Breast Cancer Thrivers Cruise Teams With Breast Cancer Wellness to make Thriver Cruise Scholarships Available

SCHOLARSHIP DETAILS
Breast Cancer Wellness will be providing a shared inside berth and travel accommodations up to a maximum of $1000 in the recipient’s name, purchased through Great Southern Travel. In addition, those traveling with scholarship recipient must purchase their berth through Julie Wilson with Great Southern Travel jwilson@ greatsoutherntravel.com or 1-800-325-4792
After the berth is purchased in the recipients name, any remaining funds of the maximum $1000 will be applied toward recipient’s airfare or travel accommodations. There are no cash allowances, or rebates, or credit of any kind if the berth and travel portions do not meet the $1000. As well, if the berth and travel expenses exceed $1000, the recipient is fully responsible for all costs exceeding this limitation.
This scholarship does not provide any air or ground transportation to the port of embarkation, taxes, government fees, port charges and fees. Shore excursions, gratuities, bar and beverage charges or any personal items are the full responsibility of the recipient. This scholarship is nontransferable and non-redeemable. There is no redeemable cash value for this scholarship.
By applying for the scholarship, the applicant agrees to these terms as well as use of their names and their photos for promotional purposes without receiving any additional compensation. Each will also be asked to participate in a short before and after video of their experience.
Federal, State and Local tax consequences are the sole responsibility of the recipient. Recipient are advised to contact their CPA for information about all tax liabilities.
SCHOLARSHIP REQUIREMENTS
Breast Cancer Thrivers Scholarship is open to any stage IV breast cancer survivor/thriver in the United States who is 18 years of age or older except employees, associates, selection committee members or family members of the Breast Cancer Wellness organization, Carnival Cruise Lines, and Great Southern Travel and sponsors of this cruise.
Scholarship awardee must attend all Breast Cancer Thrivers Cruise workshops offered by Breast Cancer Wellness. Recipient must provide an affidavit from their health care professional that she is physically able to attend the cruise.
Passports are the responsibility of the recipient and are required for each scholarship awardee. Passports applications are available online http://travel.state.gov/passport/passport_1738.html. Allow a minimum of 8 weeks for processing.

SCHOLARSHIP SELECTION PROCESS
Scholarship Application must be received by mail or email by December 15, 2011 to be considered for the Breast Cancer Thrivers Cruise. Selections will be made by the Thrivers Scholarship Committee and awardees will be notified by January 15, 2012 by phone.
