Direct to Implant and AlloDerm Breast Reconstruction
I am considering Direct to Implant and AlloDerm Breast Reconstruction instead of using the standard expander tecnique.
I'm hoping for feedback from anyone who has had this procedure.

This is what I had done. I had no problems with any out of the ordinary issues. I think that it was emotionally beneficial to wake up with a breast, although not attractive. I am so relieved that I didn't have to go through the TE process - one less thing to deal with. My surgical team does this process routinely as long as the patient has healthy skin and was a non-smoker. Anything that can make this BC crap a little easier, I am all in! I will say that the only negative was my new boob is actually a bit, little bit, bigger than my real side even after the reduction/lift. But, they weren't perfect to begin with either.

Not a single problem. I am glad to have avoided TE's . I look almost exactly as I did before. My ps was honest and did not promise me anything. He said if the one step didn't work cosmetically in the OR, he would use TE's. Happily, it worked out.

I had a UMX/direct implant with alloderm in June. Even though I did not have to have an exchange surgery, I did have another operation 5 mos. later. We did a lift on my remaining natural breast and made a nipple for the foob from the MX scar. Right before I was being wheeled into the OR, I mentioned to my doc that I was not real happy with the position of the implant and that it would move toward my armpit when I lifted my arm. He agreed and while I was in surgery, he removed the implant, took some internal stitches along the outside edge of the implant pocket and then put the implant back in. It looks much better.
As far as pain goes....I had a skin sparing MX and I was a D cup size and I am now about a full C. So I went down just a tad. I don't think my pecs would have stretched any more to accommodate a D size implant and I had just enough skin left to cover the C size implant. I used Soma (muscle relaxer) for several weeks to help with the discomfort. I had almost full range of motion on that side within 3 days. But, I am pretty "Gumby"....very limber and hyper-mobile and I practice yoga pretty regularly.
I'm all done except for the tattoo. They really look pretty good. The lifted natural breast looks amazing. I've never had perky girls until now! If you have confidence in your sugeon's ability, and they feel like you are a good candidate size wise I think you should consider going that way. Just know that you may still need some tweaking at a later time. Best of luck!

I had BMX direct to implant on 1-23-12- my non- cancer side came out great - my nipple from that side is in my belly my PS is going to create 2 new nipples out of it. My cancer side did not go so well -the implant is not postioned correctly and I have a large area with nothing and my skin has folded. The PS said it is fixable and I also saw my BS who said it was fixable just another surgery I did not plan on. I would do it again if that helps I am so happy with the other side and I am sure once this side is fixed I will be happy.

Glad I found this thread. I will be getting UMX/direct implant with alloderm on March 1st. a little scared, but was advised since I cannot take the time needed off from work to have the shoulder or stomach tissue done.

My first appointment with the PS is today at 2:30. His name is C. Andrew Salzberg, MD asociated with NY Group for PS in Tarrytown, NY. In addition to this direct implant with alloderm - I want to know more about GUMMY BEAR - cohesive silicone implants. From what I've read, they are not FDA approved and are used only in clinical trials. Call me crazy, but I love the sound of GUMMY BEAR IMPLANTS almost as much as I like the sound of PIG SKIN
I am also meeting with a PS that works with the doctor I saw at Sloan Kettering. This PS does NOT offer direct implants with alloderm. It will be interesting to listen to their opinions.
So glad to hear from all of you! I'll update you on my progress. For those of you who are new to me, I'm went a bit nuts after my diagnosis. The day before my diagnosis, my husband lost his job and along with that went our insurance. Then our marriage took a huge turn for the worst and then I alienated his family FOREVER... And then my father died. I'm trying my best to pick up all the pieces and put some normalcy back into my life. BUT it isn't easy.
Here... at BCO is the only place I feel safe. Thank you everyone....

