neulasta first step program

Thank-you BC/BS for waitingl less than 24 hours prior to treatment to tell me you my request for continuity of care is denied WTH!!

Does anyone know how to apply for the neuplasta first step program....it will apply up to $5000 towards copay/co-ins for in a 6 month period
