Implants vs. DIEPA

Just left plastic surgeon. Thought I was going to get implants go up a cup size, what the heck might as well get something outta the BC deal, he says I would be a good candiate for Deep Inferior Epigastric Artery Flap. Essentially like the TRAM but supposed to be less recovery time and better for your body. They take tissue and fat but no muscle. Good for the tummy as some fat will be gone, like a mini tummy tuck. Then if I want to go bigger I can get implants after DIEP surgery. Has anyone had the surgery done before?  Was told the implant could harden if I choose that route because of the radiation. Advise, opinions helpful.  I love this site, I have been on a roller coaster ride since dx in March this year. BC stinks  but we can all beat it!!

    I had a saline implant.   It worked out very well for me.   Feels firm but kinda natural.  I also had a breast reduction/lift on the other side.   I had DDD now I'm a C-D and very happy.  I have had 2 friends that had the Tram and they were in alot of pain during recovery and had issues and had to have other surgeries.   Good luck with your decision  PS:  I didnt get my implant until 8 weeks after radiation and only missed 3 days of work.

    ckgrayoh - Please also ask your PS to explain all the possible problems you might encounter if you decide to go with implants and are having to have radiation. 

    I lost my implant due complications with radiation. My docs confirmed that it was the radiation that caused the problem, so it's not just my opinion.  Further confirmation that the rads were the culprit is that I had a bilateral mast. and the side that didn't have radiation is just fine.  Great, now I have one perky boob, LOL!

    My PS tried to get me to choose a different reconstruction method before he would even do implants.  I was very stubborn & had already made up my mind that if I was going to have reconstruction, that was the only type I wanted.  Still, with the details from my doc, I went into with my eyes wide open.

    Please don't let this post make you think you should NOT choose implants.  There are MANY who did and did not have any, or many, complications even though they had to have rads.

    Good luck, hon!  There are so many decisions to make at the beginning.

    thanks for the replies. I finished radiation on November 17th this year.  I may take my chances with the implants but I don't want to have to go thru another surgery if they fail or get hard.  My PS will do whatever I want him to do. He said if implants don't work then we can do the DIEP. I am not sure, I just want my girls back so I can feel normal again and keep kicking ass!!

    I am going to get a second opinion from another PS. The guy I was seeing has only done this surgery one time and that was on a cadaver! He is good with a good rep BUT I would rather have someone with experience. I just don't know about that type of surgery. 12 week recovery time, cant really walk until after 2 weeks. My husband works during the day and while I do have family and friends to help I just think about how I felt after first mastectomy and node removal and it was awful!! I am going to keep doing reserach and talking to ppl who have done surgery and had implants and see.  There is also a $$ issue to think about, 4,000 dollars vs 12,000 dollars. Yikes!!

    I had DIEP and was really feeling pretty good after a week. Post you question on the diep2011 thread and you will get a lot of replies and advice from women who had the sx this year. There are several on the thread that had implants that failed leading to the diep. I may be wrong, buti do not think insurance would cover implants after diep because you wanted to go larger. I don't know though. Good luck with whatever you choose.

    Just adding my 2 cents.

    I had a 'one-step implant' after my MX, a saline implant that could have stayed in place. I also had rads.  My PS said as many as 50% of implant procedures fail with rads, but I wanted to try anyway.  I had very little reaction to rads.  It did push the impant/expander up higher on my chest, but no problems at all.

    2 months after rads, I had exchange surgery for a saline implant.  I am thrilled with the result and have had NO issues. I went in a D-cup and remain so (since I still have one natural breast I wasn't going to change the size!). Silicone is definitely softer than saline, but it's a personal decision, I know.

    My PS also said, you can always get even better results with DIEP or TRAM flap (you have to have enought abdominal fat for the DIEP), but I just wasn't ready for more invasive surgery. Who knows, someday. The option is still open to me. Good luck!

    I had an implant and got an infection so had to have it removed, a few months later chose the TRAM surgery, I am very pleased but I wont lie its a tough surgery and long recovery, Its been just over a year and it looks amazing, I would do it again and not even mess with the implant, however I did have an implant placed on the right side to even out the two, they both look amazing and Im feeling great... I would recommend getting a second opinion and going to the best reconstruction surgeon you can find, it makes all the difference, good luck

    I had a lump and rads.  Because of my history I am still at a high risk for getting a local recurrence so will be having a BMX done in March. I had problems during rads and problems with my pec muscle after rads.  I am currently in PT for that and it has greatly improved..  I am too thin for diep so my PS and I have decided that I am going to try the implants.  He said despite my problems with rads my skin looks really good. He wants me to be one year post rads before we do the surgery which would be the end of Jan and then my boss is pretty much gone until mid March so I am waiting until then.  Also he wants me to finish with PT because of my pec muscle . He said that would be even better with my rads side.  We discussed that even if it fails I will still have other options of flaps like Latisimus Dorsil or gain weight for diep. But he stressed lets not worry about that because we are going to do everything we can to make it take.  He discussed that if my healing is slow he will have me do hyperbaric treatments. If my pec muscle bothers me again he will have me back in PT.  I sought out 3 PS opinions and several BS opinions and he is the only one that said he thinks he can save my nipple and if not he can recreate it and make it match.  The other PS that I liked said she could not save it and could not make it match.

    Get another one or two opinions and get all the information you can so you can make the right choice for you.

    Thanks for all of the comments. I am going to get a 2nd opinion at end of this month. I have stomach fat but probably only enough to keep size I am now. Boo... I will decide after I speak with next PS.  The one that I just saw said if I want bigger boobs to get fat! Well that is just a license to eat whatever I want I guess BUT I am cutting out sweets and fattening food to help avoid a reoccurence. Figures the one time you get a free pass to just eat everything and know that it will go away....mmmm.. oh well.

    ckgrayoh-that was me too just enough fat to make me the size I am which is not very big.  I worked hard at changing my diet and did not want to go back and pick up bad habits again.  I will if it is my only option but for now I have other options to choose from.

    Really go to the DIEP 2011 thread. I had Diep and was skiing in 5 weeks post DIEP. The key here is to have a very skilled surgical team to do the procedure. I had radiation and I had lots of scar tissue form from radiation. I needed to have all the skin removed and replaced with my ab skin. Feels normal, looks normal acts normal. Well worth the effort.