I have a "gummy bear" implant. My PS said he has been using them for several years and has yet to have a rupture or other problems. Mine feels soft and pliable. Not my natural breast, but close.
I am so sorry to hear of all of your troubles. Man, when it rains it pours sometimes. Hang in there and when things seem overwhelming, stop and try to focus on your breathing and think of something that is good in your life. I find that helps. Good luck with your appointment.

Greenmonkey - FYI - Mentor also makes cohesive gel implants and they are readily available from any plastic surgeon.

I wish I would have heard about the direct implant before my surgery. But given the problems I have had, maybe it is better I went the route I did.

Hi DM - you are funny! I still have TE's, not scheduled for exchange until the 24th of this month but I will be getting Mentor Cohesive Gel implants. You would probably have ended up with the same problem as direct to implant involves skin sparing so the implant needs to occupy the skin and be the final size you wish to have at the time of the surgery (or can safely have). Sometimes direct to implant girls have additional problems with capsular contracture since they do not have the incremental pressure provided by a TE that is increasing over time from the inside out.

I had the same procedure and am happy with the results. It's pretty, but not perfect - slightly different shape and a little smaller. But as they say, in clothes no one knows. The surgery gave me almost no problems and I was happy not to have the wait for tissue expanders - quick and "easy" (or at least easier).
You should know that you are in very very good hands. Dr. Salzberg is a pioneer in the one step direct to implant surgery, so you have an expert surgeon. He should give you great results.
And yes, the decision process is so hard. It gets better, and you have friends here.

ladies how long does the tingling and pain go on for - it is the weirdest feeling king of painful -my surgery was January 23rd - months ?

Facing uniMX on Feb 23rd, will be talking with the PS again prior to discuss the plan. How do you decide what to do when there are so many options? I know that I can have skin-sparing but not able to keep nipple. I could go with the TRAM but not crazy about having another huge incision to heal, I have also heard that it can make your abdominal wall weak. I'll be 55 in April and am leaning towards an implant, but am fearful of the possible future rupture. I am about a C cup and want to stay the same size.
How do you know if you will need tissue expander? What questions do I need to ask the PS? Any other info would be great.

Hi Janice, I can't advice because I'm new to all this but I'll share what little I know. I audio taped all my appointments because I forget things. Funny, I grilled all of the cancer doctors but I sort of just listened to the PS. I personally like the idea of direct to implants. my doctor is really good at it - he's done over 5,000 of them (that might be a good question to ask). they are going to try and save my nipple. I like that too. I was initially freaked out about the mentor, cohesive gel implants but I'm going to calm down about it. I do things regularly that are statistically a lot more dangerous. I don't have my surgery date yet. My next step is MRI on teh 21st.
How are you holding up? When is your next appointment? have you gone for more than one opinion? do you feel you need to? I am seeing 2 PS just because of insurance reasons. The doctor I want to use is out of network but the in network PS does not do the same tecnique. I know for certain I don't want expanders and I don't want to use my own tissue. but thats just me, we all have our own reasons.

Hi - I did direct to implants - on my non cancer breast my ps saved my nipple it is insterted on my belly getting blood supply - my ps plans on splitting it in 1/2 to make me 2 new ones- I am a C cup but a broader chest so she used mentor medium plus ( i think they are called) I did not want to go bigger. I do need a second surgery one of mine is good but needs some skin removed and the other which was very high is now very low but my ps says not to worry she can make them both perfect . When she does the second surgery she will do the nipples.
I still have some skin tingling and pain at times if i move wrong, but I am getting sensation back daily, they are firm but not hard -
I had an epiphany one day that this is my skin, my life, my boobs and I better start loving them . My quest now is to find the most comfortable good looking bra I can .

I am holding my own, but the date is creeping up on me -Feb 23. I happen to be dealing with a cold so I hope that is gone in plenty of time as to not change the surgery date. I am sure I will be a whole lot more nervous in the week to come. As I go to some of these sights that show pictures it all looks just a bit grotesque. I know I am going to have to toughen up and deal with the visual aspect of it.
What is the determing factor whether a doctor can do direct to implant? Or whether he has to use expanders? The size of your boob? I would also prefer not use the TE.
I have also found that my insurance BCBS of MN, says that they only pay for reduction of the natural breast to achieve symmetry, therefore if you need to be tweeked a bit bigger they won't pay because it is then considered Cosmetic. Isn't that just ridiculous?

