I did prophylactic NSM (with TEs) on 12-14 in NYC....excellent experience so far (back to work on week 3, no complications thus far)...

BS: Dr. Alexander Swistel from Cornell Well

PS: is Dr. Mark Sultan from Roosevelt Hospital

Since they are both from different hospitals, they agreed to operate out of Roosevelt. I believe Dr. Swistel often works with Mia Talmor who is a part of Cornell Weill. A friend of mine used the Swistel/Talmor team and was very pleased as well.

I am insane about both of my docs, and so far all looks really good.

In the Dallas, Tx. area, I would highly recommed Dr. Lynn Canavan (BS) and Dr. Richard Ha (PS)They do not office together, but both have offices in McKinney and Dallas (Canavan) and Plano and Dallas (Ha.)

The BS that the radiologist recommended said that he wouldn't do NSM because of recurrence, etc.  I did a little research on my own, and found Dr. Canavan.  She is very active in BC research, and tries to save the nipple whenever she can.  Luckily, my DCIS was still at stage 0 and my margins were clear, so my odds look good.

I could not recommend these Dr.s and their staffs more highly.

I think back, and if I had seen the Dr. that was first recommended, I would have lost my nipple, and it wouldn't have been necessary.

thank you ladies and keep posting your successes.

Sue4lsu, it would seem so aggressive that with your stage 0 ductal dx, to lose the nipple when there are procedures and skilled surgeons available to save it.  With your ducts and tissue removed, no pathway for ductal cancer to reach the nipple.  One's opinion would be that there is skin and breast tissue remaining close to the nipple, the arguement would be that we are skin sparing the breast anyway so doesn't make sense to take the nipple if your saving skin.  Glad you were persistent.

I had a bilateral skin sparing, nipple sparing procedure done at Johns Hopkins in Baltimore in November of 2010.   Surgeon said that as long as the tumor was at least 2 cm away from the center of the nipple, he would try and save it.  He did a great job - both nipples survived the surgery.  BTW, he is the only surgeon there that will do it.  At least, that is what I was told back in November.  It may have changed by now.

Just found this thread so I thought I'd add my info.  I'm having NSBXM at California Pacific Medical Center (CPMC) in San Francisco.  Dr. Nima Grissom is my BS and Dr. Gabriel Kind is my PS.  My surgery hasn't taken place yet, but they are both highly recommended by my local BS and throughout this pre-surgical process I have heard repeatedly that I am in the best of hands.

    Wish me luck on Wednesday!

    Diagnosis:  12/8/10, Left ILC with LCIS, 5mm, Grade 1, ER+/PR+, HER2 -

Diagnosis:  12/20/10, Left DCIS, <1cm, Intermediate grade, ER+/PR+

Thinking of you and wishing you the very best.

    Hugs.

    Hi y'all:

Here is a recommendation for the Northwestern US: I had a NSM done on one side a week ago in Vancouver, WA. (Near Portland OR also). Both my cancer surgeon and plastic surgeon are wonderful. Private Message me for names. The PS uses botox at the time of surgery to relax the pec muscle and make it less likely to spasm. I had one fill at time of  surgery and another 2 days ago. So far, so good. Lucky60

I had the first skin & nipple sparing bilateral mastectomy done in South Bend, Indiana at Memorial Hospital in Feb '09.  I had immediate reconstruction with silicone implants, & I'm very pleased with the result. My breast surgeon was Dr. Michael Rotkis & plastic surgeon was Dr. Stephen Zucker.

Janimal, I don't know if you're still around here, but Dr Melinda Molin in Portland ME does this procedure. She was the only BS I asked, so either I got really lucky, or a lot more do it than we know. She's an amazing Dr all around. I've even found a PS who does fat gratfting with BRAVA. I didn't have to search high and low. The PS is my 2nd opinion guy. I happened to mention fat grafting on the phone with the office, and they brought up BRAVA. I'm still deciding what to do, so I cannot offer experienced advice on the procedure.

I'm not sure if you live near the woods, or if you're referring to the entire state as backwoods. If the latter, that's not been my experience at all. Keep asking around.

I have a question for those who have had this procedure. I hope it's OK to ask here. If you had implants, are you happy with them? The first PS I saw said I would not look natural naked. He also has to make serious cuts above and around the nipple. I'm going for a 2nd opinion, because I feel like I should be able to expect better. But perhaps recon really is that limited?

    Thank you for your help.

Crescemt5 - If you PM Timtam, Lilah or whippetmom they can give you access to a picture forum where you can see some photos for yourself.  I have been very happy with my results, my scars are underneath the breasts.  Hugs, Valerie

Crescent5:  I am only at the TE stage of reconstruction, but my incisions are also below the breasts.  I guess the incision site is determined not only by the location of your tumor(s) but the technique preference of your breast surgeon and plastic surgeon.  So far I'm very encouraged, but I don't actually have my implants yet...

    Crescent,

I had NS/BMX on March 18, 2011 - four weeks ago tomorrow. I hava implants and Alloderm was also used to help form a scaffold of sorts for the implants. I am still very near the time I had surgery, but at this point, I can say I am very happy I decided to have implants. My procedure was a PMX and this may make a difference in how well implants work for their success could have something to do with why they are being used. This would be a question for a medical person.

I am - at this point in time - thrilled with the way the left one looks! The right one has a mind of it's own and it taking a bit more time to "drop and fluff" but my PS assures me it will. It looks a bit different than the other breast but unless I told someone, it is unlikely they would notice this.

My husband thinks I look wonderful and he is brutally honest to a fault. I also think I look pretty natural naked and actually think I look far better than I did before the surgery for the new girls are more perky.

If I can answer anymore questions, please feel free to send me a PM and I will answer anyting I can answer.

    Hugs

    Sandy

    Melanie,

When is your surgery scheduled and what is scheduled? I am sorry but I do not know anyone who does NSMX in Canada. I have heard of a Dr. Narod somewhere in Toronto but don't recall if he did NS or not. For some reason, it seems he may have. I do not know his first name but perhaps it was Steven or Stephen. You might google the name and see what you can find.

If you do not locate him, let me know and I will look for it again for you. In what province do you live?

Juat so you know, new members are only permitted a very few - I think it is 5 - posts per 24 hour period but may send and receive unlimited PM's!

You can send me a PM if you prefer and feel free to ask any questions you would like!

    Hugs

For those of you in the Philadelphia area, University of Pennsylvania hospital, and Fox Chase Cancer Center both offer those reconstructive procedures.  I am meeting with a PS from Fox Chase next week.  I want to add that although I really like and respect my surgeon who is with a local community hospital, he did not mention the availability of this type of reconstruction when we met to discuss surgery and reconstruction options.  I don't know why this information isn't widely available to women facing mastectomy. The reconstruction portion of the cancer equation is certainly just as important to us as the treatment side.

     BS- Shawna Willey

    PS- Scott Spear

I so highly recommend them both and would be happy to talk with anyone about my experiences.
