Lumpectomy /margins not clear
I have been diagnosed with 1.1 cm moderately differentiated. had a lumpectomy. one margin within .1 cm not clear. Other margins clear. No lymph node involvement. Surgeon wants to go in and remove more tissue. I am thinking of stopping further treatment. Thoughts????Experiences??

Until you have clear margins you do not know what your diagnosis is. When the breast cancer goes up to (or almost up to) the edge of the tissue that was removed, it is likely that more cancer remains in the breast adjacent to that point. Any remaining cancer could change the stage or other characteristics.

I had a re-excision after a lumpectomy with two close margins. I had a quick recovery -- no pain and no general anesthesia. It was a no-brainer for me.

With a 1mm margin, the risk of recurrence is very high. And in 50% of cases, by the time a recurrence is found and removed, the cancer cells have already evolved to become invasive. That changes the diagnosis from DCIS to IDC - and that can be a whole different ballgame.
It's possible that when you have the re-excision, no more cancer will be found. But it's just as possible that with the re-excision you would be saving yourself from a diagnosis of an invasive cancer.
As JSwan said, usually a re-excision is a quick and easy surgery. If it were me, there's no way I wouldn't have the re-excision.

True dat! I too had a re-excision. No biggie. Same incision, etc.
The next day I was driving the car and went out to dinner with a friend. Almost a non-event, in my experience.

I had one close margin. the margins were clear, but one was to small so I had a recision. It really was no problem. the doctor didn't really want to do it. He just wanted radiation to take care of it. The radiologist agreed with me. I would rather have more taken out then worry about it 5 years from now. So I had a second surgery.
You know what to expect now since you had the first one. I was wondering if I would be in pain afterwards since they were going back in, but I wasn't. To me, it's worth a shot of a second surgery.
I assume lumpectomies are much easier to recover from than MX's. I would never just leave it close. You would probably be worrying about it all the time.

I too had a re-excision of 2 margins that were not quite as generous as the surgeon would have liked them. It was an easier surgery than the 1st lumpectomy and she used the same incision. I even healed better after the 2nd go around. You don't want to have to worry about have more surgery later, especially if you are headed to rads.

I have 2 close margins of DCIS and will have more surgery. I just wish they would give me the date. Everything else depends on getting the surgery over with. I'm feeling cross, can you tell?

i am feeling cross too! My breast aches a lot. It has been two weeks since my surgery and we are waiting to find out the results of the onca dx before surgery in case I need chemo. ugh will find out tomorrow

I had 4 close margins, plus I ended up needing an SNB anyway. So we did another lumpectomy. Nodes were clear but still had one close margin, so had another reexcision. STILL another close margin. After 3 lumpectomies, we opted for mastectomy. Wouldn't have done it any other way. The 2nd reexcision (without lumpectomy) was very easy - no major pain, had surgery on Thursday was back at work Monday, no problem. If you leave the margin, who knows how much more is still there, for me, apparently there was lots and that's what DCIS likes to do - spread out in the ducts, not continuously though.
I agree with most of the other posters - get the reexcision.

HI there, please have the re-excision. My BS at a major top 50 cancer center, said about 25% of the time the need to re-excise. This is especially true for DCIS as the surgeon is working blindly. Unlike an invasive tumor which can be seen, DCIS cannot be seen and its often a re-excision is needed. Dont leave those unclean marigns. It is not worth the risk to leave it festering and growing.

Did you have breast pain after the first surgery? How long between the first and second?

thanks. I am going to have it. We are waiting for the results of the onca dx on monday to see if I need chemo. I don't want to have that either. i don't want any of this

I was assuming that because you had posted in the DCIS forum, that your diagnosis was DCIS. But if you are waiting for Oncotype test results to determine if you will need chemo, then your diagnosis is IDC - invasive cancer - and not DCIS. You may have some areas of DCIS in addition to the IDC but because the IDC is the more serious condition, your diagnosis, prognosis and treatment will be based on the IDC.
If you have IDC anywhere near your margin, or even if you only have DCIS near the margin but your tumor already included some IDC, then it would be very risky to not have the re-excision. You do not want to leave invasive cancer cells in your breast if it can be avoided.

