jenkwa2:  There are a number of threads with comments on this.  Type "nipple sparing" or "nipple sensation" into the search box at the top of the page to find....

My breast surgeon does lots of nipple sparing procedures so I recently emailed her to ask what her observations are based on long-term patient follow-ups.  She commented that about 50% of her patients report a return of some feeling in the nipple.  However, she describes it as more of an awareness (such as clothes rubbing) rather than pleasurable sensation.  She makes the incision below the breast in the inframammary fold.

I've posed this question a number of times on various threads and the few who responded that they do have sensation concur with my BS's observations.  Some actually describe it as an uncomfortable sensation and I've never read or heard of anyone who has the same sensation as prior to mastectomy.

There are claims that the new microfat grafting procedures can regenerate feeling.  I'm obviously skeptical, but it sure sounds intriguing!!!

Yes, I did. I am not sure.  My nipples have sensation, reaction.... more their than in rest of the breast, but I am not sure. I had immediate one step direct implants.

    hope this helps

Ilovechocolate:  Is it a "feels good" sensation or just a sensation?  As you say, "I'm not sure..." sounds like it's not like pre-mastectomy....

@hmom - I know it has been a while since your post, but I am now interviewing surgeons to perform a NSM on me. I am meeting with a second GS at Abbott Northwestern tomorrow and am wondering who you had as a surgeon, if you were happy with the results and if you would use them again?  Also, who was your PS?  I have met with 3 so far, really liked one of them, but would like to talk to people who have had good results.  Thanks!

Orange, California: Dr. Helen Mabry did my NSM. She is fabulous! I also had immediate recon with DIEP. It's been two weeks, and so far so good. I am still healing, but I can feel my skin and have sensations. My breasts feel like I did while nursing. It's like my milk has come in.

I feel so blessed to have had the option brought to my attention from Dr. Swistel at our first consult.  He is trying to do as many NSS as possible depending on diagnosis and his work was like art in my opinion.  Scar/incision is under breast fold.

    NYC:

BS - Dr. Alexander Swistel - Cornell-Weill / Columbia Presbyterian working with either Dr. Mia Talmor or PS - Dr. Robert Grant - Chief of PS, Columbia Presbyterian - one of the nicest, most compassionate doctors I have ever met.

I am one of the "rare" after NSSMx patients that found unexpected cancer cells in one of the nips.  Even so, I am going to have the nip cored out in the PS office and still get to keep the areola and the other nip.  So much better than not having had the opportunity.

There are several Dr's in the San Antonio Tx area that perform NSM.  My PS Dr. Chet Nastala will be performing mine and then doing my deip reconstruction.

I must admit, I am a little concerned as I didn't realize this was so rare.  My surgeon didn't seem to have any concern about doing it at all.  I'll let you know how it turns out in the end.

Dr. Jeannie Shen in Pasadena CA is amazing, both at surgery and just being knowledgeable and compassionate. She saved my nipple and it was a very challenging case. I couldn't recommend her more!

I am 3 weeks post surgery and my nipples are pink, I have completely filled implants and I had the absolutely best BS/PS team.  I did my research and they are not in the same practice but they have worked together many times.

Boca Raton, Florida at Boca Raton Regional Hospital

    Dr Joseph Colletta - BS

    Dr Hilton Becker - PS

At one of my last appts the PS commented on what an excellent job the BS did on the masectomy.  I quote "that's how a masectomy should be done."

I had a Skin-sparing, Nipple Sparing Mastectomy on my right breast Aug 7, 2007.  It was done by Richard Clarfeld, MD at Overlake Hospital Medical Center in Bellevue, Wa.  I was originally scheduled for a lumpectomy, to be followed by radiation therapy -- but when I discussed this with the Plastic Surgeon, Dr. Clarfeld recommended (Dr. Kimberly Lu) -- she said she'd have a much harder time doing the reconstruction with a lumpectomy than a mastectomy due to the space between the two tumors found.  I elected to have the mastectomy -- which then elimated the need for follow-up radiation therapy also.

I remember asking Dr. Clarfeld what I would end up looking like, until the reconstruction -- he truthfully said " A Shar-Pei".  That sense of quirkly humor helped alot!

olearca, I went for a second opinion with Dr Swistel. He recommended the same exact procedure as what I had with my local doctors in NJ (who I liked better, btw), so I stayed with Dr Klein. Glad to hear you were so satisfied with him.

Dr. Michael Kinney of Advanced Breast Care at Northwest Community Hospital in Arlington Heights, Illinois (a northwest suburb of Chicago-close to Ohare Airport). He is brilliant, kind, and caring! He did skin sparing/nipple sparing surgery for me and works closely with an equally brilliant and caring Plastic Surgeon Dr. Stephen Madry.

Gotta give a holla out to my guy, Dr. Garrett Peel, my Lisa and the rest of the wonderful staff.  Our fairly modest town is VERY lucky to have him locally.  Google "Previty Clinic."

Northern Virginia - Dr. Cynthia Dougherty or John Williams...  Was told they only do the NSM/SSM procedures absent a medical reason not to!!!!!  Didn't realize this wasn't the common standard since it is theirs. Really think it makes a huge difference to still look like me.

Just came across this and thought I would add Mills Peninsula in San Mateo, CA.  I was offered nipple sparring, but unfortunately due to prior breast aug/lift the PS said the nipple was getting it's blood supply from the actual breast tissue so I had skin sparring UMX with direct latissimus flap/implant reconstruction.

Anyway, it was offered without me asking, which is great since at the time I didn't even know such things were possible.

exbrnxgrl - Hi, is this recent information?  When I saw my BS in San Francisco a few weeks ago (Sutter, not Kaiser) she sounded very frustrated that NSM is still considered "experimental" in so many parts of the country.  In fact, it is her understanding that Kaiser is currently NOT approving NSM.  Hopefully, that's a misunderstanding and not actually the case......

Yes, very recent! I had my NSM/SS bmx done at Kaiser Santa Clara on Sept. 7,2011. It was also a one step (straight to silicone implants) and I am pleased with the results. I did have one nipple removed several weeks later as path report showed DCIS too close to nipple, however my ps was able to maintain the implant and they look great. I do not think there is a system wide Kaiser policy regarding this. I think it just depends on the ps.

exbrnxgrl - I'm sorry they had to go back and take one nipple Cry.  Sounds like everything else went well for you surgery-wise, that's great!  And.....great to hear that Kaiser doesn't have a blanket "No NSM" policy.  My BS apparently was mistaken about that.  She's a great proponent of NSM so I'll be sure to let her know that at least they're being done at Kaiser Santa Clara.

    Best wishes!
