BMX and radiation

Has anyone out there had a BMX and was told they needed radition?  I am seeing a radiation oncologist in Thursday, I think because of the narrow margin I had.   I will fight to not have radiation but of course will go ahead if convinced that it is absolutely necessary.  My BS doesn't think I do, but he is the one that referred me to the RO.  He says it is "controversial".

Anyone have any thoughts or experience with this?   Oh, I did have a very little bit of microinvasion but as far as I can tell....that was not where the narrow margin is.

    Thanks!

I had a BMX in April for bilat BC & radiation is an option for me. The only reason is my positive nodes. By having the BMX, (on one side I could have gone lumpectomy), I was hoping to avoid radiation. Not sure how your "margins" factor in.

I did.  I had several margins that were less than 1mm.  My bs felt strongly that it would come back if I didn't do rads and the rad onc agreed.  I did fine with it and it didn't mess up my recon at all....just pushed everything back.  I was very tired at the end and for several months after.  I just didn't want to risk another go with cancer.

Hi - I had a BMX, my MO and BS said no radiation.  In reading my pathology report one of my margins was 1.5mm, i was concerned this wasn't enough of a clear margin.  Some centers say 1mm is 'clear' and others want 2mm.  I referred myself to a radiation oncologist and she said she is more comfortable with 3mm margins, and since I had multifocal disease and my young age (44) she recommended rads.  (my nodes were negative).  I just finished 6 weeks of radiation with no fatigue and very minimal skin effects.  Maybe because I didn't have reconstruction it was easier? not sure.  Really the hardest part was finding time to get the treatments (I was lucky, I know some have terrible skin reactions)

Saw the radiation oncologist this morning.   He recommends 6 weeks of radiation.   I had everything going for me to not have it except for a less than 1 mm margin.   He said that I currently have a 15% risk of reoccurence and that if I did the radiation that would drop to less than 5%.   I guess part of my issue is my DCIS was a grade 3 and my microinvations are HER2+.   I don't know.   Feeling very depressed about it today.   I need to just get over it and do everything in my power to make sure this beast does not come back again.   This will be affecting my reconstruction.   RO indicated that I would need to get my TEs expanded very quickly before radiation so that my skin would be stretched out before any damage from the radiation.

Yes, you do need to finish your fills before you start rads.  My ps had to 'sign off' that I was ready to start rads.  I didn't want to be 'big' so I only had about 3 fills with 50cc's each.  The rad treatments were really not that big of a deal.  It's very quick.  Heck, there were times I had to take my 4 kids with me!  The people were very nice and made it as pleasant as possible.  I had been given "at least 35% chance of reoccurence without rads and around 4% with".  You can do it!

mom3band1g - Did you have any problems or complications from the radiation?  I'm mostly worried about infection problems and I  guess...the firmness problem of the skin/muscle on the radiated side.   My risk right now is about 15% which would be reduced to less than 5% with the radiation.  Not as big of risk as you had but still 15% is too much!

julianna51- sorry ...we were at the beach!  I had no immediate problems with radiation.  I did end up needing PT because of muscle cording issues.  They seemed to peak about 3 months after radiation.  My rads side is def more 'firm' than my other side.  My skin did great!  I ended up being allergic to the glue in the stickers and have faint scars from that.  I exercised through my entire 6 weeks of treatment and I think that helped a lot.  All in all it really wasn't horrible.  My kids were young and I hated leaving them everyday.  I think the emotional toll when all was said and done was worse than the physical.  Don't be afraid to ask for PT!

Hey Julianna51: I wrote you in another post, I have one are with a margin of 1mm and another with 2mm, the rest were fine.  I don't know what to do, I think I'm afraid to pursue it. And how soon after surgery do we need to do it if we are going to do it?

I think 1mm is Ok...it's when it's under 1mm that they rec rads.  I could be wrong on this.  What does your surgeon say?  Have you met with a rad onc?

Can you ask your BS to bring your case before the hospital's tumor board?  I had a narrow margin of less than 1 mm--it was close to the skin. The BS wanted to re-excise at the time of my implant exchange to avoid radiation. The tumor board has ROs as well as medical oncologists present who all weigh in and look at your path slides. I also sought a second opinion from a  nearby teaching hospital and the oncologist there also brought my case before his board.  I think if my margins had been deep in the chest wall, the answer may have been different. In the end we did a re-excision-- my scar line is very long-- but no extra DCIS was found.  I was worried about doing radiation because of autoimmune issues. I hope I made the right choice, but it's very important to find out exactly which margins are in question and to get another opinion.--Julie

mom3band1g: I didn't ask him what was the "ok" margins, at the time he told me, it caught me off gaurd and I couldn't think of any questions.  I have since made a list. :)

JAT and mom3band1g: I didn't meet with a radiation oncologist yet, I think I'm afraid to, and he has already discussed it with the board of docs, and they feel it isn't necessary for radiation, but the margins are in the chest wall against the muscle, so I just don't know.

I am 2 weeks out from my BMX/reconstruction  and my pathology showed Positive Margins (2).

I Spoke W BS and RO and they both agree I need radiation.

I am from Atlanta as well and I was very concerend about the radiation.

I would love to hear about your radiation experience and give me advice and let me know which RO you chose as I am interviewing them this week.

    Hi there,

I had a BMX and radiation.  My tumor was very close to my chest wall.  Buy emu oil.  I used that and did great the whole time - just turned a bit pink, but no burning.