Had lumpectomy in June, margins not clear in path report, Dec 14th was back in and opted for a MX.... Im good and clear now... Started rads Jan 12th.... have 4 weeks left...

My two surgeries were 8 days apart. I don't remember a lot of pain from the lumpectomy, but the SNB was a (rhymes with "witch").

HI Carm, My breast pain from surgery /lumpectomy was minimal. It was the nodes biospy that was very sore. Doctor explained that are hurts more post lumpectomy as with breast you are cutting through fat and tissue. With node biopsy you cut through muscle and must dig around to get the nodes out. It creates generally more pain than the lumpectomy. I had a wide excision and the pain was minimal. I only used tylenol.Good luck and keep us posted.

Low oncotype DX so only needed re-exision surgery. Not bad, short surgery but scary. Tomorrow I get results that margins are clear. then we will proceed with short radiation 2x a day for five days. Any experience with that?

I too had unclear margins on one side. At Northside in Atl where I go, the Breast Cancer Nurse Navigator told me it is more common than not to have this happen. Because when they are operating there really isn't anything there for them to see. No lump for example. They have to go by the calcifications and this isn't an exact science right now.
so I had the second surgery, went through the same site and it was less painful afterward than the first. They got it all this time in fact they didn't find any DCIS in any of the tissue second time around, so my Doc was very close the first time.
I would have the surgery if I was you, so you don't have to worry about it. Just a bump in the road and evidently very common. Good luck Hang in there.
Although I do have a question for others. Before I start radiation they my Doc wants me to have another Mammogram and I have talked to others that did not have to do this. Does anyone have a thought on this. I grateful the MM saved my life but not looking forward to having another so soon on a still sore left girl.

Hi, BKZOE. I go to Northside in Atlanta, too! If they do another mammogram post-surgery, they are very gentle. It is not nearly as "clamped down" as a regular mammogram.

I agree that the SNB and port placment where much more painful than the the first lumpectomy. Had my re-excision yesterday and it has been much easier, still taking pain meds though, and am extremely tired. But overall a much easier experience than the first

are you going to do radiation. They inserted a mammosite two days after my 2nd surgery and will start the 5 day 2x a day tomorrow.

Thankfulforlife. Tks for the reassurence about mammogram prior to radiation. Glad to hear they "go easy" on a girl.
. Just ready to start radiation. But understand I have 3 visits before that happens and the mammogram. And my job lol. I really respect those that have to do chemo too it's a lot to juggle. Keep the faith everyone.

Definitely have a re-excision if they want one. I had a lumpectomy on Jan 4 with DCIS too close to 2 margins. Had the re-excision on Jan 27. Path report yesterday showed a new spot of IDC with different markers from the first (slightly larger also) and DCIS at the margins again. Not visible on the mammogram. I saw BS today and we decided on a MX on 2/22. She's also ordered an MRI to see if the left one is just as sneaky. My DH said today he could see how people could find a small tumor and decide on a bmx.

I had 2 lumpies, pronounced clear and referred to the RO to start radiation. RO pulled a mamo and found more DCIS, on to a 3rd lumpie prior to the rads
but guess what, I am 3 1/2 years out and all clear, looking forward to ending my meds in a year or so
do all you need or are recommended to do is my suggestion, BS said at my start, if it comes back, it comes back nasty

BK: I had another mammo. before my rads started. It was RO's idea (not my BSs), she wanted to have a baseline before rads started and I suppose also to make sure one more time that everything was out . . .which I'm glad about.

Thankful4life -- I have been diagnosed with LCIS and I have Stage 1, Grade2 ILC, LN negative - lumpectomy came back with uncleared margins. Dr is suggesting BMX. Do you regret your decision?
