Diep 2012
Comments

I had my first follow up. I notice my stomach is still swollen, so my ps said gets spanks, since I find the abdominal binder so uncomfortable.
So ladies, I saw my Breast. It's flat. Nothing like what I saw with the diep result. I guess that's life.
Anyways, I will be going for my 1 st refill next week. By two months the stretching shud be done, and I once again will have a boob. -
My PS told me I do not have enough fat for a Diep and will need to have expander/implant instead. I wondered if this could be true because I am not a skinny woman (5' 7" and 155 lbs). It seems to me I have plenty of belly fat, etc. I wondered if there are ulterior motives as to why a PS would recomment/prefer a certain procedure. I do not really want to have implants and had assumed that I would be able to get a Diep. I am not sure if I should proceed. I also have a pretty crummy HMO.
Any thoughts/advice would be great. Surgery is scheduled for February 7.

vangoghpro - can you get a recommendation for another PS who does DIEP and then go get a second opinion? I went for a second opinion at one point in my journey and my BS was totally supportive of my doing it. If your doc is not and gives you a hard time about getting a second opinion, that would be a cue to RUN the other direction. Even if you can't do DIEP there are other options that aren't implants. Look around here for the options for surgery, BCO does have some info and it will be a start at least.
There are a number of DIEP docs in the Los Angeles area. If you just want a second opinion and are willing to drive a bit (I see you're in Cerritos), my PS at UCLA was great.
Take a look here too (although I don't think it's up to date as my PS isn't listed but his group at UCLA is): http://www.diepsisters.com/diepdoctors.html
Good luck. And remember, your comfort level is most important and if this doctor won't do what you want, see if you can find one who will.

@vangoghpro - I am 5'3" and weigh 125 lbs. I got two nice DIEP breasts although I didn't have much extra fat. I did not want implants either. Some PSs will recommend only what they know how to do. I would look around for a PS who specializes in microsurgery and DIEP reconstruction. From the ladies on this board, I've read there are a number of good ones in Southern California. Keep looking and Good luck to you! PS - I have an HMO and it covered it all but my out-of-pocket deductible.
-
Vangoghpro - I am 5'2", weighed 128 and was told by my first PS that I didn't have enough fat for two DIEP breasts, so I let him insert TEs at the time of my BMX. Well, we moved to NH and I saw a PS at Brigham&Women's in Boston, and he told me I did qualify for DIEP. So get that second opinion, for sure. I ended up having another surgery to remove my TEs before radiation, and I'll have DIEP later in the summer.
-
Vangoghpro - I am 5' 7" and 155 pounds (sound familiar) and I was told by the PS here in CO that I would most likely not have enough belly fat to make two c cup breasts as I had. I would have to go smaller. I ended up going to the Center for Restorative Breast Surgery in New Orleans. In NOLA I was told I had plenty of belly fat (that is what I thought!) and that if it turned out I didn't they would do a stacked DIEP and Gap which also takes tissue from your butt/hips. I was able to get by with just a DIEP and about the exact same size (maybe slightly larger) than I was pre surgery. I think NOLA is the only practice that does stacked DIEP but I'm not positive on that. This at least gives you some options. It does sound like maybe you should keep looking for that PS that can give you what you want!

I drove my car today two weeks out from diep incisions. It was easy. Went with mom to grocery shopping, I'm taking slowly. Now I just had lunch and going to rest. Good day so far
-
VANGO...I also had a local PS tell me I didn't have enough fat.(he didn't do DIEPs).. so I also went to New Orleans and I didn't have enough ab fat, but I did have enough butt fat.. so I got two c+ breast made out of butt fat,, I did have to have one re-done and then they used my abs to rebuild the one side... So 2 full C+ breasts AND a butt lift... all good.
-
Bdavis- u sound as if you have had lots of surgeries
-
Vangoghpro - I would get a second opinion for sure. I'm 5'6", 130 and my PS managed to get 2 C cups!
-
Red... I have had 3 surgeries for my reconstruction so far... and will have one more next year I think... I plan to let everything settle, enjoy 2012 and then tweak in 2013 and get tatoos.
-
Run don't walk to see a PS who specializes in this procedure. Many, many of us had to travel to get it done and man it is worth it.
-
VanGoghpro - I agree with the other ladies. Definately get a second opinion and use the diepsisters site or this website to find PS who do a lot of DIEP. The PS could definately not be comfortable doing the DIEP so is recommending implants instead.
I'm 8 weeks out yesterday and stopped napping about 2 weeks ago and started real exercise about 3 weeks ago. I go back to work in a little over a week. I am a slow recoverer though
But I look great in the dresses I am trying on for a wedding this weekend with the fairly flat ab, if I do say so myself!
-
Readingmama- did u get any fat bulging over slightly over incision. I'm two weeks out, and I wear my binder which helps. Is this just normal swelling and does it go away in time?
-
Redinrah - I had that swelling after my Stage 1 DIEP. I was told it was normal. I had it until my Stage 2 which was 4 months later. I think I was a unusual in that it lasted that long - but having it for several weeks is not abnormal.
-
just laura-oh kool, thanks for the advice, what did u have done in stage 2?
-
Redinrah - at Stage 2 I had my ab incision revised in 3 places. I had both breast incisions revised. I had dermal grafts on both nipples (I had nipple sparing). I also had lipo to my thigns and hips. And I had fat grafting to both breasts for symmetry. I also had a small scar on my leg that had nothing to do with breast reconstruction revised. It had been bugging me for a long time so I thought I would ask and he was happy to do it.
-
With revisions did u have to take time off for work
-
Red.. I had all that stuff too... and planned my surgery before Thanksgiving, so I took a week off work and then another week for Thanksgiving vacation (I work in a school)

