Please bump SN biopsy thread!

Sorry, can't find a thread regarding the benefits of having a sentinel node biopsy before a BMX. At my second opinion appointment today my BS recomended a sentinel node biopsy- bilateral. I have no breast cancer in my right side so I'm a little confused. Also, why wouldn't I just have the biopsy done at the the time of surgery? I got a second opinion primarily because dr. #1's PS didn't take my insurance and I was really interested in immediate reconstruction with TE (mainly to skip an additional procedure.) but my second opinion is suggesting the Bilateral SN biopsy one week before my BMX, which would be another surgical procedure!!!!! Can anyone shed light on a route to take? Anyone else have the SN biopsy before surgery?

    Had mine done same time as surgery. It is a staging tool. I don't know why if you are having a MX they wouldn't do it at the same time.

    I had mine done before surgery.  When my surgeon did it - he saw the tumor right there so he just took it out even though I was planning a BMX.  I was glad I not only had SNB results, but the tumor pathology before surgery.  Once they did the BMX I did have one other small area that was invasive so I did not find that out until after BMX.  I don't recall for sure, but I thought the SNB before was more accurate than the one they do during surgery.  It has been a while so read up on that or maybe someone else can comment on that and provide more information.

    I had a SNB right before my BMX surgery. They scheduled the SNB procedure at 7 am with the injections of dye and pictures....then at time of BMX surgery, they remove those node(s) that the dye goes into to test for malignancy.

    I had mine done during my left mx.  As far as I know they will not do it any sooner.....at least in Canada.

    I went for the dye injection the day before surgery. Then I had the SNB done at the time of mx. They sent it to the lab while still doing my mx. SN was clear so they didn't need to do an auxilliary dissection

    My second opinion BS wants to do it at least 7 days before my BMX. Guess I need to have more discussions with her. Thanks everyone for your input :)

    So my snb is tomorrow at noon. My BMX is unofficially scheduled for March 8 (three weeks after snb!) I've been warned that the snb is incredibly painful so wish me luck!!!

    My sentinal node was performed about an hour before I was wheeled into surgery for a lumpectomy.   Cancer had already been diagnosed via a core biopsy.

    I had my lumpectomy and SNB at the same time.  I was injected with dye and scanned, then taken into surgery where both were done.  It was not painful at all...a non event.  I recovered quickly.

    I imagine your surgeon is doing the bilateral SNB as a precaution.  It never hurts to be absolutely sure.

    I had both my lumpectomy and the SNB as part of the same surgery.  It was an in-and-out procedure; I checked into the day ward around 9:00 am, had the dye injected; had the surgery; and was home for dinner.  No stitches, no drain, no pain.  Within 24 hours, I ditched the bandage and wore a gauze pad in my bra; much more comfortable.  Didn't need any "down time" at all, was back to hiking, biking, and doing my regular activities within a few days.

    I had the same experience as SelenaWolf and Kaara. My SNB and lumpectomy were done at the same time, and I was "out" for both. Recovery was easy...just a little soreness. I even went to a movie later that evening.

    I had dinner out with my husband to celebrate that "... cancer was gone...".  The next day, I was bandageless; just had a gauze pad in my bra.  Three days later, I went on a 20 mile bicycle ride.  Ironically, the "cancer" part was easy; it was treatment that was HARD.

    Hi,

    I had SNB before BMX with DIEP.  I think it was for staging.  I had neoadjuvant chemo.  So I "think" if your nodes are not clear, you might need additional scans and maybe treatments.  It takes time to recover from BMX so maybe they want to do just SNB to make sure you don't need anything else or they can start chemo ASAP and do BMX later if need be.

    I think Pickle, above, may have given the reason it's being done this way. If the SNB is done before surgery the doc can find out in advance whether an axillary node dissection will be necessary. That is, if no cancer is found in the SN, they'll leave the rest of the nodes alone when you have your BMX. 

    While some testing of the nodes can be done during surgery, my understanding is that the during-surgery testing is sort of an educated guess. Sometimes the nodes appear to be clear during surgery, but when pathology is complete it turns out there was an invasion. (And then you get to go back for another surgery! Hurrah! -- not.)

    I think the surgeon is doing you a kindness by trying to avoid removing all the axillary nodes. And yes, while the SNB recovery isn't the easiest part of the process, I didn't think it was THAT bad.

    You are completely right cycle path, my snb came back positive so during my BMX she took the remaining nodes in that region. I'm glad it happened in that order, I would hate to have to go back in after my BMX.
