyone! So sorry that I did not respond to the questions as I've not been on here again since I last posted...it's been a busy few weeks.
My plastic surgeon is highly recommended and I have the highest respect for him, but I do think he is new to the fat grafting procedure. I did not use the Brava system, and after reading all the posts, I think that if I decide to continue on with this, that I will ask him about that, perhaps the fat retention will be greater. I also did not use any compression for my tummy, and think that I need to at least wear "spanks" or some type of support to help reduce the pain and swelling I had from the donor site.
He was honest with me, in that he feels it will take most likely 3 procedures to get the fullness I am hoping for. Sometimes I wonder, after being through so much, is this worth it?

janny99, I understand your comment about wondering if it is worth it. I was definitely willing to go through 3 procedures and the extended Brava wear to get beautiful breasts. However, since my reconstruction was interrupted after two procedures (the 2nd one having to be done before being maximally expanded) b/c of needing radiation and chemo (surprise!), I have my doubts about doing additional procedures. Now, to fat graft a radiated breast, I would probably need 2 or 3 more surgeries to get the size I'd like, vs just maybe one more procedure for my nonradiated breast. There are several women who amaze me with their determination and patience with having 4 or 5 surgeries after radiation. And they have achieved AMAZING results. Just not sure that I am up for it. Ah well, I don't have to make a decision yet b/c I'm getting chemo and Herceptin so it will be well over a year before I can even truly contemplate it. I suspect that by that time I will have more than enough medical intervention to last me a lifetime and will continue to be happy with being an A cup. That's what I was before my BMX so it's no difference really. We'll see. I may change my tune by then. Perhaps it's like women who have a baby. They forget all the pain of childbirth once they have it and decide to go for more! LOL

Did not find the discomfort from fatgrafting to be unreasonable. Lipo on thigh areas healed rapidly. Was back to work in a short period of time. For me fatgrafting--is a gift. Definitely willing to go thru two more surgeries for the desired results. Eileen

Well gee, I hope I didn't come across as negative about fat grafting! I am thrilled with my results! Just bummed that I had to have radiation and if I want to get bigger I need way many more procedures than I feel like going through. Just complaining about my situation, not the fat grafting at ALL. I was willing to do 3 surgeries total...not so sure about 5 or 6 surgeries total.
I know ladies do it. I just don't think I have the strength. I have so much respect for those who push through 5 or 6 surgeries and Brava!!!

I'm about 2 weeks behind Eileen with the same surgeon. I don't find the pain unreasonable, either. What gets me is the post-surgery nausea, and the thought of having to go back to the hospital at least twice more is not so much daunting as just a drag. I'm tired of being a patient. But I hope that by this fall I'll be in a better mood about it.

Never used any pain meds. Just love the look of my legs. Thighs look great already. 90 Percent of my pain from the implant is gone--and I must admit I now have "young breasts"--high, perky--my outfits look better than ever.
So glad I kept my old outfits. My lymphedema is gone. Energy has improved. Yes--surgery is a bit of a pain--but definitely worth the results. My first surgery was the most complicated, so Dr. Ahn says I will only have to miss a few days of work.
Looking forward to totally removing my implant. Eileen

Hi everyone, could someone tell me what BRAVA means regarding the micro fat grafting? what the acronym means? and, I am very interested in doing this and have read thru these posts that your insurance may take it due to it being "breast reconstruction" - my question is, will it cover more than one procedure due to hearing from these posts, that the retention is not great? Thanks everyone for any help. This sounds too good to be true.

Hi. I am also interested in anyone that has done the brava. I had a double mastectomy in Jan 2011 and I have had three expancers input with two of them getting infected. Five years ago I had a tumor on my spine and I had to have four bones removed in order to get to the tumor so doing a flap is not an option for me. My surgeon will not implant anything else but my fat (he says I scare him). Now I am fighting my insurance as they will not pay for the brava (no code and they say it is experimental) but they should cover the lypo. Please anyone that may have advise, sure could use it. I know there are people like Suzanne Summers that have done stem cell but she has a lot of money and then there are us that are the real world. Please anyone with some advice?