The insurance stuff is sooooooo frustrating!!! I'm trying not to focus on the money aspects of this disease but its not easy.
I don't know what the determining factor is whether a docor can do direct to implants or not.

In my case, my PS thought he could do direct to implant reconstruction, but said he would not be certain until he got a look at me in the OR. He said that size wise, I was a candidate. I was about a D cup going in, but my remaining right breast was a little smaller, so he thought he would be able to use a full C size implant. That is as large as he will go with direct to implant. He explained to me that it all depends on how much skin you have, how flexible your pec muscles are...can they stretch enough to accommodate the implant... plus some misc. things that he sees in the OR. He has been doing these kind of implants using alloderm along the bottom of the pocket for a long time and he just has a feel for what will work. He told me that he takes the right size TE with him into the OR just in case he can't do a direct implant. So, I did not know what I had until I woke up. I looked down and I had what looked like a boob under the bandage.
Just know that if you go the direct route, that a muscle relaxer like Soma will be your friend for some time. I took the Soma for a long time after I quit needing the pain meds.
Again, this was just my experience and your PS may say and do things very differently. That is one thing I have learned from these boards. There does not seem to be much "protocol" for a lot of things that our doctors do. They all have their own ways.
Try to stay calm if you can. All that stress is not helpful for healing and preparing for surgery. Hang in there!

Hi all, I am also in the debate of what to do. The 1st ps I saw would not do a direct implant because she said about half aren't happy with the results. I really don't want the expanders and an additional surgery. I have an appointment with a different ps on 3/1. The 1st one said she would be using donor tissue. Has anyone had this. She also said she couldn't help me on the side that had radiation 7 yrs ago and I have read on this site that many women have. I have gotten used to the deformity on that side, but the thought of waking up with nothing on the other side freaks me out.

Alloderm is donor tissue. It is used to stregthen the pectoral muscle to hold up the implant and/or tissue expander which is heavier than breast tissue.

Hello Everyone! I am happy to have found this thread. Just about 2 1/2 wks out from my BMX , nipple sparing , direct to implant w/ Alloderm. My surgeon performs these quite often and felt that I was a good candidate due to my skin or amount of skin available I'm guessing due to how it responded to breast feeding 3 babies.. I was a "B" cup prior to babies, then sagged on down to an "a". When I discussed this w/ my PS, I explained to her that I would love to be a "B" again but really had no desire to be any larger than that. I went into surgery not knowing if she would actually get the implant in or TEs until my skin was "ready". So I woke up w/ my implants, lucky me! My pain level seems to be improving, trying to reduce the percocet to 1/2 at night only and e.s. tylenol in daytime. At times I get "zings", wondering if I should ask for muscle relaxers. I HATE that dopey feeling! I also get that itchy/tingly feeling mostly at night and find it aggrivating, mostly because I'm tired too. Havent returned to work yet, giving it a good 2 more wks or so since Im a dental assistant and have a pretty physically demanding position. I will be returning PT too. Allergan Natrelle is a line of cohesive gels as well, avaiable in 3 profiles. Implants are not sized as cup sizes, i.e. There is no c cup implant but by cc's. Two people can have implants that are the same cc weight but end up with different cup size. I have Natrelle style 15 , direct to implant, 457 cc's. My bra size is now a 36B or small C, depending on the brand. The Alloderm creates an internal sling so I often don't wear a bra at all!

PS: gummies, considered true cohesive gels, are tear drop shaped. Natrelle's are round. Some have had trouble with the gummies turning but I don't think it's a major issue.

Yes, you are right about the implant not coming in cup sizes. My Mentor cohesive gel implant is a 375 cc size. My PS just talked in terms of cup sizes so that I could visualize how it might look.
