Okay, ladies, I considered starting a new topic entitled "How Dry I Am, " but let me introduce (har-dee-har) this non-sexual-but-related issue here. I have not had intercourse for quite a while and am menopausal, but I have never had a problem having a gynecological exam. The other day at my annual visit, my gyn could not get a speculum of any size into my vagina anywhere near my cervix for a PAP smear. She said it was too dry and the walls were thin and inflexible because of the Fasolodex.( Her attempts hurt like hell as well.) Fortunately I've had normal PAPs forever, so that's of minor concern. But what about the possibility of UTIs?
Of course I can't use anything estrogen-based to address this. Thoughts?

Tina, you can use Vagifem.....10 mg....2 x a week. While it is estrogen, it builds up the vaginal wall and the estrogen does not penetrate into your body. My gyn is very knowledgeable about this and I trust him. We also checked with the MO who agreed. You need a gyn who is up to date on this as it does seem to be counterintuitive ... but it's not. It has made a huge difference in my QOL.

Thanks ladies for all the good tips! Now that I got my all clear from surgery and pathology, I might actually think about having sex again and I'm sure I will need something. I was taken off all of my BHRT several months ago, so dryness and atrophy is setting in again!

I do not understand your docs saying no. What about the oncologist? Would he/she be willing to "consult" with some of our Oncologist's? I would be willing to ask mine to speak with them if it would help.

Jancie- Shrunk? is that possible? Thay may be it- i have always had a narrow opening and at my anuals they always had trouble getting the small speculum (duck thingy) in comfortably, so maybe it did LOL how do i unshrink it LOL
I think there should be a after cancer gyno field- so they know whats up with all of us! Like its just a big coincidence! Yeah right, something is def going on with a lot of women after treatment

Actually, I do see an oncological gynecologist now, ever since my ooph/hyst -- and it IS a good thing. He consults with my oncologist for any decisions regarding my care.

That's my next plan of attack - to ask my onc to write the script. If it comes to it, I'll take you up on your offer for sure! I'm desperate, angry, sad, and angry. (Did I mention I'm angry?)

"E"---If all else fails, perhaps having your dh consult with HIS doc to write you the script. I mean, MEN do look after their own. Knowing a man is in need, I bet you'd get what you need them in the way of scripts. Amazing that these docs are so ignorant to think they know all and are all.
I've decided after 2 years of celebacy, we are going to go this route and see how the cookies fall. I'm tired of having to wait and see if things improve, or having the gyno disagree with the onco or the onco with the gyno or primary. It's MY life and it's going to be MY way or the highway for them all, dangit~
And yes, moderators....please if do a little research and find enough support to list this on the 'Vaginal Dryness' forum, please do, b/c not ONE Of the things you listed helped me over the last few years.
Just sign me out as: tired of being cranky, in pain and frustrated with a medical field that can cure E.D. and all their problems, but we are supposed to 'lay down' and accept defeat~ Not anymore I SAY!!!!!!

I don't think any of the stage IV ladies object to posting in the stage IV forum. I think what we do resent (and forgive me for speaking for all) are, in no particular order:
1) Try this cottage cheese/coffee enema/insert your own words here cure!
2) Stay positive. Um, I think we all try to be as positive as we can, but it's kinda difficult, you know?
3) Gosh, I don't want to end up like you!
4) You're faking/stop whining/get a job.
There are probably others, but I think you get my drift. :-)
Welcome, wishiwere. This is a topic of importance to all! Oh, and there is no DH, just a boyfriend. I think if I hold myself hostage with an estrogen pill (You gimme the Scream Cream or I stick some estrogen up you-know-where!) the doctors might get how serious I am about this.

My pharmacist Moe is looking into the price of doing a batch of 500 grams for me (lol 500 nookies lordy lordy) He said the price will drop dramatically. At $140 for a 30 gram jar in Canada ...weeeell its a bit much, (but still worth it) Lol, he's called my home now 4 times.... even to ask if it worked... Also figured I would see what it was like on a day I just felt very dry in general (uncomfortably so) and it worked well. Heh heh didnt feel I had to jump in the sack, just a pleasant natural feeling.
Have to run.. hoping to get a noble pine tree tonight, almost thought I wasn't going to bother for the first time ever! But changed my mind.

enjoyous that was hilarious- so true though, no amount of coffee enimas are gonna cure us LOL But i am gonna swear by my almond chocolate ice cream shakes, (thats my excuse for eating them often- its my cure LOL)
And yes this is a subject that knows no stage boundries so all are welcome if they adhear to the rules of engagement on this forum

Okay... I got a prescription from my gyn-onc, and I think she wrote it worng. The "recipe" says something about a "transdermal vehicle" which I assume refers to the cream base. My doc's spacey PA wrote "transdermal patch."
Do you think I can get it filled anyway? Just bring the printout with me and explain to the pharmacist that it's a typo in the Rx?

