Nordstrom's lingerie dept. They have relationships with some insurance companies, and they'll file a claim on your behalf.

In the Boston area, Lady Grace stores are pretty good (though it depends on the fitters in different stores). I've had really positive experiences at the Friend's Boutique at Dana Farber (anyone can use it).

In NJ- sort of central jersey: Bra and Girdle Factory in Lakewood.

I highly recommend Ocean of Hope in Toms River! My experience with Karen has always been very positive, unlike my experiences with theBra and Girdle Factory. The B&G factory tried to get me to buy what they had in stock, not what I wanted. Also tried to sell me the "grandma bras". Karen at Oceans of Hope will work with you to find what really makes you happy. She will order stuff for you to try until you find what you like.
That being said, different strokes for different folks. -
In Los Gatos, CA (San Jose, Santa Clara County area) The Next Step Boutique, Los Gatos- Almaden Rd, at National Ave. Excellent staff, beautiful shop.

Alliton's Pharmacy in Port Jervis, NY has a wonderful fitter. I didn't go through them because they didn't take me insurance. Even still, the fitter was unbelievably helpful answering my questions and was willing to continue to help even when we determined that I wouldn't be getting anything through them.
Medi-Fair in Middletown and Monticello, NY is okay for a fitter and products.
