Elevated Alakaline Phosphatase during chemo

Options

Help! A little freaked out... Yesterday when I went for my infusion, I had my bloodwork done and went to meet (as usual) with my MO before my treatment. She examined me and checked my bloodwork on her computer. She said everything she had (checks like 50-70 things!) was good and she was just missing a couple of results, but to go over and get started. I did and they did start the infusion, I asked the nurse if they got the final results and she said yes, they'd get me a copy. Never asked for a copy before. Read it this morning and every test was in the normal range except for the alkaline phosphatase. It was 155. Normal ranges are 34-104. Before I started chemo it was 63, after first tx it was 145, now 155. This is all on the paper in columns. I called my MO and she said she wasn't concerned...feels that is was normal before chemo and is most likely caused by the chemo or the Neulasta. But, she offered to send me for a bone scan while trying to assure me that she's just doing this to ease my mind. Anyone else look at all their bloodwork? Out of normal ranges?

I get a copy of my bloodwork every time I do it.  I thought all oncologist's offices did that!

Abnormal numbers are so common during chemo that it would be more surprising if it wasn't off.  I'm glad your doctor is willing to order tests that will ease your mind but I bet you are fine.

Good Morning Momof3boys, Greetings from Australia.

I can understand you freaking about the elvevated levels of ALP, as I also did the same.  Mine was normal after having 3 lots of surgery and radiation and I did not have Chemo.  About 3 months after starting the AI, Femara, my ALP started to gradually climb, each 3 months it was up each time, until it reached 155, and yes I was worried, as I had a positive node.  My Onc, was not overly worried, maybe he was, but certainly didn't show it.  He just said  at that level he was not overly alarmed, and suggested we just watch it.

I guess because I had had an oophorectomy 4-5 months earlier, and also being on the AI, I think he thought it was being caused by bone loss which they both cause.  After each test, I would sweat it out, waiting for the results, and it stayed on the 155 for about 12 months, and then gradually started to fall.  My last test was last June 11, and it had finally lowered back onto the edge of acceptable.  So I am holding my breath for my next test in June 12, to see if it is still stable or has risen again.

    regards Ched

Momof3boys, my liver enzymes were a little high a few times during chemo too. My last chemo was last week and a week later they were a liitke high again. My onc was not concerned at all, although she did get me an ultra sound and bone scan because I kept asking for some tests (my onc doesn't test tumor markers). I do ask to see copies of all blood work.

Thank you everyone for your reassurances. Boy, BC is a whole new world! Never even had headaches before, now every ache and pain is gut wrenching.

    Hi Momof 3,

My ALP is now 115. Its been rising over the last 3 years. I had Lump, rads and no chemo. I have blood work again on the 20th. If it rises again, I'll be asking my Onco too.

I did read it can increase from rads and chemo so as you are trying not to worry. I'm hopeful you are fine and this post BC stinks. All worries all the time. Prayers being sent to you that you're healthy!

Thanks Bevin... And thanks for the heads up that it can rise during rads. I'm in the middle of chemo and looks like rads may be next.

Good luck with your tests on the 20th! Prayers to you too!

Good luck with the rest of chemo tx. I'd assume you'll have rads too. Let us know how you make out.

I am having the same issue with elevated ALP going for a liver Ultrasound on Wednesday and then to schedule a bone scan. I finished the Taxol/Herceptin in Nov and the Dr said my ALP levels should be back to normal by now. She said she held off on getting me the scans because it could be due to the Nuelasta. My ALP was 147 three weeks ago and now 149.  My AST is 47 and ALT is 39 which are also slightly elevated. Everything else in my bloodwork looked good the Lymph and EO were also slightly elevated. I am so scared of this spreading to my liver. She also did a Vit D test and GGT but those were normal so she wants me to get the scans. I was diagnosed 3/2011 with stage IIB had a double mastectomy 4 AC and 12 Taxol/Herceptin and am now on Herceptin every 3 weeks. No lymph node involvement with 3 tested. I am also experiencing some low back pain but was thinking that was due to the tissue expanders and my poor posture.

Also have an elevated alk phos.  It was normal after I completed chemo, a year and a half ago.  Did get Neulasta during chemo.  Started arimidex a month or so after that and alk phos has been slowly increasing since then.  Had an ultrasound a month ago, which showed a fatty liver (an us 10 years ago was normal).  Haven't talked with my MO since getting the US results.  Will get repeat alk phos in a couple of months.  Am certainly concerned.............