I would think so Jeskachi, but take it in at a time when they can call your Doctors office just in case they want it confirmed. Mine was written out wrong as well, but it was the internal/external part (got the SC & lidocaine backwards) so it wasnt an issue at all. Hope it all goes smoothly, seems lots of us have hit bumps getting it filled
Guess its because its not your standard RX.

No, apple, I think it's actually an enema. The cottage cheese frees up Spirit of the Dairy Cow and the coffee Wakes It Up. Or something like that.
Tell your daughter that sex only hurts if you have numbskulls for doctors! I screen-shotted mine repeatedly; surprisingly enough it didn't work. Go figure.

hrf, Is Vagifem different than Premarin Cream? I got an RX for Premarin from my GP, and used it once, but then chickened out. My GP called my Onc and asked his opinion, and the Onc was not happy with the idea at all. It is so frustrating, and I am getting desperate. I have tried Replens, KY, Astroglide, Coconut Oil, etc. etc. Nothing seems to work, and it seems like it just keeps getting worse and worse. I don't want to use something that will counteract the Femara (Don't want to have these stiff joints and other SE's for nothing!), but there has to be SOMETHING out there! I see my Onc in January and am getting up the nerve to ask for the SC and Lidocaine, but just wondering about the Vagifem.........

I do think they are totally different products. The Vagifem is a little tablet that is inserted vaginally and it is estrogen. My gyn and onc did confer (thourgh me). From the arimidex, I was in so much pain on a daily basis that it was awful. Gyn told me that the vaginal tissue was badly being destroyed which explained the pain. He said not unusual for Arimidex to do that. He does work at the the same hospital/cancer centre as my med onc. He said Vagifem builds up the vaginal wall and once it's built up the estrogen does not permeate the vaginal wall and stays local so that there is no worry about it causing problems for ES+ breast cancer. The key is keeping the vaginal wall nice and thick. He said there are med onc who don't understand this and ask patients to lower the dose (mine did as well) - he said this is not good because then the vaginal tissue thins out again and the estrogen can seep out. There is a very low dose of Vagifem - i think it's 10 mg only 2x week ....and it does the job. From a QOL issue, I couldn't go on without some help. This has worked for me. I think the SC serves a more pleasurable purpose so you might want to give it a try as well. My gyn has always worked in research and is on staff at the medical school as well so I do trust him to be knowledgeable.

HRF, My Gyn Onc put me on vagifem in spring of 08, for the same reason the v walls were thin. Unfortunately my body over reacted to it.. I felt so overly stimulated for days that it was uncomfortable. It might have gone away if I had continued to use it, but I didnt like the feeling so never got the nerve up to try it again. Could be I was overly sensitive to it?? Lol, I just looked in my drug drawer and its still there. They were 25MCG to be used twice a week. Maybe time to clear out all the old meds....
I could be wrong but I think SC is working internally at helping to thicken up the lining as well. Im allowed to use it every day so Ive been using a dab here & there during the day when I feel really dry internally. It feels better quite quickly... the Gyn Onc had also said stimulation or orgasms will naturally help it.... maybe its the use it or lose it??
Linda two - Yuck on all the astroglide, KY, replens and everything else thats just messy. I didnt find any of it to help. Its all just very slippery and some I found irritating as well. On the SC & lido, if you want to ask your Onc (& are uncomfortable) just print it out along with a few posts on how to use it. Our doctors have heard it all... most of the time its us who are more embarassed. But we shouldnt be, men ask for viagra, why cant we ask for SC if we think it will help us.

My gyn told me not to use the Replens as he said it would irritate the thin vaginal lining. Suz, they have recently changed the dosage of the Vagifem to only 10 mg twice a week. It's a new dosage they have come up with. But if it feels strange, then I get it. My tissue was crumbling and I was in a lot of pain so I needed something. Like I said, had nothing to do with sex or libido or anything .... my body was falling apart. thank goodness it is working for me without an issues. I can't comment on whether or not it would make any difference for sex. I thought the SC was more like a viagra for women....which seems to be great for those using it.

Hrf - She put me on it because the menopause hit me hard between Chemo & Oopherectomy. It wasnt just for intimate reasons although thats why I had the appointment. It was just plain old painful & so dry. She explained the reasons behind the pain, one of being the thinning of the lining. Seeing the dose change that you've mentioned I imagine it would be much less intense... she wanted to work me up to the ring thats just left inside for a period of time (cant remember how long) I appreciate the info on dosage. It might be enough for me to consider trying it again. Lol, I could just break the pill which is quite big into three.
I dont find the SC to be like viagra... It feels really natural. Its more like a light stimulation that gets your body to react the way its supposed to. Hard to explain, I'm guessing because it brings blood flow to the area, your bodies natural response seems to kick in. It doesnt feel like a strong stimulant but in the right situation... well it really works. When I used it in a non sexual situation it just seems to get rid of that dry feeling inside.
I should ask Moe the pharmasist if he would write out what exactly each ingredient does.. he did tell me but I didnt write it down (was just happy I got it)