Hi everyone--I had posted before in the Jan-2012 Surgery topic and wanted to provide an update here. I went into the OR on 1/19 expecting that I would have muscle-sparing free TRAM flap immediately following the left MX. I was in the OR for 9 hours and the first thing I remember my husband telling me is that Dr. was able to spare your entire muscle! In fact he was able to do an SIEA flap. The PS was so attentive--nurses in the ICU said he called many times throughout the night to see how the flap was doing with the hourly Doppler checks. Then he showed up at 6:30 am the next morning to check on me. Did anyone else have a warming blanket contraption(they called it a bear-hugger, and I think it is usually used for patients with hypothermia)? Nurses said this PS wants the flap to be kept warm at all times. Here is the weird part that I haven't seen anyone else mention before: Because he was concerned about the SIEA flap 'taking' and wanted a back-up plan, he buried the other half of my belly tissue and my belly button inside my abdomen just in case. He would have been able to go in and do a DIEP with that tissue if necessary. Therefore, as the dr. described to me, my belly currently looks 'half pregnant'. That tissue will be removed on 1/31 in a 2 hour same-day procedure in the Hospital. I feel pretty strange without a belly button right now but it will be back. I have had a lot of bruising and the new 'breast' seems a bit swollen but PS is pleased with the progress. Still taking Lovenox blood thinner and aspirin and he wanted me to avoid chocolate/caffeine. Pain has truly been minimal and I feel so blessed--he said that there is less pain because no muscle was touched. Still waiting to hear from the Breast surgeon about the final pathology and whether any tumor was found--previously Paget's cells were confirmed in the nipple, but MRI etc, did not identify any other specific tumor.
-
I've never heard of the SIEA flap, I'll have to go look it up.
I had the bear hugger - it was very very warm and I tended to take it off of my legs, they didn't need the warm. It's interesting that they saved the other half. My PS didn't but did say he could have given me a third boob (he only did one side) with the rest of the flap, I'm glad that he didn't.
I'm amazed (as were the nurses), I had NO bruising on mine, but apparently it's common to have bruising.
Here's hoping that the pathology is good.

Hi ladies, I am scheduled for BMX with DIEP on Feb 1 in new Orleans at ochsner medical center. My PS is an LSU surgeon named Matthew Whitten Wise who works with Dr. Corsetti from Ochsner, who will perform the BMX. Dr. Wise has extensive experience with DIEP so i am hoping for a good outcome. Wish me luck and I will send an update on my progress after surgery.
-
Redinrah - I took 3 weeks off with my Stage 2. I could have gone back at about 2 weeks but I didn't have to so i didn't push it.
-
LOUIENJ.. I am also from South Brunswick... Where do you live?? And who did your surgery?
-
Ship and tamylyn:
I'm so glad to see someone else here from Pittsburgh. I'm having my surgery February 3rd with Dr. Gimbel. I'm praying for an uneventful recovery. I had some problems in the past with my C-sections and hysterectomy opening back up, but he didn't seem concerned about that. I've tried to prepare myself the best I can. Now it's up to the doctors.
-
Hi Judy! About the bear-hugger blanket, I did the same as you--kept it off my legs. He wanted the temp kept high in my ICU room too, and I was way too hot--I am a 'compliant' patient so I was kind of wearing the warmer side-ways to be sure the flap stayed covered. My sister is a Nurse Practitioner who works with surgery and wounds and she kept telling me they use it to "Keep the baby warm". Hi bdavis--I sent u a PM, too. My BS is Rachel Dultz/PS is Matthew Lynch.
-
Sadie - Dr. Gimbel will take very good care of you - we've joked he has OCD, and that's exactly what you want in a PS. I assume you'll be at Magee? Who is your BS? My surgery was Jan 9, and I am pleased with my recovery so far (okay, I'm a little bored and impatient, but all things considered, I am pregressing very well). What questions can I answer for you?

-
Hi ship. Thanks so much for replying. My BS is Dr. Ahrendt and I really like her. I wanted to ask Dr. Gimbel though what his personal failure rate was on the flaps. She has seen only one in all her years, but I'd like to find out about him. But, I was very happy to get Dr. Gimbel. I'm a little freaked out about the ng tube (I didn't know about that) and the drains of course. Other than that, I think I'm at peace with the surgery. I had several abdominal surgeries before so I'm prepared for the pain of that and frankly, he's going to reduce me from a huge D to a perky C and I'm kinda liking the sound of that.
-
Well I am home today from my surgery. All did not go as expected. After being in the or for 14 hours they had to give up on the left side because it kept clotting and they couldnt figure out why. The right side did get done but it is SOOO small I dont even know if its an an A and I had Lots of belly fat. I have before pics and I will take after pics eventully and put them in the forum for whoever wants to see.. I am so disapponited and upset about it all. I dont even think I am going to get a flat stomach out of it because they said they didnt stretch it that much and it doesnt pull at all. I can stand straight I am just not suppossed too. Will talk more about it later when I am feeling better. Good luck everyone.
-
Tamyln - I'm so sorry. After you've recovered from this, what will be your next step?
Diagnosis: 12/22/2011, DCIS, 6cm+, Stage 0, Grade 2
